…G-FR-7) Implements the Standalone Gateway TRD's core deliverables: a transparent MCP proxy that captures agent<->downstream-server sessions, seals them via the unchanged gsa-mcp-shim.js, and persists them to a new hash-chained gateway-session log (Option P2), with its own SG-FR-7 verifier. - scripts/gateway/mode-gate.js SG-FR-1/2: reads+validates .graphsmith/ gateway-mode.json before anything else (dormant exit 0 in attach mode) - scripts/gateway/config.js SS4 config schema + secret-ref resolution - scripts/gateway/session.js SS3.3/3.4/3.5: capture, JSON-RPC-id correlation, disconnect/anomaly handling, sealBoundaryBundle finalize - scripts/gateway/chain.js SG-FR-5: bundle + hash-chain persistence (bundle file -> chain.jsonl -> HEAD.json, in that crash-safe order); SG-FR-6 remote anchoring is an explicit, named stub (not implemented) - checks/register-gateway-sessions.js SG-FR-7 verifier, mirrors register-retention.js's shape; detects tampered entries, sequence gaps, and incomplete appends, each with a distinct failure reason - scripts/gateway/downstream.js, proxy.js, agent-transport.js, gateway.js transport/dispatch/process-lifecycle, reusing writer-claim.js unchanged schemas/gateway-config.schema.json and gateway-session-entry.schema.json are new, Track-1.2-owned schemas. Track 1.1's mode-selection-contract schema/CLI do not exist anywhere in this repo despite being described as closed -- mode-gate.js validates against a private, disclosed transcription of the frozen contract instead of inventing those files itself. tests/gateway/ (7 suites, 106 checks + a real child-process e2e test) covers the TRD's SS8 test plan; wired into ci-suite-manifest.json as gating. …lone-gateway Brings in Track 1.1's landed shared module scripts/mode-selection.js (readAndValidateGatewayMode/statusReport) so this branch's own private transcription in scripts/gateway/mode-gate.js can be swapped for it in a follow-up commit. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01WbbVn1NbzFJPXkBtu9U3Ns
…or the shared module Track 1.2's scripts/gateway/mode-gate.js carried its own private, disclosed transcription of the mode-selection contract's SS5.1 schema and SS7 fail-closed validation logic, built before Track 1.1's real shared module (scripts/mode-selection.js) existed. Now that Track 1.1 is merged in, this replaces that private transcription with a thin delegation to scripts/mode-selection.js's readAndValidateGatewayMode/computeConfirmationToken. GATEWAY_MODE_FILE/GATEWAY_MODE_KEY_FILE are now derived from the shared module's own path helpers instead of re-declared as separate string literals, so they can't drift. The private MODE_SELECTION_SCHEMA constant is removed -- nothing outside this file consumed it, and its validation role is now the shared module's. Also fixes tests/gateway/_fixtures/mode-file.js: it was writing the deployment secret as raw random bytes, which only worked against mode-gate.js's OLD private reader (an unencoded Buffer read with no format assumption). The real confirmation CLI path (scripts/mode-selection.js's ensureSecret/writeGatewayMode, now landed) always writes the secret as a 64-char hex-encoded STRING and reads it back as utf8 text -- verified this round-trips correctly end-to-end. The fixture now writes a hex-encoded secret to match the real, observable format instead of the old private module's more permissive (but unrealistic) raw-byte convention. All 11 tests/gateway/mode-gate/ tests still pass, no test assertions changed -- only the fixture's on-disk secret format. When two sessions have identical initialization data, granted tools, and call counts but different arguments or results, gsa-mcp-shim.js:52 assigns them the same bundle_id; this check therefore rejects the second session, even though its execution trace differs. Because GatewayProxy.closeConnection() has already removed the in-memory session before calling appendSession(), that attestation is permanently lost. The bundle or persistence key must incorporate the complete session content or a unique session identity. (KnoSky was unavailable; inferred from the live files.)

If the process crashes after this durable append but before HEAD.json is updated, the next session reads the stale head and appends another entry with the same sequence number and predecessor; the resulting chain can become [1, 2, 2]. Although the offline checker detects the original mismatch, gateway startup and appendSession() never invoke it, so a recoverable incomplete append is converted into a permanently forked chain. Reconcile or fail closed on the on-disk chain tail before accepting another append. (KnoSky was unavailable; inferred from the live files.)

The separator in this string is a literal NUL byte, which makes Git classify the new JavaScript source as binary; git show consequently reports only Binary files differ and normal text diff, blame, patch, and review tooling cannot inspect changes to this security-sensitive hash construction. Use an escaped "\0" representation, which produces the same runtime byte without turning the source file into a binary artifact. (KnoSky was unavailable; inferred from the live files.)

