Summary

Fixes two Codex findings from PR #29's review, per Paul's explicit instruction: not folded into #29 itself, but built as a separate, external-panel-reviewed design ("Option C") and shipped here as its own PR.

  • Finding 1 (session.js): a completed tools/call result lived only in memory until the whole agent connection closed. A gateway crash between "downstream returned a result" and "the agent disconnects" silently dropped that evidence from the signed attestation chain.
  • Finding 2 (proxy.js): nothing tied a retried tools/call to its original attempt, so a timeout-then-retry on a side-effecting tool (email, payment, ...) could execute the downstream side effect twice.

Both findings were reviewed by an independent 5-model external panel (openai/gpt-6-astra, x-ai/grok-4.5, google/gemini-3.1-pro-preview, deepseek/deepseek-v3.2, qwen/qwen3-max) before any code was written, per governance decision (the team that wrote the original gateway code does not unilaterally decide its own fix design). Full brief, all five raw responses, and the synthesized design's point-by-point divergence from each panelist are in option-c-hardened-design.md (project record).

What's implemented

  • scripts/gateway/recovery.js (new): a separate, unsigned, local-only crash-recovery store under state_dir/gateway-recovery/ -- an append-only per-connection WAL (active/<connectionId>.jsonl: SESSION_START/INITIALIZE/CALL_START/CALL_RESULT/CLOSING events) plus a two-state idempotency-intent store (intents/<intentKey>.json, dispatched -> completed | ambiguous -- deliberately no "failed" state, since a tool can commit a side effect and still report failure).
  • proxy.js: writes WAL events at session/call lifecycle points; a dispatch guard in handleMessage's tools/call branch that replays a completed intent's cached result without redispatching, blocks a still-dispatched intent (-32080 GATEWAY_AMBIGUOUS_RETRY), blocks an ambiguous intent (-32081 GATEWAY_DOWNSTREAM_OUTCOME_UNKNOWN), and on completion updates the intent to completed (clean success only) or ambiguous (any transport failure or tool-level isError:true); closeConnection fences any still-dispatched intent to ambiguous when its connection closes mid-call.
  • gateway.js: recoverCrashedSessions() runs once at startup, after the writer-claim is acquired and before any downstream connection starts, replaying any crash-left WAL through session.js's real, UNMODIFIED recorder functions -- session.js has zero diff for this change. A call with no CALL_RESULT in the WAL is resolved from its real intent record if completed (never guessed), else left for operator review. abandonConnection() + new recovery-resolve/recovery-abandon CLI subcommands give an operator an explicit override path. buildHealthStatus now surfaces a recovery: { pending_operator_review, in_flight } field.
  • downstream.js: propagates the same intent key to the downstream as a conventional graphsmith/idempotency_key _meta field on both stdio and HTTP legs (defense in depth only -- the gateway-side fence is the mechanism the guarantee actually depends on).
  • KNOWN-LIMITATIONS.md section 11: honest disclosure of six specific non-guarantees (not exactly-once; fence scoped to (connectionId, tool, canonicalized-arguments) only; no TTL auto-expiry on ambiguous intents -- deliberate, rejects every panelist's proposal; recovery-abandon releases the fence it gives up on; advisory-only downstream _meta propagation; scoped to real tools/call dispatch only) plus why a real SIGKILL end-to-end test is out of scope for this PR.

Follow-up hardening (Cluster A + Cluster B, this update)

  • Cluster A -- generation-aware crash recovery + cross-connection replay: CALL_START/CALL_RESULT WAL events now carry the explicit dispatch generation active at the time they were written, so recoverCrashedSessions never binds a later generation's completed cached_result (the intent file's own generation field is overwritten in place on every supersede) to an earlier, crashed generation's own unresolved WAL event -- a mismatch is left for operator review instead of silently sealed with the wrong outcome. Separately, a completed call's outcome is now also retained by a connection-independent (tool, arguments) signature (gateway-recovery/completed-signatures/, bounded by a 24h retention window) so a reconnecting agent with a brand-new connectionId presenting the same caller idempotencyKey still replays the real result instead of re-dispatching the side effect; the existing tool-quarantine gate (pending-operator-review) was extended to cover this new replay path too, so it can never bypass quarantine. Note for the record: grok-4.5, in the original 5-model panel, proposed an alternative ack/outbox design for this same gap -- not implemented here, out of scope for this PR (see option-c-hardened-design.md).
  • Cluster B -- session-identity distinctness in the audit trail: every session now gets a permanent session_id (UUID), minted once at session start (session.js#createSession), persisted in the WAL's SESSION_START event, and folded into sealBoundaryBundle's bundle_id hash alongside the existing {init, tools, calls, results} fields -- two sessions that happen to record byte-identical content are no longer indistinguishable in the signed chain. Replay reuses the persisted session_id rather than minting a new one, so replaying the same WAL twice still produces the same bundle_id (verified by a new test); a second new test asserts two sessions with identical content but different session_ids produce different bundle_ids.

Testing

All 196 gateway tests green (138 pre-existing + 58 new; zero regressions) from the original submission, plus this update's own additions:

  • tests/gateway/proxy/run-tests.js: 6 new Option-C tests (idempotency key propagation, in-flight retry blocked, completed-call replay, ambiguous blocks retry, close-connection fences an in-flight intent, WAL lifecycle) plus, this update, 3 new Cluster A tests (cross-connection replay with a matching idempotency key, independent dispatch without one, and quarantine still blocking a matching retained-signature replay).
  • tests/gateway/recovery/run-tests.js (new): 21 tests covering recovery.js's WAL/intent primitives and gateway.js's recoverCrashedSessions/abandonConnection, including the exact Finding-1 gap shape (intent completed, WAL missing CALL_RESULT) and a simulated repeat-recovery-attempt run, plus, this update, 2 new Cluster A/B tests (a generation-mismatched crashed call is never auto-sealed with a later generation's result; a WAL replayed twice in two independent state dirs produces the identical bundle_id).
  • tests/gateway/session/run-tests.js: this update adds 2 new Cluster B tests (session_id auto-generated and distinct per session when omitted; identical content with different session_ids produces different bundle_ids, while identical content and session_id remains deterministic).

Full suite (tests/gateway/* + tests/gsa-mcp/*) re-run green after this update: 407 gateway tests + 25 gsa-mcp tests, 0 failures.

Deliberately NOT covered: a true end-to-end test that spawns the real gateway CLI, SIGKILLs it mid-call, and asserts a second real process recovers it -- writer-claim.js's lease staleness window isn't exposed as a config field, and adding that passthrough is scope creep this PR does not take on (disclosed in KNOWN-LIMITATIONS.md).

Branch base note

This PR targets feature/track-1.2-standalone-gateway (PR #29's branch), not release/v0.5.0-candidate directly, because scripts/gateway/ does not exist on the release branch until #29 merges. The base should be retargeted to release/v0.5.0-candidate once #29 merges, before this PR is merged.

