Subject: [PATCH] chore(release): prepare 0.1.6

The VS Code Marketplace rejected Busy Octopus 0.1.5 because its icon
exceeds the new size limit, so 0.1.5 reached npm but not the
Marketplace. The icon fix is ready for a versioned release.

Prepare version 0.1.6 for candidate review and publication.

  - Bump the synchronized package and extension version to 0.1.6.
  - Record the icon fix in the versioned changelog.