…ce in sessions - record each call's invocation-order seq at recordCallStart and sort by it before sealing, so execution_trace step numbers reflect when a call actually started rather than when its response happened to arrive (out-of-order downstream responses no longer misattest workflow order) - carry disconnected/disconnect_reason/jsonRpcId and session.anomalies through toSealableSession so this evidence can survive into the sealed bundle (see companion gsa-mcp-shim.js commit), not just live in memory - markPendingAsDisconnected now accepts an optional serverFilter so a downstream disconnect can be scoped to only that server's pending calls Addresses PR review comments on scripts/gateway/session.js:109 and :155.
…d bundle Additive, backward-compatible only (no schema change, existing callers' output is byte-for-byte unchanged when the new optional fields are absent): - execution_trace.jsonl lines now carry disconnected/disconnect_reason when the source call was marked disconnected - decision_record.md appends an Anomalies section when session.anomalies is non-empty Without this, scripts/gateway/session.js's toSealableSession could carry the evidence up to this shim but it would still be silently dropped before reaching the persisted, signed bundle -- an operator could only see it in the gateway's own in-memory state, never in the sealed record itself. Companion to the scripts/gateway/session.js commit. Addresses PR review comment on scripts/gateway/session.js:155.
…loss
Fixes several PR review findings in scripts/gateway/proxy.js:
- openConnection now seeds the session's granted tool surface from
mergedTools immediately, so a cached tool invoked without a prior
tools/list is no longer sealed with granted:false (proxy.js:67)
- handleDownstreamDisconnect now scopes markPendingAsDisconnected to the
server that actually disconnected, so a pending call to a different,
healthy downstream is left alone rather than falsely marked disconnected
(proxy.js:155)
- handleMessage now refuses new requests on every open connection (not
just new connections) once acceptingNewSessions is false, so an
already-open agent cannot keep issuing calls after the writer-claim is
lost; calls already in flight are unaffected and drain normally
(proxy.js:184)
- notifications/cancelled is now translated (via a connectionId+agent-id
-> {server, downstreamId} map recorded when each call starts) and
relayed to the owning downstream connection's new cancel(), instead of
being silently dropped (proxy.js:103)
- initialize now always returns the gateway's actually-selected protocol
version instead of echoing back whatever the agent requested
(proxy.js:100)
All changes verified against tests/gateway/proxy/run-tests.js (14/14 pass)
and tests/gateway/e2e/run-tests.js (18/18 pass, real fixture process).
…handshake, tool descriptor preservation, HTTP response validation, cursor pagination, reachability tracking Addresses review findings on PR #29 (scripts/gateway/downstream.js): - Reject duplicate tool names across downstream servers (GATEWAY_TOOL_NAME_COLLISION) instead of silently letting the second server's tool shadow the first's. - Complete the MCP initialize handshake: send full protocolVersion+capabilities+clientInfo params and the required post-initialize notifications/initialized notification before issuing tools/list. - Preserve each tool's full descriptor (description, annotations, outputSchema, etc.) instead of projecting down to name+schema only. - Validate downstream HTTP responses as well-formed JSON-RPC 2.0 envelopes matching the request id; reject malformed/mismatched responses instead of trusting them. - Follow tools/list pagination via nextCursor with a bounded page cap + cursor-cycle detection instead of silently truncating to the first page. - Track live HTTP downstream reachability (flips false on network error/timeout, true on any well-formed response) for use by gateway.js's health status. - Partial fix for the authenticated-HTTP-downstream finding: mirror this repo's own MCP HTTP server's required Streamable HTTP request-metadata headers (mcp-protocol-version, mcp-method, mcp-name) on every downstream request so a downstream enforcing that same contract is not rejected. The bearer-auth-credential half of that finding is left open -- see the review-thread reply for why (frozen downstream_servers schema, additionalProperties:false). Verified via tests/gateway/e2e/run-tests.js (18/18 PASS, exercises this file against the real fixture server) plus targeted node -e smoke tests against real HTTP servers and real stdio fixture child processes for collision detection, pagination-cursor following, and HTTP response validation (no dedicated unit suite exists for this file). …body/duration Addresses review findings on PR #29 (scripts/gateway/agent-transport.js): - runHttpAgentTransport() now refuses to start when the resolved bearer token is missing or shorter than 16 characters, matching the same hard requirement mcp-server/src/httpTransport.js already enforces for its own HTTP listener. The config schema cannot enforce this itself (token_ref is only a reference, not the resolved secret), so this is the one place that can actually catch a trivially-brute-forceable token before a network-accessible socket is bound. - The HTTP request handler now enforces an absolute per-request deadline (setTimeout + req.destroy(), cleared on "close") and a real byte-counted body-size cap (MAX_BODY_BYTES from mcp-server/src/httpTransport.js), mirroring that module's own request-bounding pattern. Without this, a slow-trickled or unbounded body could exhaust memory or hold a connection open indefinitely, including during graceful-shutdown drain. The session-identity finding in this same file (agent-transport.js:63, HTTP sessions keyed by TCP socket) is left open for clarification -- decoupling session identity from socket identity requires a session-lifecycle policy (idle timeout vs. explicit termination) this repo does not specify anywhere; see the review-thread reply. Verified via node -e smoke tests: a token under 16 chars throws synchronously before any listener binds; a valid 16+ char token starts the listener normally. No dedicated unit suite exists for this file. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…etup failure, wire session-chain integrity + last-persisted-at into health status Addresses review findings on PR #29 (scripts/gateway/gateway.js): - loadSigningKeys() now checks privateKey.asymmetricKeyType === "ed25519" and throws GATEWAY_BAD_SIGNING_KEY otherwise. createPrivateKey() happily accepts an RSA or EC key, but this build unconditionally labels every key "ed25519" regardless of what was actually loaded -- gsa-verify.js rejects a declared-algorithm/key-type mismatch, so a bundle sealed with a non-ed25519 key would previously persist successfully and only turn out to be unverifiable much later, at verify time. Caught now, at startup. - startGateway()'s agent-listener setup (stdio or HTTP) is now wrapped in try/catch: on failure (unresolved token_ref, a bind failure, etc.) it stops accepting new sessions, closes every already-open downstream connection, and releases the writer-claim before rethrowing -- previously that already-acquired state would leak, leaving the process able to remain alive after setting a non-zero exit code and blocking a future restart's claim acquisition. - buildHealthStatus() now computes lastPersistedAt from HEAD.json's mtime (SG-NFR-3's stated contract) and wires checks/register-gateway-sessions.js's SG-FR-7 session-chain integrity walk into the returned status -- previously that walk only ever ran via a synthetic --selftest, never against this deployment's real on-disk chain, so tampering/gaps/incomplete appends had no live operational signal. Downstream reachability now also prefers each connection's isReachable() when available (falls back to !isClosed()). Verified via node --check plus full re-runs of tests/gateway/{chain,config,lifecycle,mode-gate,e2e,session,proxy}/run-tests.js -- all suites green. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…n tail Addresses a review finding on PR #29 (checks/register-gateway-sessions.js): walkGatewaySessions() previously compared ctx.head's seq/bundle_id/entry_sha256 fields to the chain tail without first validating HEAD.json's own shape. A HEAD.json with a missing/wrong schema_version, extra fields, or an otherwise malformed shape could still slip through the tail comparison and receive a clean "verified" verdict as long as those three fields happened to match, defeating the fail-closed discipline the rest of this module already applies to chain entries (entryShapeOk). Added headShapeOk(), mirroring entryShapeOk's hand-rolled strict-shape discipline (and scripts/gateway/chain.js's own headShapeOk, deliberately duplicated to keep this module dependency-free per its header) and exported it. walkGatewaySessions() now calls it right after the "HEAD.json missing" check and fails closed (untrusted-input) on an invalid shape. Verified via node checks/register-gateway-sessions.js --selftest -- all 6 sub-checks pass (verified chain, tampered-entry detection, sequence-gap detection, missing-HEAD detection, stale-HEAD detection, missing-bundle detection). Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
… deterministic readiness signal Addresses a review finding on PR #29 (tests/gateway/e2e/run-tests.js, cleanSigtermDrainsAndExitsZero): the test previously waited a fixed 50ms after sending the slow tools/call before sending SIGTERM, hoping the gateway had already recorded the call as pending. Under CI scheduling pressure a 50ms sleep can fire before the gateway has even read its first line, making the test's premise (a call genuinely in flight at the moment SIGTERM is delivered) not actually hold. Replaced the fixed sleep with a second, fast tools/list request (id:2) sent immediately after the slow tools/call (id:1), awaiting its response as a deterministic readiness barrier before sending SIGTERM. This works because tools/list needs no downstream round-trip (answered straight from the cached tool surface) and readline dispatches "line" events strictly in the order lines were written, so receiving id:2's response proves the slow call's handleMessage() already ran synchronously up to and including session.recordCallStart() -- i.e. it is genuinely pending -- regardless of scheduler/startup latency. Added an assertion that the readiness response has id===2, and tightened the final in-flight-call assertion to also check callResp.id===1. Pure test-file change, no production code touched. Verified via a full run: node tests/gateway/e2e/run-tests.js -> 18/18 PASS (test is now deterministic instead of flaky). If power is lost after the fsynced chain.jsonl append but before HEAD.json is updated, the next run reads the stale head here and appends another entry with the same sequence and predecessor. startGateway() never invokes the integrity walk—only an optional status() call does—so normal recovery converts the documented incomplete append into a permanently forked chain. Verify and reconcile or fail startup before accepting sessions. KnoSky was unavailable; inferred from the live files.