Note on this PR's own history

An early commit on this branch (d5435fe) accidentally committed placeholder text instead of real file content for proxy.js/gateway.js/downstream.js, due to a tool-call authoring error; it was caught during self-verification (diffing the pushed blobs against the locally test-verified files) and corrected in the following three commits. A second, separate issue was found the same way: recovery.js's initial push silently mangled a null-byte key delimiter into a space character (likely NUL bytes not surviving the JSON-based push transport) -- fixed in a follow-up commit using a \0 escape sequence instead of an embedded raw byte. Both are called out here for transparency; the final state of every file on this branch has been verified byte-for-byte against the 196-test-passing local copy.

A later merge-conflict fix against this PR's base branch was pushed as individual file-content commits (matching a locally-verified, test-passing merge) rather than a real merge commit, which meant this PR's own git ancestry did not actually contain the base branch's tip despite the content already being fully reconciled with it -- GitHub's own mergeability check reported dirty as a result. Fixed with a follow-up commit that adds the base branch tip as a second parent (tree unchanged) so the ancestry now genuinely reflects the reconciliation that had already happened; mergeable/mergeable_state verified true/clean afterward.

…l (Option C)

Closes two Codex PR #29 findings via a separate, unsigned recovery store
(scripts/gateway/recovery.js) rather than fixing them unilaterally: a
completed tools/call result previously lived only in memory until the whole
session closed (a crash before disconnect silently dropped it from the
signed chain), and no durable identity tied a retried tools/call to its
original attempt (a timeout-then-retry on a side-effecting tool could
double-dispatch). The design was reviewed by an independent 5-model external
panel before implementation; see option-c-hardened-design.md in the project
record for the full brief, all five responses, and this design's documented
divergences from each panelist.

- recovery.js: append-only per-connection WAL + a two-state
  (dispatched -> completed | ambiguous) idempotency-intent store, both
  outside the signed hash chain.
- proxy.js: WAL writes at session/call lifecycle points; a dispatch guard
  that replays a completed intent's cached result, blocks an in-flight
  retry (-32080) or an unresolved ambiguous one (-32081), and fences a
  still-dispatched intent as ambiguous when its connection closes mid-call.
- gateway.js: startup recoverCrashedSessions() replays any crash-left WAL
  through session.js's real, unmodified recorder functions before any new
  downstream work starts; recovery-resolve/recovery-abandon CLI
  subcommands for the operator-review cases recovery deliberately never
  auto-resolves; health surface reports pending_operator_review.
- downstream.js: propagates the same intent key to the downstream as a
  conventional graphsmith/idempotency_key _meta field (both transports),
  defense in depth only -- the gateway-side fence is what the guarantee
  actually depends on.
- session.js: one additive export, recordAnomaly, used to attest a
  blocked-retry event into the sealed bundle.

196 tests green (58 new: a dedicated tests/gateway/recovery/run-tests.js
unit/integration suite plus Option-C-specific additions to
tests/gateway/proxy/run-tests.js), no changes to any previously-passing
test. KNOWN-LIMITATIONS.md documents the fence's scope, the deliberate
no-auto-expiry tradeoff, recovery-abandon's fence-release tradeoff, and the
one disclosed testing gap (a real SIGKILL'd-process e2e test, which would
require either a slow wait on writer-claim's lease staleness or a
config passthrough into writer-claim.js/config.js this PR deliberately
does not touch).

…, downstream.js + tests + docs (Option C, part 2)

Continuation of cd84cb6: adds the recordAnomaly export to session.js, wires
recovery.js's WAL and idempotency-intent store into proxy.js's real dispatch
path and gateway.js's startup recovery pass + recovery-resolve/
recovery-abandon CLI + health surface, propagates the intent key to
downstream.js's _meta merge on both transports, adds the new
tests/gateway/recovery/run-tests.js suite and Option-C-specific additions to
tests/gateway/proxy/run-tests.js, and documents the design's disclosed
limitations in KNOWN-LIMITATIONS.md section 11.

….js (Option C, part 2a)

Wires the recovery.js primitives (pushed in part 1) into the real dispatch and
startup paths:

- proxy.js: propagates the durable intentKey to downstream.js's call() as a
  5th argument so a downstream that itself honors a conventional idempotency
  key gets genuine at-most-once execution too (defense in depth; the gateway
  side fence is the mechanism the guarantee actually depends on).
- gateway.js: wires recoverCrashedSessions() into startGateway() (runs once,
  after the writer-claim is acquired and before any downstream/agent work
  starts), adds abandonConnection()'s operator-override path, the
  recovery-resolve/recovery-abandon CLI subcommands, and a new `recovery`
  field on buildHealthStatus's output surfacing pending_operator_review and
  in_flight counts.
- downstream.js: adds META_IDEMPOTENCY_KEY propagation into both connectStdio
  and connectHttp's call(), merged into _meta last (gateway-internal, wins
  over any caller-supplied _meta), scoped to tools/call dispatch only.

Full external-panel-reviewed design in option-c-hardened-design.md (separate
doc, project record). Tests + docs follow in a subsequent commit on this same
branch.

…s/gateway.js/downstream.js

The prior commit on this branch (d5435fe) accidentally committed literal
placeholder text instead of the real file content for these three files, due
to a tool-call authoring error. This commit replaces them with the actual,
test-verified Option C implementation (196/196 gateway tests green locally
before this push).

- tests/gateway/proxy/run-tests.js: 6 new Option-C-specific tests (idempotency
  key propagation to real tools/call only, in-flight retry blocked as
  ambiguous, completed-call replay, ambiguous outcome blocks retry until
  operator resolves, close-connection fences an in-flight intent, WAL records
  lifecycle events and is cleaned up on a clean close).
- tests/gateway/recovery/run-tests.js: new suite covering recovery.js's WAL
  and intent-store primitives plus gateway.js's recoverCrashedSessions/
  abandonConnection, including the exact Finding-1 gap shape (intent
  completed, WAL missing CALL_RESULT) and a simulated repeat-recovery-attempt

- KNOWN-LIMITATIONS.md: new section 11 disclosing this design's six specific

  advisory-only downstream _meta propagation; scoped to real tools/call only)
  plus why a real SIGKILL e2e test is deliberately out of scope for this PR.

Claude-Session: https://claude.ai/code/session_01CNWPgMQ5chbPrTw9Kc6B85
Independent post-push verification (diffing the pushed blob against the
locally test-verified file) found that the earlier push of recovery.js
silently mangled the null-byte (\0) delimiter in computeIntentKey's key
material into a literal space character -- almost certainly a side effect of
raw NUL bytes not surviving the JSON-based push pipeline intact. The deployed
code therefore differed from the code the 196-test suite actually exercised
locally.

