Testing

• Extended tests/git-diagnostic.test.ts, tests/rust-diagnostics.test.ts, and tests/docker-diagnostic.test.ts.
• Added tests/exit-code-reference.test.ts which parses docs/reference/exit-codes.md and compares it to the source constants.
• Run npm test (Vitest) locally before review.

Security

• Fixtures assert that raw stderr/secrets are never reflected in diagnostic output.
• No production behavior or shell fallback is changed.