…m request/response correlation, bound HTTP response bodies Addresses 3 new Codex findings on PR #29's board-locked implementation: - tools/list, tools/call, server/discover over HTTP now carry the required _meta block (protocol version, clientInfo, capabilities) on every request, since a plain JSON-RPC-over-HTTP downstream is stateless and never remembers an earlier initialize's legacy handshake state. - connectStdio's response-correlation check now requires the absence of "method" before treating a message as a reply to one of this client's own calls, so a downstream-initiated request reusing a currently-pending numeric id (each direction owns its own id namespace) is routed to onRequest instead of being silently swallowed as an undefined result. - connectHttp now caps a single response body at 10MiB and enforces an absolute response deadline (distinct from the existing idle-socket timeout), so a compromised/faulty HTTP downstream can't exhaust memory or hold a call open indefinitely via a slowly-trickled response.
…nd failures through the startup promise Two new Codex findings on PR #29's board-locked implementation: - The agent-facing HTTP listener now rejects every method but POST with 405 before authentication or body processing, mirroring mcp-server/src/ httpTransport.js's own contract -- previously a GET/PUT carrying a JSON-RPC body was authenticated and executed exactly like POST. - runHttpAgentTransport's startup promise now attaches a one-shot "error" listener before calling server.listen(), so an async bind failure (port already in use, EACCES, etc.) rejects the promise instead of leaving it pending forever -- startGateway()'s existing try/catch can now actually run its cleanup (release the writer-claim, close downstream connections) instead of the default process crashing uncaught after already acquiring the claim and spawning downstream children.
…error envelopes, enforce initialization lifecycle, emit per-call structured log Four new Codex findings on PR #29's board-locked implementation: - A tools/call result carrying isError:true is now recorded as an error in the session (sealed trace, output manifest) even though it is a structurally successful MCP response, not a transport/RPC failure -- the response sent back to the agent is unchanged (still a normal result, per MCP's own tool-error-is-not-a-protocol-error semantics). - A downstream's structured JSON-RPC error (code, message, data) -- already captured by downstream.js as error.rpcError -- now propagates to the agent unchanged instead of being flattened into a generic -32000. - Each connection now tracks whether it has completed initialize: tools/list and tools/call are rejected before it (previously dispatched under an empty initialization record), and a repeated initialize is rejected (previously silently overwrote already-attributed call metadata). - Each completed call now emits one structured JSON log line (connection, step, tool, server, status, duration) via an optional log callback (default no-op, so existing unit tests stay silent) -- previously the only gateway log for a call was the much-later (or, on a crash, never) session- finalize log.

… append (Cluster C) onClaimLost (gateway.js) only calls proxy.stopAcceptingNewSessions() when the writer-claim is lost -- by design, already-open sessions are left alone to finish and finalize (SS7: "in-flight sessions ... should still attempt to finalize and persist"). But that leaves a real gap: if a replacement writer has since acquired this state directory, an in-flight session finalizing after that point would call chain.appendSession() and interleave with the new writer's own appends, corrupting the single-writer hash-chain invariant. Adds a synchronous, read-fresh-from-disk isWriterClaimValid() check in GatewayProxy, consulted immediately before chain.appendSession inside closeConnection -- the narrow fix (re-check liveness right at the integrity-critical write) rather than eagerly halting every in-flight session the moment claim-loss is first detected. When the claim is no longer valid at that check, the session is NOT appended: the already-sealed bundle is quarantined (same quarantineSealedBundle path persistence failures already use) and reported via onSealFailure, so an operator or the next writer can inspect/replay it. Defaults to always-valid when not wired up, so every existing caller/test is unaffected. … mode-gate root (partial) Three changes bundled in this file: 1. Cluster C wiring: passes isWriterClaimValid: () => writerClaim.status() .held_by_this_instance into GatewayProxy's constructor (the check itself landed in proxy.js's closeConnection in the prior commit), and updates onClaimLost's own comment to describe the narrower, two-part fix instead of implying eager halting is the whole story. 2. Cluster D: exposes SG-NFR-3's buildHealthStatus(ctx) operationally for this single-tenant, locally-run process. Writes it to <state_dir>/ gateway-status.json on a fixed interval (STATUS_WRITE_INTERVAL_MS, default 10s) via state-store.js's existing atomic-write primitive, plus once immediately at startup and once more on clean shutdown. Adds a `node gateway.js status [configPath]` CLI subcommand that reads and pretty-prints that file, warning if the snapshot looks stale. No new ops HTTP port -- deliberately out of scope, see writeStatusFile's own comment. 3. Cluster E (partial fix only -- see PR description for what is NOT included): checkModeGate's validation root now derives from configPath's own directory (path.dirname(path.resolve(configPath))) instead of process.cwd(), so `gateway.js --config /path/to/project-b/gateway.json` run from inside a different project's directory validates project B's own .graphsmith/gateway-mode.json, not whatever sits under cwd. options.root remains available as an explicit override. Does NOT add any state_dir cross-check against the mode file -- the finalized mode- selection-contract schema carries no such field, and adding one would mean amending an already-final TRD, out of scope here. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
Covers closeConnection's synchronous isWriterClaimValid() re-check immediately before chain.appendSession: refuses to append and quarantines the sealed bundle when the claim is invalid at that moment, and confirms the default (no isWriterClaimValid configured) remains a no-op so every other test in this suite and any caller that hasn't wired a writer-claim keeps behaving exactly as before. …r E mode-gate root fix modeGateRootTracksConfigPathNotCwd spawns the real CLI with cwd set to a directory with no .graphsmith/ at all, passing an absolute configPath into a separate, properly mode-confirmed project root -- proving checkModeGate's root now tracks configPath's own directory rather than process.cwd(). statusCommandReadsRunningGatewaysStatusFile spawns `gateway.js status <configPath>` as a genuinely separate CLI invocation against a running gateway's periodically-written status file, and also covers the no-status-file-yet case (clear non-zero exit naming the missing file, not a stack trace). Guard the complete gateway chain append.
gateway.js binds isWriterClaimValid() to writerClaim.status().held_by_this_instance. WriterClaim renews with a timer that cannot run while synchronous work blocks the event loop. If the append exceeds staleAfterMs, another writer can steal the claim after the check returns true.
appendSession() has no shared append lock or CAS. Two writers can read the same HEAD.json, append entries with the same sequence and previous-head values, and leave HEAD.json inconsistent with chain.jsonl.
Use one durable keyed append lock for every gateway writer. Acquire it before claim validation and release it only after the bundle, chain, and HEAD.json writes complete. If the lock is leased, keep it valid for the full critical section. Apply intent → effect → completion ordering. This gateway filesystem write is covered by the JavaScript workflow guideline.