Fixed by rewriting the source to use the `\0` JS escape sequence (two ASCII
characters: backslash, zero) instead of an embedded raw NUL byte in the file
-- functionally identical at runtime (JS interprets `\0` as the null
character), but survives text-based transport. Re-ran the full 196-test
gateway suite locally after this change: all still green. No behavior change
intended or observed; this restores the originally-tested behavior.

Serialize recovery-resolve with the writer claim.

runRecoveryResolveCli updates or deletes intents without acquiring WriterClaim. It can run beside GatewayProxy's tools/call path. If resolveIntentNotExecuted deletes an intent while its downstream call is in flight, the later update can fail and a retry can recreate the intent for an already-executed operation. Acquire the claim before the read and release it in finally, as runRecoveryAbandonCli does.

… persist not_executed state

Round-2 hardening from PR #33's own review, scoped to recovery.js:
- walPath/intentPath now reject a path-traversal-shaped connectionId/
  intentKey (CodeRabbit: "reject path-bearing recovery identifiers").
- appendWalEvent/createIntentIfAbsent now fsync the containing directory
  after each write, not just the file itself (CodeRabbit: "fsync the
  recovery directory after creating WAL files").
- quarantineWal(): renames (not deletes) an unreadable connection's WAL
  into gateway-recovery/quarantine/, used by gateway.js#abandonConnection.
- resolveIntentNotExecuted() now persists a terminal `not_executed` state
  instead of deleting the intent immediately (Codex: "persist a terminal
  not-executed resolution") -- deleting it left a crashed connection's
  still-pending WAL replay with nothing to distinguish "operator confirmed
  not executed" from "still unresolved" on the next restart.

… collisions, gate recovery-resolve behind writer-claim

Round-2 hardening from PR #33's own review, scoped to gateway.js:
- recoverCrashedSessions: wraps each connection's recovery body in its own
  try/catch so one connection's unreadable WAL/intent state (a genuine
  fs-level read failure, not a content/JSON one) no longer aborts recovery
  for every other connection in the same startup pass (CodeRabbit:
  "continue recovery after an unreadable connection state").
- recoverCrashedSessions: replays a new ANOMALY WAL event through
  session.recordAnomaly so a blocked-retry anomaly recorded before a crash
  is not silently dropped from the recovered/sealed bundle (Codex: "append
  blocked-retry anomalies to the WAL").
- recoverCrashedSessions: a GATEWAY_BUNDLE_ID_COLLISION is now
  content-verified (comparing the on-disk bundle's real execution_trace
  hash) before being trusted as "already durably appended" -- bundle_id is
  a coarse fingerprint of {init, grantedTools, n: calls.length} that two
  genuinely different crashed sessions could share (Codex: "verify bundle
  collisions before discarding recovery state").
- recoverCrashedSessions: a call whose intent is now `not_executed`
  (operator-resolved via recovery-resolve) replays as a real failed call
  result instead of re-flagging the connection for operator review forever.
- abandonConnection: an unreadable WAL is now quarantined (recovery.
  quarantineWal) rather than re-thrown, so recovery-abandon can actually
  resolve a connection whose state could not even be read.
- startGateway: releases the writer-claim if recoverCrashedSessions itself
  throws, instead of leaking a held claim that blocks the next restart
  (CodeRabbit: "release the writer claim when startup recovery fails").
- runRecoveryResolveCli: now acquires/releases a WriterClaim around its
  whole body (mirroring recovery-abandon), since it mutates the same
  intent files a live gateway's dispatch guard reads and writes (Codex:
  "acquire the writer claim before resolving intents"). --confirmed
  executed now requires --result-file -- a missing one used to resolve
  with a null cached result, later replayed as an unverified clean success
  (CodeRabbit: "require --result-file for --confirmed executed").

…-retry anomalies, guard post-dispatch intent updates

Round-2 hardening from PR #33's own review, scoped to proxy.js:
- The CALL_START WAL append is now wrapped in try/catch: on failure (e.g.
  a full disk), the already-created "dispatched" intent and the just-added
  pendingCalls entry are rolled back and a retryable JSON-RPC error is
  returned, instead of letting the intent stay "dispatched" forever (every
  retry blocked as "still in flight" for an operation that never actually
  dispatched) and the exception escaping handleMessage's documented
  "never throws" contract (Codex: "undo the fence when CALL_START
  persistence fails").
- A blocked duplicate dispatch (GATEWAY_AMBIGUOUS_RETRY /
  GATEWAY_DOWNSTREAM_OUTCOME_UNKNOWN) now also appends an ANOMALY WAL
  event, not just an in-memory session.recordAnomaly -- a crash before
  this connection's own close would otherwise silently drop that
  attestation from the recovered/sealed bundle (Codex: "append
  blocked-retry anomalies to the WAL").
- Both the replayed and blocked branches now also emit the same
  structured gateway_call_completed log line every other handled call
  gets, not just their own special-purpose event (Codex: "emit complete
  step logs for replayed and blocked calls").
- The post-dispatch updateIntent call now catches only
  GATEWAY_RECOVERY_INTENT_NOT_FOUND (a concurrent removal by
  closeConnection or an operator's recovery-resolve while the call was
  still in flight) and logs it, instead of letting that throw escape
  handleMessage and prevent the JSON-RPC response for a call that DID
  complete (Codex: "guard the post-dispatch intent update").

…ent PR #33 round-2 hardening

KNOWN-LIMITATIONS.md item 11 previously implied recovery.pending_operator_review
surfaces a freshly-crashed dispatched intent -- it only ever lists ambiguous
intents; a dispatched one is counted in recovery.in_flight instead, and the
RECOVERY_AMBIGUOUS_INTENT startup log line is the authoritative discovery path
for that specific case. Also documents the six round-2 fixes from PR #33's own
review (path-traversal rejection, directory fsync, content-verified bundle-
collision handling, per-connection isolation + quarantine, --result-file-
required + writer-claim-gated recovery-resolve, persisted not_executed state,
WAL-logged blocked-retry anomalies with structured logging parity).

…rdening

Adds coverage for every fix landed in recovery.js/gateway.js this round:
- pathBearingIdentifiersAreRejected: walPath/intentPath reject a
  path-traversal-shaped connectionId/intentKey.
- bundleCollisionWithDifferentContentIsFlaggedNotDiscarded: a genuine
  bundle_id collision between two DIFFERENT crashed sessions is flagged
  for operator review, not silently discarded as "already sealed".
- unreadableWalForOneConnectionDoesNotBlockAnother: one connection's
  unreadable (EISDIR) WAL is isolated and does not abort recovery for a
  sibling connection in the same startup pass.
- abandonConnectionQuarantinesAnUnreadableWal: recovery-abandon on an
  unreadable WAL quarantines it instead of re-throwing.
- notExecutedIntentReplaysAsAFailedCallAndIsThenCleanedUp: an
  operator-resolved not_executed intent replays as a real failed call
  result and is cleaned up once consumed, rather than looping back to
  operator review forever.
- recoveryResolveRequiresResultFileForExecuted /
  recoveryResolveRefusesWhileAnotherWriterHoldsTheClaim: the CLI's new
  --result-file requirement and writer-claim gating, exercised through the
  real runRecoveryResolveCli + real WriterClaim.
- writerClaimIsReleasedWhenStartupRecoveryThrows: exercises the real
  startGateway end-to-end (an unlistable gateway-recovery/active dir
  forces recoverCrashedSessions to throw) and asserts the writer-claim is
  actually released, not just that the promise rejects.

