Are you sure that it's not the fact that you're using the output of an inline macro (repo:...[]) as the list label?

My mistake. I posted the original code (repo:..) in the first example. But then decided to reproduce it without the inline macro.

So yes I'm sure that it is not related to the inline macro.

Just a note, this is also done by AsciiDoctor. Empty lines between list items are consumed and I suppose callouts are treated like numbered lists.

I'd still consider this a bug, however, because the labeled list is not indented as a child element of the callout, but is just dropped.