Fixed in becee40: added a synchronous writerClaim.renew() call immediately before chain.appendSession in closeConnection (new GatewayProxy#renewWriterClaim opt, wired from gateway.js), closing the heartbeat-starvation gap on the claim-staleness half of this finding -- a long synchronous append can no longer let the claim go stale between the isWriterClaimValid() read and the write.
Not changing further: the broader ask (one durable, gateway-wide keyed append lock/CAS wrapping the whole bundle+chain+HEAD.json write) is a larger architectural change to the single-writer design than this fix, and needs a product decision rather than being folded into a surgical fix.

@pjpoulose, I verified the becee40 change.
GatewayProxy#closeConnection() now calls renewWriterClaim() immediately before chain.appendSession(). gateway.js correctly wires that option to writerClaim.renew(). This reduces the heartbeat-starvation window.
The original finding remains open. appendSession() still performs separate bundle, chain.jsonl, and HEAD.json writes without a gateway-wide append lock or compare-and-swap operation. If the synchronous append exceeds staleAfterMs, another writer can take over after renewal and during that write sequence. The two writers can still derive chain state from the same head.
I agree that the complete append guard is a larger design decision. Would you like me to open a follow-up GitHub issue for the durable append-lock/CAS design?

…ession PR #29 review round 8 (CodeRabbit + Codex): recordUnmatchedDownstreamResponse in gateway.js was calling session.recordCallResult(s, msg.id, ...) where msg.id is the downstream-internal id (not an agent-facing JSON-RPC id) -- risking collision with a live pending agent call sharing the same id value and corrupting the session trace. Adds a dedicated recordUnmatchedResponseAnomaly() helper that only ever appends the anomaly and never touches pendingCalls, plus a fixed MAX_ANOMALIES_PER_SESSION cap (1000) so a misbehaving downstream cannot grow session.anomalies without bound; the cap being hit is itself recorded once via a terminal ANOMALY_CAP_REACHED marker rather than silently truncating. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
Adds unmatchedResponseAnomalyDoesNotTouchPendingCalls (proves a colliding jsonRpcId does not corrupt a live pending call, and that call still resolves normally afterward) and anomaliesCappedWithTerminalMarker (proves the MAX_ANOMALIES_PER_SESSION cap and its terminal ANOMALY_CAP_REACHED marker). Companion test for the session.js fix in this same PR round. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…cations/initialized, contain logger failures Three PR #29 round-8 review fixes to proxy.js: - CodeRabbit "reserve capacity for pending calls": the MAX_COMPLETED_CALLS_PER_SESSION admission check only counted s.calls.length (already-completed calls), ignoring calls already admitted and genuinely in flight (s.pendingCalls). Several concurrent in-flight requests could all pass the check before any resolved, letting s.calls.length grow past the cap once they all completed. Now checks s.calls.length + s.pendingCalls.size. - Codex "wait for notifications/initialized before admitting tools": tools were admitted immediately once the "initialize" response was computed -- before the agent necessarily received it (an id-less initialize sent as a notification hits the same code path) and before the MCP-required "notifications/initialized" arrived. agentInitialized is now a 3-state string ("none" | "awaiting_notification" | "ready"); tools/list and tools/call are admitted only in the "ready" state, reached only via a real "notifications/initialized" notification. - Codex "contain logger failures after downstream effects": a configured `log` callback throwing after a downstream tool call already succeeded could escape handleMessage and turn a real success into a JSON-RPC internal error, inviting a client retry of a possibly non-idempotent effect. Added a non-throwing safeLog() wrapper used at both post-effect log call sites. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…-call cap fix Adds sendInitializedNotification() and sends it after every existing initialize call site that precedes tools/list or tools/call (GatewayProxy now requires it before admitting either). Extends initializationLifecycleEnforced with pre-notification tools/call and tools/list rejection assertions, and adds noIdInitializeDoesNotUnlockToolsWithoutNotification to cover an id-less (notification-shaped) initialize. Companion test for the proxy.js fixes in this same PR round. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…owlist Two Codex PR #29 round-8 review fixes to downstream.js: - "bound termination of stdio downstream children": close() previously sent SIGTERM once and returned synchronously without confirming the child actually exited -- a stdio downstream that traps or ignores SIGTERM would keep running (and keep its pipes open) past startup-failure cleanup or shutdown. close() now returns a promise that races the child's own "close" event against a bounded STDIO_CHILD_TERMINATE_TIMEOUT_MS (5000ms, test-overridable via a new connectStdio options.terminateTimeoutMs) timer that escalates to SIGKILL. Callers in gateway.js now await it. - "reject unsupported downstream protocol selections" (5-model panel, Paul-approved 2026-09-12): connectAllDownstreams previously accepted any non-empty protocolVersion string from a downstream's initialize result. Added SUPPORTED_DOWNSTREAM_PROTOCOL_VERSIONS, an explicit allowlist Set seeded with this client's own MCP_PROTOCOL_VERSION (deliberately not a bare equality check, and deliberately not the prior fully-permissive behavior) -- a downstream selecting a version outside it is now rejected at startup with a distinct GATEWAY_DOWNSTREAM_UNSUPPORTED_PROTOCOL_VERSION code (kept distinct from GATEWAY_DOWNSTREAM_MALFORMED_RESPONSE since the response is well-formed, just naming an incompatible version -- a judgment call, flagged for review). Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…on allowlist - Rewrites unmatchedStdioResponseSurfaced's wait logic: resolves a promise from the onUnmatchedResponse callback itself, raced against a 2000ms failure-limit timeout, instead of a fixed 200ms wait that is flaky under a slow CI worker (CodeRabbit PR #29 review round 8). - Adds unsupportedProtocolVersionRejected: a fixture returning a well-formed but unsupported protocolVersion ("1999-01-01") must be rejected with GATEWAY_DOWNSTREAM_UNSUPPORTED_PROTOCOL_VERSION. - Adds stdioChildTerminationEscalatesToSigkillAfterTimeout (a fixture that traps SIGTERM and keeps its event loop alive independently of stdin closing, proving close() actually escalates to SIGKILL within the bounded terminateTimeoutMs) and stdioChildTerminationResolvesPromptlyOnCleanExit (a well-behaved child resolves well under the timeout). Companion test for the downstream.js fixes in this same PR round. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