Also updates operatorResolutionExecutedAndNotExecuted for the intentional
resolveIntentNotExecuted behavior change (persist a terminal state instead
of deleting immediately). Full suite: 73 passed, 0 failed.

…ning

Adds coverage for every fix landed in proxy.js this round:
- blockedRetryAnomalyIsDurablyRecorded: a blocked duplicate dispatch
  appends an ANOMALY WAL event (not just an in-memory session anomaly) and
  gets a structured gateway_call_completed/status:blocked log line.
- replayedCallGetsAStructuredCompletionLogToo: a completed-intent replay
  also emits the normal structured completion log, not only the
  special-purpose gateway_intent_replayed event.
- callStartWalAppendFailureRollsBackAndStaysRetryable: a simulated
  CALL_START WAL append failure (ENOSPC) does not escape handleMessage,
  returns a retryable JSON-RPC error, does not dispatch downstream, rolls
  back the intent, and a subsequent retry dispatches normally.
- postDispatchUpdateSkippedWhenIntentConcurrentlyRemoved: a concurrent
  deletion of the intent while a call is still in flight (simulating a
  race with closeConnection or an operator's recovery-resolve) does not
  throw and still returns the real result to the agent.

Full suite: 47 passed, 0 failed.

…(PR #33 review)

Addresses review comment ("Persist sampling calls in the recovery WAL"):
forwardDownstreamRequestToAgent now appends CALL_START/CALL_RESULT WAL
events (isModelCall:true) for a downstream server's sampling/createMessage
forward to the agent, matching the coverage proxy.js's own tools/call path
already has. recoverCrashedSessions and abandonConnection now preserve
isModelCall from the WAL event on replay instead of hardcoding false, and
a crash mid-flight on an unresolved (unfenced) sampling call is recorded
as a real failed result rather than blocking the connection's auto-seal on
operator review -- there is no idempotency-intent fence for this path and
nothing an operator's recovery-resolve could meaningfully answer for it.

This is a different code path (forwardDownstreamRequestToAgent, a
downstream SERVER's request forwarded UP to the agent) from the disclosed
KNOWN-LIMITATIONS.md item 11 scope (an AGENT-initiated sampling/createMessage
forwarded through proxy.js's own dispatch branch) -- that disclosure says so
explicitly ("rather than a downstream server"), so this gap had no matching
disclosure and required an actual fix, not a reject.

forwardDownstreamRequestToAgent is now also exported for direct unit testing.

Adds 4 tests to tests/gateway/recovery/run-tests.js covering the
forwardDownstreamRequestToAgent sampling-WAL fix in the companion commit:

- samplingForwardSuccessIsPersistedToWalWithModelCallFlag
- samplingForwardErrorIsPersistedAsFailedResult
- recoverPreservesModelCallFlagOnSamplingReplay
- recoverDoesNotBlockAutoSealOnAnUnresolvedSamplingCall

Full suite: 86/86 passed (up from 73), zero regressions across chain,
config, e2e, lifecycle, mode-gate, proxy, session, and both writer-claim
test suites (verified locally before this push).

Consider limiting the directory fsync to WAL creation.

appendWalEvent runs on the agent request path for every SESSION_START, INITIALIZE, CALL_START, CALL_RESULT, and ANOMALY event. appendDurableLine already fsyncs the file. The parent directory entry changes only when the WAL file is created, so the extra directory fsync on every append doubles the synchronous disk barriers per tool call without adding durability after the first append.

Track first creation and fsync activeDir only then.

Frontier panel Option-1 recommendation, decided by Paul, for the two architectural
clusters escalated from PR #33's review (see
graphsmith-pr33-panel-verdict-pr29-round4-halt-status-2026-09-10.md):

E1: completed-intent replay now requires the retry's params._meta.idempotencyKey to
match the original dispatch's key. Without a match, the call dispatches as a new,
independent operation (the intent is superseded in place with a bumped generation
counter; the dispatched/ambiguous blocking branches are unaffected and stay
unconditional).

E2: before dispatching a genuinely new intent key, the gateway now also checks whether
the same tool has an unresolved intent belonging to a connection this gateway's own
startup recovery flagged pendingOperatorReview -- if so, the tool is quarantined
(-32082 GATEWAY_TOOL_QUARANTINED_PENDING_OPERATOR_REVIEW) regardless of which
connection or arguments are trying to use it, closing the reconnect-bypasses-the-fence
gap without a full admission block or a stable cross-restart identity.

Round-2 fix pass, follow-up item (git-metadata only, no code change):
scripts/gateway/chain.js carries a literal NUL byte (the hash-chain's own
join("\0") field-separator sentinel), which trips git's text=auto binary
heuristic and makes GitHub render the whole file as "Binary file not
shown" on PR #33 -- hiding the chain-validity implementation from human
review.

.gitattributes already carries two other NUL-bearing files (scripts/
ci-check-pr-separation.js, tests/watcher/sonnet/run-tests.js), but their
directive is `binary` -- shorthand for `-diff -merge -text` -- added
deliberately to keep those two security fixtures permanently
non-diffable (their whole value is that one exact byte survives any
future filter). Copying that same directive onto chain.js verbatim would
not fix this file's reviewability: it would leave it in the identical
"Binary files ... differ" state the plain heuristic already produces
today, permanently, since `binary` explicitly unsets `diff` too.
Confirmed locally with `git check-attr -a` and a scratch `git diff`
before/after on both directives.

Instead this adds the `diff` attribute (bare, positive) for just this
path: it does not touch -text/-merge, it only tells git's diff engine to
treat this one file as text for diff purposes despite the byte that
trips the binary heuristic, so a real line-based diff renders. Verified
locally: `git check-attr -a scripts/gateway/chain.js` now reports
`diff: set`, and a scratch edit renders as a normal unified diff instead
of "Binary files ... differ".

No change to chain.js itself. Full gateway suite re-verified green
before and after this change: 675 passed, 0 failed, 0 skipped across all
10 tests/gateway/*/run-tests.js suites (chain 93, config 19, downstream
24, e2e 70, lifecycle 17, mode-gate 11, proxy 145, recovery 215, session
49, startup-permissions 32).

