Drop decision URL requirement

I wonder if we should consider dropping the Decision URL requirement? As per the new/evolving process, it's pretty much an expectation that we will auto-publish all documents.

Maybe we can start baking auto-publish into charters instead?

This requirement comes from the W3C Process. All publications must be done with "record the group’s decision to request publication". The fact is such decision could be a blanket decision, so it's fine to reuse the same decision for all publications coming from a group.