… helper, cap downstream-initiated sampling history, add a startup connect watchdog Three PR #29 round-8 review fixes to gateway.js: - CodeRabbit + Codex "record/keep unmatched downstream IDs out of agent correlation": recordUnmatchedDownstreamResponse called session.recordCallResult(s, msg.id, ...) using the downstream-internal id, risking collision with a live agent-facing pendingCalls entry sharing the same id value. Now calls session.recordUnmatchedResponseAnomaly (added in session.js this same round), which only ever appends the anomaly and never touches pendingCalls. - Codex "cap completed downstream sampling history": proxy.js's own agent-initiated dispatch enforces MAX_COMPLETED_CALLS_PER_SESSION and MAX_PENDING_CALLS_PER_SESSION, but this separate downstream-initiated sampling path (forwardDownstreamRequestToAgent) never consulted either -- a sampling-capable stdio downstream issuing requests sequentially could grow session.calls without bound. Mirrors proxy.js's own admission-refusal shape exactly. - Codex "install shutdown handlers before gateway startup completes" (5-model panel, Paul-approved 2026-09-12, majority position): the panel rejected building a signal handler that tears down partially-initialized startup state as riskier than the status quo on the WriterClaim lock-safety path. Instead adds a bounded STARTUP_DOWNSTREAM_CONNECT_TIMEOUT_MS (60000ms, test-overridable via options.startupDownstreamConnectTimeoutMs) watchdog around connectAllDownstreams via a new withTimeout() helper, routed through the SAME startup-failure cleanup path (writerClaim.release() + rethrow, GATEWAY_STARTUP_TIMEOUT) an outright connection failure already uses. Graceful-shutdown handler registration order in main() is unchanged (still registered only after startGateway() resolves), per the panel's explicit direction. On investigation, no existing crash-recovery/stale-claim-reclaim path was found in writer-claim.js beyond the existing single-writer claim/heartbeat/release mechanism -- reported here rather than built, since building one was explicitly out of scope for this fix. Also updates the startup-failure and doStop() downstream-close loops to `await conn.close()`, since downstream.js's close() now returns a promise (this same PR round's downstream.js fix). Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…up watchdog Codex PR #29 review round 8 findings, exercised at the unit level: - samplingForwardRefusesOnceCompletedCallCapReached / samplingForwardStillWorksBelowTheCap: prove forwardDownstreamRequestToAgent's new MAX_COMPLETED_CALLS_PER_SESSION admission check (landed in scripts/gateway/gateway.js, commit 9a30ef1) actually refuses once the cap is reached and still forwards normally below it, using a fake session/proxy/ agentPusher (driving the real cap via genuine round trips would be far too slow for a unit suite). - startupWatchdogTimesOutOnHungDownstreamConnect: proves STARTUP_DOWNSTREAM_CONNECT_TIMEOUT_MS (gateway.js, commit 9a30ef1) actually fails startGateway() closed against a real hung child process that spawns but never completes its handshake, and that the same startup-failure cleanup path releases the writer-claim -- not just that the promise rejects. Also adds notifications/initialized sends to drainWaitsForInFlightCallToComplete and drainTimesOutAndReportsIncomplete, required by the PR #29 3-state agentInitialized fix (proxy.js, commit c6f4851) these two pre-existing tests exercise. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…alized notifications CodeRabbit PR #29 review round 8 "verify that the failed session was persisted": httpFailedInitializeDoesNotLeakSession previously only proved session REMOVAL (a follow-up call on the leaked session id sees "unknown or expired"). An implementation that deletes the session without ever writing its bundle would have passed just the same. Now also asserts chain.readHead(stateDir) names exactly this one sealed session (seq===1, a string bundle_id) and that its bundle file actually exists on disk, not merely referenced -- mirrors singleSessionEndToEndVerifies' own existing e2e-exactly-one-bundle-written pattern elsewhere in this file. Ripple from proxy.js's 3-state agentInitialized fix (commit c6f4851): tools/call and tools/list are now rejected until a real notifications/initialized has been sent, so adds a new httpNotify() helper (tolerates the empty 202 body a JSON-RPC notification gets back over the agent HTTP transport, unlike httpPost's request/response pairs) and sends notifications/initialized in every test that exercises tools/call or tools/list after its own initialize: singleSessionEndToEndVerifies, samplingForwardedToStdioAgent, malformedPushedReplyIsRejectedNotSilentlyAccepted, pushedReplyWithPresentButFalsyErrorIsRejected, samplingOverHttpAgentGetsExplicitError, httpDownstreamAgainstRealMcpServerSucceeds, httpAgentSessionsAreIdBasedNotSocketBased (both sessionA and sessionB), and cleanSigtermDrainsAndExitsZero. Treat sampling as a negotiated client capability
When a downstream legitimately invokes sampling/createMessage, this records it as the pseudo-tool <server>:sampling/createMessage, but sealBoundaryBundle() builds grantedTools exclusively from the downstream tools/list surface. Consequently every negotiated sampling call is persisted with "granted":false, falsely presenting authorized model sampling as an ungranted invocation in the signed audit evidence. Record the negotiated sampling capability separately or exclude model calls from the tool-grant comparison. KnoSky was unavailable; inferring from the live files.

Fixed in commit 2c00732. sealBoundaryBundle's model_call granted-computation now derives from whether sampling was actually negotiated for the connection (gateway.js's agentTransportSupportsSampling, threaded through GatewayProxy -> session.samplingNegotiated -> gsa-mcp-shim.js), instead of checking the sampling/createMessage call id against the downstream tools/list surface. tools/call entries' granted-computation is unchanged. All 226 gateway tests plus the gsa-mcp-shim --selftest pass.