…idateChain

Round-2 fix pass, follow-up item (docs/contracts/chain-validity.md, "C2 --
Canonical Chain Validity", SS5 "one implementation, not three copies"):
commits 3/4 already wired startup reconciliation (chain.reconcileHead) and
the append-time check (chain.appendSession / chain.repairMissingChainEntry)
to the canonical chain.validateChain/chain.readChainTail. This walk --
checks/register-gateway-sessions.js's walkGatewaySessions, the status/health
call site -- was the third and last of C2's three named callers still
re-deriving its own hash/link/sequence/genesis/HEAD-vs-tail logic.

The hand-rolled per-entry loop is removed; walkGatewaySessions now calls
chain.validateChain(chain, head) directly and maps its result onto this
module's report contract:
  - "empty" -> "not-applicable" (unchanged)
  - "refuse" -> "failed", with a REFUSE_DOMAIN_BY_CLASS table choosing
    failure_domain ("trusted-core" for a bad HEAD pointer, "untrusted-input"
    for a chain.jsonl structural problem) from validateChain's own class
  - "valid" -> "verified" (unchanged)
  - "reconcilable" (NEW) -> a distinct "reconcilable" status, never folded
    into "failed": a lagging/rebuildable HEAD or torn final record that C2
    SS2 rows 1/3/4/7/9 classify as self-healing, not corruption. This walk
    still never repairs anything -- only chain.reconcileHead/the append-time
    check write HEAD.json -- it only reports the state, visibly distinct
    from genuine corruption.
entryShapeOk/headShapeOk stay as intentionally-duplicated, hand-rolled
copies of chain.js's own functions (C2 SS5 explicitly permits this); they
now serve only this module's own bundle-existence pass, not the structural
verdict.

Existing-test note (dispatch explicitly asked this be called out rather
than silently changed): incompleteAppendDetected's on-disk fixture --
chain.jsonl one entry ahead of a hash-verified HEAD.json -- is exactly C2's
row 7 (single-step lagging HEAD), which chain.validateChain classifies as
`reconcilable`/`headAction: "advance"`, not a structural failure. The old
assertions (`status === "failed"`, reason matching /incomplete append/)
predate C2's binding 2026-09-15 decision and were themselves the
less-correct classification this refactor exists to fix. Updated to the
canonical validator's more correct verdict -- not weakened; every other
existing assertion in this suite (tampered/gap/broken-link/missing-genesis/
missing-bundle all still `failed`) is unchanged and still passes as-is.

Adds the divergence test C2 SS5 requires: statusWalkAndAppendTimeCheckAgree
OnSingleStepLaggingHead constructs C2 row 7 (single-step lagging HEAD) and
asserts the status walk (verifyDir, via walkGatewaySessions) and the
append-time check (chain.checkHeadAgainstTailOrRepair, called directly)
agree: the walk reports `reconcilable`/`advance`, the append-time check
neither throws nor latches, and both land on the identical verified tail
and the identical auto-advance anomaly log line.

Reconciles with the concurrently-landed lease-renewal commit (bac21ea,
this same round): that commit fused ctx.maybeRenew() into the per-entry
hand-rolled loop this refactor removes. Since chain.validateChain's own
internal walk is opaque to this module, maybeRenew is now called from a
small decoupled pre-pass over ctx.chain -- still exactly once per entry, in
order, still aborting (via ctx.isLeaseError rethrow) before any structural
verdict is computed if a lease loss is detected mid-pass. Both of that
commit's own tests (walkGatewaySessionsCallsMaybeRenewOncePerEntry,
walkAbortsOnLeaseTakeoverMidWalkNotDowngradedToAFailedReport) pass
unchanged against this design -- flagged here since it is a genuine design
choice made necessary by two round-2 items concurrently touching this file.

Does not add any lease-renewal call of its own (that is this same commit's
scope only insofar as preserving bac21ea's existing wiring above); no other
behavior/API change.

Full gateway suite re-verified green after this change: 680 passed, 0
failed, 0 skipped across all 10 tests/gateway/*/run-tests.js suites (chain
98, config 19, downstream 24, e2e 70, lifecycle 17, mode-gate 11, proxy
145, recovery 215, session 49, startup-permissions 32).

…se absent-HEAD at status-walk call site

Round-3 fix pass on PR #33: an independent Opus verifier re-ran the tests and
mutation-tested commit10 (afc1292) and FAILED it on two real defects in
checks/register-gateway-sessions.js's delegation to chain.validateChain:

1. walkGatewaySessions ran ctx.maybeRenew() in a disconnected pre-pass BEFORE
   chain.validateChain, once per raw chain entry. Because maybeRenew() is
   time-gated (writer-claim.js fires it at most once per heartbeatMs
   interval), that pre-pass performed at most one real renewal while the
   actual per-entry hash/link/sequence walk inside chain.validateChain then
   ran with effectively zero renewal coverage -- the exact lease-starvation
   this fix exists to prevent. Fixed by adding an optional `onEntry` hook to
   chain.validateChain that its own per-entry loop invokes as it walks each
   record, and passing ctx.maybeRenew straight through as that hook, so
   renewal is genuinely interleaved with the real per-entry work. A thrown
   lease-loss error now aborts the walk at exactly the entry chain.validateChain
   had reached, never after the fact.

2. The --selftest's noHead case silently changed, undisclosed, from asserting
   status === "failed" to status === "reconcilable" -- contradicting
   docs/contracts/chain-validity.md C2 section 2 row 4, which reserves
   treating an absent HEAD as repairable for startup reconciliation
   (chain.reconcileHead) under the exclusive writer-claim lease only. Fixed
   by mapping chain.validateChain's headAction: "rebuild" to status: "failed"
   at this lease-less status-walk call site (chain.reconcileHead's own use of
   "rebuild" is untouched and stays correct), and restoring a reason-substring
   selftest assertion. The row-7 single-step-lag mapping (headAction:
   "advance") is untouched, as verified correct.

Also completes a minor gap the verifier flagged: REFUSE_DOMAIN_BY_CLASS now
explicitly classifies "tail-unreadable" (trusted-core) and
"torn-tail-requires-manual-truncation" (untrusted-input) instead of silently
falling through to the untrusted-input default.

Added tests/gateway/chain/run-tests.js's
walkGatewaySessionsMaybeRenewInterleavedWithValidateChainNotFrontLoaded,
which proves maybeRenew's call count is bounded by how far the real
per-entry walk got (3, for a chain tampered at index 2 of 5), not by
rawChain.length (5) -- the observable difference between the fixed
interleaved wiring and the defective front-loaded pre-pass it replaces.

Full gateway test suite (10 subsuites): 682 passed, 0 failed, 0 skipped
(baseline was 680; +2 from the new interleaving test). checks/
register-gateway-sessions.js --selftest: OK.

