fix: normalize shared diagnostic timeout classification

Overview

This PR standardizes how timeout outcomes are classified across the external-tool diagnostics, so that Docker, Git, and Rust diagnostics report timeouts consistently without changing the public doctor result model. Timeout output is bounded and actionable, no raw subprocess output or secrets are reflected, and no shell fallback is introduced.