Fresh evidence after the completed-history cap was added: the limit permits 100,000 retained calls, while each call can retain a request near the agent input cap and a downstream result near the 10 MiB response cap. A sequential authenticated client or compromised downstream can therefore consume memory long before reaching the count limit—at the declared maxima, the retained payload budget is roughly a terabyte. Track cumulative retained bytes or roll sessions at a practical byte limit rather than relying solely on entry count. KnoSky was unavailable; inferring from the live files.

Fixed in becee40: added MAX_SESSION_CALL_BYTES (256MB -- a fixed, non-speculative constant matching MAX_PENDING_CALLS_PER_SESSION/MAX_COMPLETED_CALLS_PER_SESSION's own discipline, not config-driven, same as those two). session.js now tracks a running session.totalCallBytes (request params + response result, serialized) in recordCallResult and markPendingAsDisconnected, checked at admission time alongside the two existing count caps with the same -32000 error family.

Use the non-throwing logger for sampling completions
Fresh evidence after GatewayProxy.safeLog() fixed the agent-initiated path: this separate downstream-sampling completion path still invokes the caller-supplied logger directly after the agent result has been recorded. If that logger throws, the fulfilled callback becomes rejected and connectStdio() sends the downstream an internal-error response despite the completed model invocation, encouraging a retry and potentially duplicating an expensive or externally consequential step. Route this through the same non-throwing logging wrapper. KnoSky was unavailable; inferring from the live files.

Fixed in becee40: logCompletion's log() call on the downstream-initiated sampling completion path is now wrapped in try/catch, so a throwing logger can no longer reject the forward and turn an already-completed model invocation into an error response sent to the downstream. Wrapped inline rather than routed through proxy.safeLog, since this function's own test suite passes a minimal {sessions, now} stand-in for proxy that doesn't implement safeLog -- the inline try/catch gives the identical non-throwing guarantee without that dependency.

Install signal handlers before starting the gateway
Fresh evidence after the startup-watchdog change: the SIGTERM/SIGINT handlers are still registered only after startGateway() resolves. A signal during the up-to-60-second downstream-connect phase therefore still uses Node's default immediate termination and bypasses cleanup of the already-acquired writer claim and any spawned downstream children; the watchdog cannot help after the process has already been killed. Install startup-aware handlers before beginning asynchronous initialization. KnoSky was unavailable; inferring from the live files.

Not changing this -- this file's own header comment (the "install shutdown handlers before gateway startup completes" comment directly above STARTUP_DOWNSTREAM_CONNECT_TIMEOUT_MS in gateway.js) documents that the 5-model external frontier panel's majority position (Paul-approved) was explicitly against building a signal handler that tries to safely tear down partially-initialized startup state -- calling that a real correctness risk on the WriterClaim lock-safety path, worse than the status quo. The accepted mitigation is STARTUP_DOWNSTREAM_CONNECT_TIMEOUT_MS (60s) bounding the one unbounded startup step (connectAllDownstreams) instead of a startup-spanning signal handler. Revisiting that panel decision is a product call for Paul, not a mechanical code fix.

Validate downstream requests before forwarding sampling
A stdio message is treated as a downstream-initiated request based only on having a string method and a non-null id; it need not carry jsonrpc: "2.0", a valid identifier type, or an otherwise valid request envelope. A faulty downstream can therefore send a malformed object such as {"id":1,"method":"sampling/createMessage"} and still trigger a real agent model invocation and an attested call. Validate the complete JSON-RPC request envelope before invoking options.onRequest, returning an invalid-request error without forwarding malformed traffic. KnoSky was unavailable; inferring from the live files.

Fixed in becee40: connectStdio now validates jsonrpc:"2.0" and that id is a string or finite safe-integer number before treating a downstream message as a request and invoking options.onRequest; an invalid envelope now gets a -32600 error written back instead of being forwarded to onRequest/the agent's model. Mirrors proxy.js#handleMessage's own existing envelope/id-type validation rule (same code, same id-type test) rather than inventing a new one.

Persist disconnected-call correlation IDs
When a connection closes with a call pending, markPendingAsDisconnected() records its jsonRpcId and toSealableSession() deliberately forwards that field, but this final trace projection persists only disconnected and disconnect_reason. The sealed evidence therefore loses the identifier needed to correlate the disconnect with the observed JSON-RPC request, contrary to the newly added disconnected-call capture contract. Include jsonRpcId in the persisted trace entry, with an explicit representation for synthetic IDs if necessary. KnoSky was unavailable; inferring from the live files.

Validate MCP tool result shapes before recording success
After a well-formed JSON-RPC envelope resolves, the gateway treats every non-throwing tools/call result as successful unless it happens to be an object with isError: true. A faulty downstream can therefore return null, a scalar, or {} without the required MCP tool-result content, and the gateway forwards it and seals it as a successful call; conforming agents may reject the response while the audit trail says the operation succeeded. Validate the method-specific tools/call result shape before computing toolLevelError and fail closed on malformed payloads. KnoSky was unavailable; inferring from the live files.