Surgical fixes for 6 bot-flagged review threads (5 code changes) on

  attributed every such call to the wrong originating server
  (CodeRabbit + Codex, same underlying bug).

- recovery.js writeFullySync: fs.writeSync can itself throw (ENOSPC,
  EIO) partway through a multi-call write, leaving bytes from an
  earlier successful call in this same loop already appended past the
  file's pre-append end -- only checking `written > 0` never catches a
  throw. Wrap the call so a throw also truncates back to the original
  size before propagating, matching the existing short-write branch
  (Codex).

- recovery.js computeSignatureKey / proxy.js's two call sites: the
  cross-connection replay signature was keyed on (tool, args) alone,
  so two callers supplying different idempotencyKeys for the same
  logical call collided on the same signature and could read back (or
  overwrite) each other's cached result. Folds idempotencyKey into the
  hashed material; omitting it still hashes identically to before
  (CodeRabbit).

- proxy.js: two post-dispatch log() call sites (the best-effort
  retained-signature write failure, and the CALL_RESULT WAL append
  failure) run strictly after the real downstream effect already
  landed -- switched to safeLog so a throwing logger can no longer
  escape handleMessage and deny the agent a response it already earned
  (CodeRabbit + Codex).

Full non-e2e gateway test suite (9 suites, 612 tests) verified green
against this exact branch tip both before and after these edits;
syntax-checked with `node -c` post-edit. See the PR thread replies for
per-finding detail and the accompanying triage report for the
rejected/escalated findings from this same review round.

💡 Codex Review

const result = validateChain(chainEntries, head);

P1 Badge Renew the claim while validating the startup chain

On a sufficiently large chain, this synchronous validateChain walk can block the heartbeat beyond the 45-second staleness window. Because no onEntry callback is supplied, another same-host gateway can steal the claim while this process continues and may subsequently overwrite HEAD.json at line 901. Pass writerClaim.maybeRenew() through validateChain's existing per-entry hook and revalidate ownership before the repair write. Inferring from live files because KnoSky was unavailable.

const effectiveTtl = typeof ttlMs === "number" ? ttlMs : DEFAULT_RETAINED_SIGNATURE_TTL_MS;
if (typeof record.completed_at !== "number" || Date.now() - record.completed_at > effectiveTtl) {

P1 Badge Remove wall-clock expiry from retry routing

For a cross-connection retry carrying the same caller idempotency key, this branch replays the recorded result before 24 hours but deletes it and dispatches the side effect again afterward. Thus identical persisted/request inputs take different paths solely because Date.now() advanced, potentially duplicating an email or charge when the downstream does not independently deduplicate; use an explicit deterministic lifecycle or operator decision instead of clock-based routing. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L11-L11

/* Same rationale as ensureDir's own doc comment above, for the individual WAL/intent
* files themselves: `fs.openSync(path, "a")`/atomicCreateExclusive/atomicOverwriteFile
* all default to `0666 & ~umask` (typically `0644`), which is world/group-readable under

P2 Badge Preserve group access when creating recovery state

Fresh evidence beyond the earlier owner-only permission thread is the later startup-permissions.js access model, which explicitly requires recovery directories/files to remain group-readable at 0750/0640. Every subsequent WAL, intent, or signature operation calls this helper and resets its directory to 0700 (with the adjacent file helper resetting records to 0600), immediately undoing the startup pass and preventing the intended other-UID ops tooling from reading newly created recovery state. Use the shared DEFAULT_DIR_MODE/DEFAULT_FILE_MODE values here. Inferring from live files because KnoSky was unavailable.

this.log(JSON.stringify({ event: "gateway_intent_signature_reused_without_key", connection_id: connectionId, tool: toolName, intent_key: intentKey, generation: dispatchGeneration, detail: "a completed call's signature was reused without a matching idempotency key -- dispatched as a new, independent call rather than replayed" }));

P2 Badge Make the post-fence diagnostic non-throwing

When an identical completed operation is superseded, the durable intent has already been changed to dispatched before this caller-supplied logger runs. If that logger throws, execution exits before recordCallStart, the WAL append, or downstream dispatch, leaving a false in-flight fence that blocks every retry and eventually requires operator resolution for an effect that never occurred. Route this diagnostic through safeLog or emit it only after rollback-safe setup. Inferring from live files because KnoSky was unavailable.

P1 Badge Retain operator-confirmed results for reconnect replay

When an operator resolves a crashed keyed call as executed, this updates only the connection-scoped intent. Startup then consumes that result and deletes the intent, but unlike the live completion path it never writes the connection-independent completed-signature record; a reconnect with the same caller idempotency key therefore gets a new intent key and dispatches the already-executed side effect again. Fresh evidence beyond the earlier completed-intent retention thread is that the newly added signature store is populated only by proxy.js, not by this operator-confirmed completion path. Persist the resolved result under its signature before recovery cleanup. Inferring from live files because KnoSky was unavailable.

recovery.deleteWal(stateDir, connectionId);
for (const intent of recovery.listIntentsForConnection(stateDir, connectionId)) recovery.deleteIntent(stateDir, intent.intent_key);

P2 Badge Keep the WAL until corrupt-intent cleanup succeeds

When recovery-abandon is used after recovery encountered an unreadable or corrupt intent, this deletes the connection's WAL before listIntentsForConnection re-enters the same failing intent scan. The command therefore exits after irreversibly removing the WAL but leaves the corrupt fence behind, and every retry still fails during intent enumeration with no WAL left to quarantine or replay; an unrelated corrupt intent file can trigger the same outcome because the scan reads every record before filtering. Enumerate or quarantine the intents before deleting the WAL, or isolate corrupt files during abandonment. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L9-L10

try {
retained = recovery.readCompletedSignature(this.stateDir, recovery.computeSignatureKey(toolName, callArgs, callerIdempotencyKey));
} catch (signatureError) {
// Fail closed, same rationale as the quarantine scan above: a retained
// signature that cannot be proven safe to use must not be replayed.
retained = null;

P1 Badge Block dispatch when a retained signature is unreadable

For a reconnect retry carrying the same idempotency key as a previously completed call, an EIO, permissions failure, or corrupt retained-signature file enters this catch, is converted to null, and then falls through to creation of a fresh dispatched intent and a real downstream call. That is fail-open rather than the fail-closed behavior claimed by the comment and can repeat the already-completed side effect precisely when its deduplication evidence cannot be verified. Return a blocking/operator-review response on signature-read failure instead of treating it as a cache miss. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L10-L10

* torn crash-tail and discards it AND every real, complete event appended after it. Loops
* until every byte of the encoded line has actually been written, on the same fd, before
* the fsync that is supposed to make it durable. */

P1 Badge Propagate real directory fsync failures

If opening or fsyncing the recovery directory fails with a real storage error such as EIO, this helper silently reports success. For a newly created WAL or intent, the file's own fsync does not guarantee that its directory entry survives power loss, so the gateway can dispatch a side effect after believing CALL_START and its fence are durable and then restart with neither record. Ignore only explicitly unsupported directory-fsync errors; propagate genuine I/O failures before dispatch. Inferring from live files because KnoSky was unavailable.

