Add the Demand Letter and Fee Structure Certificate - #2002

Both build on the bonafide certificate: the same header, signatory, reference numbering and record table (two new certificate_type values on BonafideCertificate). Demand Letter: a semester-wise fee demand for a chosen semester and due date, with the Academic/Mess fee split read from the same FeeStructure the student is linked to, and the institute's bank transfer details read from a new shared DemandLetterBankAccounts singleton -- one record, common to every programme and academic year, editable from the Fee Structure page rather than hardcoded. Fee Structure Certificate: the full per-head fee break-up (Semester-I / Semester-II / Total) for the student's current academic session, derived from the same FeeStructure heads the Fee Structure admin screen already manages.
