Summary

• Added troubleshooting guidance for migration verification failures in CI.
• Documented how to reproduce verification using a disposable PostgreSQL database.
• Explained connection failures, schema drift, unapplied migrations, and migration-history mismatches.
• Clarified the difference between static migration checks and database-backed verification.

Scope

This is a documentation-only improvement related to #437. The existing db:verify script and migration verification CI job were already present.

Testing

Documentation-only change; database and CI simulation tests were not executed.