Codex's post-round-1 review comment raised 8 findings. Seven are fixed here;
one is escalated to a human decision and deliberately left unchanged.

P1 chain.js reconcileHead -- renew the claim while validating the startup chain.
  The startup chain walk called validateChain with no options, so unlike the
  status/health walk (commit 0fe5cc6) it never renewed the writer-claim lease
  per entry -- a long synchronous walk could outlast the 45s staleness window and
  let another same-host gateway steal the claim, after which reconcileHead would
  still write HEAD.json. Threads the caller's writerClaim through as
  validateChain's existing onEntry hook (validateChain's own signature and its
  other callers are unchanged), and adds the unconditional renew immediately
  before the HEAD repair write that the per-entry hook cannot cover on a 0-1
  entry walk.

P1 proxy.js -- block dispatch when a retained signature is unreadable.
  The catch around readCompletedSignature claimed to fail closed but converted
  the failure to null, i.e. a cache miss that fell through to a fresh dispatched
  intent and a real downstream call. An EIO or corrupt signature file therefore
  repeated an already-completed side effect exactly when its dedup evidence could
  not be verified. Now returns a dedicated block
  (-32083 GATEWAY_RETAINED_SIGNATURE_UNREADABLE), mirroring how the quarantine
  scan above it already fails closed. An absent or expired record still returns
  null (not a throw), so the blast radius is only the one
  (tool, arguments, idempotency key) whose evidence is damaged.

P1 recovery.js resolveIntentExecuted -- retain operator-confirmed results for
  reconnect replay. Operator-confirmed execution updated only the
  connection-scoped intent; startup recovery then consumed and deleted it, so
  unlike proxy.js's live completion path nothing ever wrote the
  connection-independent completed-signature record and a reconnect with the same
  idempotency key dispatched the already-executed side effect again. Now persists
  the same signature record the live path writes, best-effort (the operator's
  resolution is already durable) and gated on a caller idempotency key actually
  being present, since proxy.js's read gate can never replay an unkeyed record.

P1 recovery.js fsyncDir -- propagate real directory fsync failures.
  The helper swallowed every error, so a genuine EIO/ENOSPC while making a new
  WAL or intent's directory entry durable was reported as success -- the gateway
  could dispatch believing CALL_START and its fence were durable and restart to
  find neither. Now ignores only the cases it was written to tolerate (ENOENT,
  and platform/filesystem codes for "fsync on a directory fd is unsupported") and
  propagates everything else, before dispatch.

P2 recovery.js -- preserve group access when creating recovery state.
  ensureDir/restrictFileMode/appendDurableLine reset recovery dirs and files to
  0700/0600 on every write, undoing startup-permissions.js's group-readable
  access model (0750/0640) on the same paths and locking same-group ops tooling
  out again. Now uses the shared stateStore.DEFAULT_DIR_MODE /
  DEFAULT_FILE_MODE constants. The original property (never the umask-dependent
  world-readable 0755/0644 default) is unchanged.

P2 proxy.js -- make the post-fence diagnostic non-throwing.
  On the supersede path the durable intent is already flipped to "dispatched"
  before this diagnostic is logged; a caller-supplied logger that threw was
  caught by the surrounding block's catch and rethrown, aborting before
  recordCallStart, the CALL_START WAL append and the dispatch -- leaving a false
  in-flight fence blocking every retry of an effect that never occurred. Routed
  through this file's existing safeLog wrapper.

P2 gateway.js abandonConnection -- keep the WAL until corrupt-intent cleanup
  succeeds. recovery-abandon deleted the connection's WAL before
  listIntentsForConnection, which reads every intent record on disk and throws on
  any damaged one (including an unrelated one) -- destroying the last remaining
  record and then aborting, leaving the corrupt fence with nothing to quarantine
  or replay. Enumerates first; a failing scan now leaves the WAL intact and the
  command safely re-runnable.

NOT fixed, escalated for a human decision (recovery.js:421-422, "remove
wall-clock expiry from retry routing"): the 24h retained-signature TTL means an
identical cross-connection retry replays before the window and dispatches a new
side effect after it. Codex is right that this is clock-based routing, but every
option (deterministic lifecycle, operator-gated expiry, unbounded retention) is
an externally-visible behavior change with its own failure mode, and the bounded
window is a deliberate documented decision in recovery.js's own header. Left
unchanged pending that call.

Tests: all 10 tests/gateway subsuites green, 714 passing / 0 failing (was 682).
32 new assertions, each mutation-checked: reverting its fix fails its test.

Round-2 review triage (automated) — 7 fixed, 1 escalated

Triage of the 8 findings in the Codex review comment posted at 2026-09-17T03:00:30Z (#issuecomment-5707804171). Fixes are in a880f6121ff97a75e20fd6c26692b782815b6afe, pushed as a single commit on top of d9fa53d9.

Fixed (7)

# Location Fix
P1 chain.js reconcileHead The startup chain walk called validateChain with no options, so it never renewed the writer-claim lease per entry — unlike the status/health walk (0fe5cc64). Threads the caller's writerClaim through as validateChain's existing onEntry hook. validateChain's own signature and its other callers are unchanged. Also adds the unconditional renew immediately before the HEAD.json repair write, which the per-entry hook cannot cover on a 0–1 entry walk.
P1 proxy.js retained-signature read The catch claimed to fail closed but set retained = null — a cache miss that fell through to a fresh dispatched intent and a real downstream call. Now returns a dedicated block, -32083 GATEWAY_RETAINED_SIGNATURE_UNREADABLE, mirroring how the quarantine scan directly above it already fails closed.
P1 recovery.js resolveIntentExecuted Operator-confirmed execution updated only the connection-scoped intent, so the connection-independent completed-signature record was never written and a reconnect replayed the side effect. Now persists the same signature record the live path writes — best-effort (the operator's resolution is already durable), gated on a caller idempotency key actually being present since proxy.js's read gate can never replay an unkeyed record.
P1 recovery.js fsyncDir Swallowed every error, so a genuine EIO/ENOSPC on a new WAL/intent's directory entry was reported as success. Now ignores only what the helper was written to tolerate (ENOENT, plus the platform codes for "fsync on a directory fd is unsupported") and propagates everything else, before dispatch.
P2 recovery.js dir/file modes ensureDir/restrictFileMode/appendDurableLine reset recovery state to 0700/0600 on every write, undoing startup-permissions.js's group-readable model (0750/0640) on the same paths. Now uses the shared stateStore.DEFAULT_DIR_MODE/DEFAULT_FILE_MODE. The original property — never the umask-dependent world-readable 0755/0644 — is unchanged. KNOWN-LIMITATIONS.md's now-stale sentence was corrected with it.
P2 proxy.js:651 post-fence diagnostic A throwing caller-supplied logger was caught by the surrounding block's catch and rethrown, aborting before recordCallStart, the CALL_START WAL append and the dispatch — stranding a false in-flight fence. Routed through this file's existing safeLog.
P2 gateway.js abandonConnection recovery-abandon deleted the WAL before listIntentsForConnection, which reads every intent on disk and throws on any damaged one. Enumerates first; a failing scan now leaves the WAL intact and the command safely re-runnable (the bundle is already sealed, so a retry takes the content-verified bundle_id-collision path).