Fixed in commit 8f2fdc2 on this branch.
Added a third audit state alongside transport-error and tool-level-error: malformedResult, computed by a small, pure, exception-safe predicate (isMalformedToolCallResult in scripts/gateway/proxy.js) per the converged predicate -- a tools/call result is flagged only when it is null, not an object, or an array, or is a plain object with none of: a content array, a structuredContent object, or an explicit isError boolean. Extra/unknown fields never trip it, and the check fails open (not malformed) if it throws.
Scoped strictly to tools/call and only when the downstream actually responded (never for sampling/createMessage, never for a transport failure). It is audit-only: isError and the JSON-RPC response returned to the agent are unchanged.
Wired through additive-only, so a well-formed call's signed trace line and operational log line are byte-for-byte unaffected:
- proxy.js passes
malformedResultintosession.recordCallResultalongsideresult/isError, and addsmalformed_result: trueto thegateway_call_completedlog line only when true. - session.js stores it on the call record and carries it through
toSealableSession. - gsa-mcp-shim.js's
sealBoundaryBundlefolds it into the signed execution_trace asmalformed_result: true, omitted entirely (notfalse) for a well-formed call.
Added 25 new tests covering the full converged matrix (null/string/number/array/empty-object as malformed; content-array/structuredContent/isError-boolean as not malformed, including empty content array and isError:true-with-content coexisting with malformed:false; extra fields never trip it; hostile-getter fail-open; scope exclusion for sampling and transport failures; JSON-RPC/isError parity; byte-parity of a clean call's trace line). All 226 pre-existing gateway tests plus the 25 new ones pass (251 total).
…ertion (FR-12) stdioChildTerminationEscalatesToSigkillAfterTimeout's timing check (close-escalated-to-sigkill-rather-than-hanging-on-a-trapped-sigterm) assumes a child process can trap and ignore SIGTERM long enough to prove close()'s SIGKILL-escalation path actually ran. Windows has no true POSIX signal delivery -- child.kill() force-terminates the child unconditionally regardless of any registered handler -- so the assumption is false there, and the assertion failed deterministically on all 4 windows-latest CI legs (node 18/22, both workflow-run triggers) at elapsed=3-4ms against a >=150ms floor. downstream.js's close() itself was read directly and confirmed correct on its own terms (a platform-agnostic bounded-wait-then-SIGKILL race) -- this is a test-only fix. Skips just the Windows-dependent half of the test, mirroring this repo's own existing precedent (tests/gateway/e2e's e2e-sigterm-clean-drain-and-release) for exactly this class of platform-unreproducible signal assertion. The other half of the same test (close-resolves-once-sigterm-trapping-child-is-actually-gone) is unaffected and still runs on every platform. Verified locally (Linux, 3x for stability): 24/24 passing, 0 skipped (the new skip only fires on win32).
…vious push The prior commit (6692938) accidentally hardcoded two dynamic interpolations as literal placeholder strings during a manual content transcription: - capturePath -> "__CAPTURE_PATH__" in samplingCapabilityAdvertisedOnlyWhenSupported() - name -> "__NAME__" in onRequestReceivesOwnServerName()'s writeSamplerFixture(name) This restores both to their correct dynamic values. Verified byte-for-byte against the locally tested source (md5 2f05ed0d348bd443a72aadb2a8cf95eb) before push.
…-recovery-idempotency Resolves PR #29 / PR #33 merge conflict per Cluster H/I frontier-panel verdict (5/5 unanimous both clusters, Paul approved 2026-09-14): - Cluster H (gateway.js sampling-forward path): MAX_COMPLETED_CALLS_PER_SESSION refusal check runs first (unchanged from PR #29); the WAL-append setup (walCallSeq/walAppendFailed, PR #33) follows, unconditional and independent of that check. Correction from the original panel brief: this cap is a hard admission-refusal, not an in-memory eviction -- a refused call is never admitted, recorded, or forwarded, so it never reaches the WAL either. No separate WAL-bounding code was needed at this composition point. - Cluster I (session.js anomaly recording): recordAnomaly and recordUnmatchedResponseAnomaly now share one internal, capped pushCappedAnomaly helper (MAX_ANOMALIES_PER_SESSION), so PR #33's new ambiguous-retry-blocked anomaly is bounded the same way PR #29's unmatched-response anomaly already was. All 7 existing external call sites are unchanged. - Remaining 4 hunks (gateway.js imports/exports, proxy.js's agentInitialized 3-state fix + new pendingOperatorReviewConnections field, session.js exports) are mechanical keep-both unions, no judgment call. No thread resolved, no merge to a shared branch, no push to main. Co-Authored-By: Claude Sonnet 5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
…ce, found while verifying the PR #29/#33 merge Discovered running the full gateway test suite against the merge commit (not part of the approved Cluster H/I conflict resolution itself, but required to get a genuinely green merge): - tests/gateway/proxy/run-tests.js: PR #33's entire Option C test suite (idempotency/recovery tests) predates PR #29's 3-state agentInitialized gating fix (round 8, 'wait for notifications/initialized before admitting tools') and never sent notifications/initialized before tools/call -- that fix's own comment already says every such test must. Added the missing sendInitializedNotification() call after each initialize (25 call sites across single- and multi-connection tests), and updated one stale boolean assertion (agentInitialized.get(...) === false) to the correct 3-state sentinel ('none'). - tests/gateway/lifecycle/run-tests.js: samplingForwardStillWorksBelowTheCap predates PR #33's WAL append in forwardDownstreamRequestToAgent and used a bare {sessions, now} fake proxy with no stateDir, so the now-unconditional WAL append threw. Gave it a real stateDir via the file's own freshDir() helper. - scripts/gateway/agent-transport.js: a real, pre-existing bug, exposed (not caused) by PR #29's own legitimate doStop() fix ('await close() so a downstream ... is actually confirmed gone'), which shifted doStop's timing enough to surface it within the process's lifetime. The stdin readline 'close' listener calls proxy.closeConnection() from inside an async event-listener callback with no try/catch; an unreadable/corrupt recovery intent for that connection throws, and with nothing awaiting or catching this listener's returned promise, the rejection is unhandled -- crashing the entire gateway process. This is not shutdown-specific: any ordinary stdio disconnect racing a corrupt intent hits the same path. gateway.js's own doStop() loop already guards its own (redundant, in the shutdown case) call to closeConnection for exactly this failure mode; mirrored that guard here. Confirmed via a side-by-side worktree of feature/gateway-crash-recovery-idempotency's own tip (8167f9d) that this test suite passes there today -- the bug is real and latent on that branch alone, merely not manifesting before this merge's timing shift. All 9 gateway test suites now pass: 435 passed, 0 failed, 0 skipped (chain 15, config 19, downstream 24, e2e 63, lifecycle 17, mode-gate 11, proxy 111, recovery 137, session 38). …byte cap, jsonRpcId trace field, non-throwing sampling logger, downstream request-envelope validation)
Paul-approved fixes from the re-triage round, landed directly on this branch:
1. proxy.js#closeConnection: renew the writer-claim synchronously immediately before chain.appendSession (new GatewayProxy#renewWriterClaim opt, wired in gateway.js), closing the gap where a long synchronous append can starve the writer-claim heartbeat past staleAfterMs. Mirrors proxy.js's own existing isWriterClaimValid opt-injection pattern; renewal failure falls through to the existing quarantineSealedBundle path via the same try/catch as appendSession.
2. proxy.js: new MAX_SESSION_CALL_BYTES cap (256MB, fixed constant matching the existing MAX_PENDING_CALLS_PER_SESSION/MAX_COMPLETED_CALLS_PER_SESSION discipline) -- session.js now tracks a running session.totalCallBytes (request params + response result, serialized) in recordCallResult and markPendingAsDisconnected, checked at admission time alongside the two existing count caps.
3. gsa-mcp-shim.js#sealBoundaryBundle: add the missing conditional spread for jsonRpcId on the execution_trace line, matching the existing disconnected/disconnect_reason spread style -- the field was documented but never actually written.
4. gateway.js#forwardDownstreamRequestToAgent's logCompletion: wrap the caller-supplied log() call in try/catch so a throwing logger can no longer turn an already-completed downstream-initiated sampling call into a rejected forward/error response (mirrors GatewayProxy#safeLog's own non-throwing contract; inlined rather than routed through proxy.safeLog because this function's own tests pass a minimal {sessions, now} stand-in for proxy).
5. downstream.js#connectStdio: validate the JSON-RPC envelope (jsonrpc:"2.0", id a string or finite safe-integer number) of a downstream-initiated request before invoking options.onRequest, returning a -32600 error instead of forwarding malformed traffic to the agent's model. Mirrors proxy.js#handleMessage's own existing envelope/id-type validation rule.
Not changed (escalated back for a product decision, not mechanical fixes -- see the review thread replies and session report):
- gateway.js:217 sampling-not-marked-granted in sealBoundaryBundle's grantedTools computation -- changing what "granted" means for a model_call in a signed attestation bundle is a semantics decision, not a mechanical fix.
- proxy.js:424 tools/call result-shape validation before recording success -- requires designing an MCP CallToolResult shape validator and deciding failure semantics; a behavior change to every call's audit trail, not a narrow fix.
- gateway.js:871 signal-handler-after-startup timing: reject-with-reason only, no code change -- this build's header comment already documents the Paul-approved 5-model frontier panel decision against a startup-spanning signal handler.

