docs: align PR descriptions with Walrus Sites changelog style - #969

Add a pull request template and a short contributing note so humans and agents write user facing PR bodies the same way Walrus Sites does, for uniform release tracking on docs.wal.app/changelog.
Use relayer (not server) for area prefixes; simplify changelog to two non exclusive boxes; rename section to When opening a PR; drop the agents and humans template sentence.

MemWal PR descriptions for user facing work now follow the same shape as Walrus Sites release notes, so Walrus Memory entries on https://docs.wal.app/changelog stay uniform with the rest of Walrus.
