What changed and why

ihe-atna 0.2.3 is now the only supported release (it fixes the canonical text of an openEHR-matched category map, FerroHEALTH/FerroAUDIT#64). FerroFED takes both ihe-iti and ihe-atna, so ihe-iti takes ihe-atna = "=0.2.3" and moves to 0.0.43. 0.0.42 (#39, on 0.2.2) was not published and stays a gap in the crate line. Nothing else changes.

Checked locally: clippy on all twelve ihe-iti feature rows, nextest, the crate-version guard and the publish dry run.