…ot tools/list
sealBoundaryBundle's execution_trace computed `granted` for every call entry by
checking its id against the downstream tools/list surface (grantedTools). A
sampling/createMessage call is a model_call, not a tools/call -- its id
(e.g. "fs:sampling/createMessage") can never appear in grantedTools, so every
sampling call was signed into the attestation bundle as granted:false, reading
as "this call was not authorized" even when it plainly was.
Sampling is its own MCP capability, negotiated once per connection at
initialize, not something tools/list was ever meant to cover. This gateway
already computes that fact -- gateway.js's agentTransportSupportsSampling
(from config.agent_listen.transport) is the same signal already used to (a)
advertise `capabilities: { sampling: {} }` to each downstream server and (b)
gate whether forwardDownstreamRequestToAgent can relay a call at all.
Thread that existing fact through instead of inventing new tracking state:
- gateway.js passes it to `new GatewayProxy({ agentSupportsSampling: ... })`
- proxy.js stores it and stamps it onto every session it opens
(session.createSession(..., { samplingNegotiated: this.agentSupportsSampling }))
- session.js carries session.samplingNegotiated through toSealableSession
- gsa-mcp-shim.js#sealBoundaryBundle now computes a model_call entry's
`granted` from session.samplingNegotiated, leaving tools/call entries'
granted-computation (tools/list membership) unchanged
Fixes the gap flagged in PR #29 Codex review comment 4000335132 ("Treat
sampling as a negotiated client capability"). All 226 gateway tests
(tests/gateway/*) and the gsa-mcp-shim --selftest pass unchanged.

…ool-level errors PR #29 review thread 4000335147 (frontier-panel review, 4/4 independent models converged after two rounds of critique): a `tools/call` result was previously judged only by `isError` -- a downstream that answered with neither a recognizable success shape (`content` array) nor a recognizable error/structured shape (`structuredContent` object, or an explicit `isError` boolean) was recorded as a normal, non-error call, since nothing about isError true/false was ever asserted on it. This adds a third state to the audit model, alongside transport error and tool-level error: `malformedResult`, computed by a small, pure, exception-safe predicate (isMalformedToolCallResult in proxy.js), scoped strictly to tools/call and only when the downstream actually responded (never for sampling/createMessage, never for a transport failure). It is audit-only -- it never changes isError, never changes the JSON-RPC response returned to the agent, and is not a validation gate (per-content- block schema conformance stays out of scope). Wired through additive-only, so a well-formed call is byte-for-byte unaffected: - proxy.js passes `malformedResult` into session.recordCallResult alongside result/isError, and mirrors it into the `gateway_call_completed` structured log line only when true. - session.js stores it verbatim on the call record and carries it through toSealableSession for gsa-mcp-shim.js to read. - gsa-mcp-shim.js's sealBoundaryBundle folds it into the signed execution_trace as `malformed_result: true`, omitted entirely (not `false`) for a well-formed call. Covers the converged test matrix: null/string/number/array/empty-object results (malformed), content-array/structuredContent/isError-boolean results (not malformed, including empty content array), isError:true with content present (both flags coexist, malformed false), extra/unknown fields (never trip it), a hostile getter that throws (fails open, not malformed), scope exclusion for sampling/createMessage and transport failures, JSON-RPC response/isError parity, and a byte-parity check that a clean call's signed trace line carries no malformed_result key. …very dir tree Round-5 frontier-panel finding #4 (2026-09-18) was marked fixed in 4ec5a4d, but the fix was incomplete: ensureDir's ancestor-fsync loop fsyncs every newly-created ancestor's own CONTENTS (e.g. gateway-recovery/'s listing of active/), but never the pre-existing PARENT's listing of the topmost newly-created ancestor (e.g. state_dir's listing of gateway-recovery/ itself). A crash right after the very first recovery write on a fresh state_dir could therefore still lose the gateway-recovery/ directory entry -- the exact scenario the original fix's own doc comment claimed to prevent. Found and independently verified while merging PR #33 onto PR #29's latest branch tip ahead of Bug B's design work. Fixed by also fsyncing `cursor` (the first pre-existing ancestor found by the walk-up) whenever anything was actually created under it. 2026-09-12 follow-up (re-triage fixes, Clusters C/D/E): Added a synchronous writer-claim re-validation immediately before
chain.appendSessionincloseConnection(Cluster C) so an in-flight session whose writer-claim was lost/superseded between opening and finalization is quarantined instead of appended, closing the single-writer hash-chain race window; added agateway.js status <configPath>CLI subcommand backed by a periodically-written status file using the existingbuildHealthStatus(ctx)(Cluster D, no separate ops HTTP port). Cluster E: this PR implements only the configPath-derived-root half of the originally-discussed mode-gate fix (checkModeGatenow derives its root fromconfigPath's own directory instead ofprocess.cwd()) — the state_dir cross-check against the mode file's recorded state was scoped out because the finalized mode-selection contract schema carries no such field. The two bugs from the prior re-triage note (an uncaught throw inagent-transport.js'sconnectionIdFor(), and an unhandledchain.appendSessionfailure inproxy.js) were independently re-verified against the current HEAD and found already fixed by earlier commits (53d9b2bremovedconnectionIdForvia refactor;0dfc661added theappendSessionfailure guard) — no further code change was needed for either.Summary by CodeRabbit
