Subject: [PATCH] Rename the agent instruction files CLAUDE.md to AGENTS.md

AGENTS.md is the one instruction file every coding agent reads. Rename the
root and swapr/ files, say in their intros that they are for all coding
agents, and point the two documentation lists and one test comment at the
new name.
