feat!: drop the review panel; which host reviews is the orchestrator's call

Summary

The plugin stops naming hosts for review. /sdd-review --panel, the panel lines at the end of /sdd-deliver and agents.review_panel are removed. A second pass in any host that loads the plugin is /sdd-review --pr N. A reviewer that does not load the plugin gets the review request in docs/ai-workflow.md § Review.