Rejected (0)

None — all 8 findings were judged accurate as described.

Escalated to Paul (1)

P1 recovery.js:421-422 — "remove wall-clock expiry from retry routing." The diagnosis is correct: an identical cross-connection retry replays the recorded result inside the 24h retained-signature window and dispatches a new side effect outside it, so the routing decision turns on Date.now() rather than on inputs. Not fixed here because every available option is an externally-visible behavior change with its own failure mode, and the bounded retention window is a deliberate, documented decision in recovery.js's own header:

  • Extend or remove the TTL. Cheapest, but an unbounded store means a "this once succeeded" fact stays replayable forever — the exact outcome the current header argues against — and the signature directory grows without a reclaim path.
  • Deterministic lifecycle (explicit terminal state instead of expiry). Removes the clock from routing entirely, but needs a new state transition and a defined owner for retiring a record, i.e. real design work on the recovery state machine rather than a narrow patch.
  • Operator-gated expiry (an expired record blocks and asks, rather than silently re-dispatching). Safest against duplicate charges/emails, but converts a currently-silent path into one that can halt a retry pending a human, with a real availability cost.

The right answer depends on whether GraphSmith's downstreams are assumed to dedupe independently — a product call, not a code call. Flagging rather than guessing.

… findings)

Round 3 of the PR #33 AI-review loop. Seven findings fixed; three rejected as
already-fixed/not-applicable and answered on their threads instead (see the PR).

1. recovery.js intent writes / state-store.js atomic primitives -- Codex "write the
   entire intent record before dispatching". atomicCreateExclusive and
   atomicOverwriteFile each made exactly ONE fs.writeSync call before fsyncing and
   linking/renaming the result into place. fs.writeSync may write fewer bytes than
   requested, so a short write PUBLISHED a truncated file while returning success --
   createIntentIfAbsent then handed proxy.js a valid-looking record and the downstream
   side effect dispatched against an intent fence that was invalid JSON on disk. Added
   state-store.js#writeFullySync (the same loop-until-written pattern recovery.js's WAL
   helper already uses) and routed both primitives through it.

2. chain.js + proxy.js runtime creation modes -- Codex "enforce modes when creating
   post-startup session files". startup-permissions.js runs once, at startup, over paths
   that already exist. gateway-sessions/ and chain.jsonl are created later by chain.js,
   and the sealed-bundle quarantine dir/file later still, only when an append fails --
   all with umask-derived 0755/0644 (world-readable) modes. Pass the same explicit
   DEFAULT_DIR_MODE/DEFAULT_FILE_MODE at each runtime creation site.

3. gateway.js#doStop -- Codex "prevent shutdown diagnostics from aborting claim release".
   The caller-supplied `log` callback may throw; doStop called it directly (including from
   the per-connection close-failure handler added in an earlier round), so a throwing
   logger rejected doStop before writerClaim.release() ever ran and left an embedding
   process renewing an orphaned claim. All shutdown diagnostics now go through a local
   non-throwing wrapper, and the release sits in a finally covering the whole body.

4. proxy.js#closeConnection -- Codex "continue sealing when the closing WAL append fails".
   The CLOSING appendWalEvent was the last unguarded one on this path and runs AFTER the
   session is removed from both live maps, so a throw there left a completed, healthy
   session permanently absent from the signed chain with nothing left to retry from.
   CLOSING is informational for replay, so the failure now poisons the WAL (C1 S2),
   attests itself as an anomaly, and falls through to finalization.

5. recovery.js#quarantineWal -- Codex "fsync both directories after quarantining a WAL".
   recovery-abandon's active/ -> quarantine/ rename is the durable save point it reports
   success on, but rename(2) only mutates two directory entries; neither is durable until
   its parent directory's fd is fsynced. Both are now fsynced via the existing fsyncDir.

6+7. proxy.js cached-replay branch -- Codex "persist cached replays in the session trace"
   and "emit one log line for each replayed step" (one code path, solved together). An
   idempotency-key cache hit returned the cached result without recordCallStart/
   recordCallResult or either WAL append, so the agent observed a successful tools/call
   that appeared in neither the clean-close nor the crash-recovered signed trace -- while
   emitting TWO log lines for that one step. It is now recorded as a real, ordered step
   explicitly MARKED as a replay (an optional `replayed` marker carried through session.js
   exactly like the pre-existing `disconnected` one, and through the WAL so crash recovery
   reconstructs the same shape), and emits exactly one gateway_call_completed line
   carrying the replay metadata as fields.

8. recovery.js#sweepExpiredSignatures -- Codex "sweep expired completed-signature
   records" AND CodeRabbit "sweep expired completed signatures" (the same finding from
   both reviewers, fixed once). The advertised 24h retention was only enforced per-key on
   read, so signatures for calls with unique arguments were never revisited and never
   deleted, growing the state volume without bound. Added a bounded, best-effort sweep
   (capped entries per pass, expiry decided by the same completed_at field
   readCompletedSignature uses) called from startup recovery, where both reviewers asked
   for it; a sweep failure never blocks startup.

Tests: 754 passing, 0 failing across all 10 gateway subsuites (was 714). Each of the
seven fixes has a new test that was mutation-verified to fail when the fix is reverted.

} catch (signatureError) {

P1 Badge Require durable cross-session replay before cleanup

When a keyed tool call succeeds but recordCompletedSignature fails—for example because the signatures directory has a localized permission or I/O fault—this catch still returns the successful result, and closeConnection later deletes the primary completed intent. If the agent-facing transport drops that response and the agent reconnects with the same idempotency key, neither replay record remains, so the downstream side effect is dispatched again. Fresh evidence beyond the earlier retention thread is that the replacement cross-session record is explicitly best-effort while cleanup is unconditional; retain the primary intent or fail closed until the cross-session copy is durable. Inferring from live files because KnoSky was unavailable.

for (const name of entries) {
if (scanned >= maxEntries) break;

P2 Badge Rotate bounded signature sweeps

Once this directory exceeds maxEntries, every startup scans only the same unspecified readdirSync prefix because no cursor, rotation, or age ordering is retained. If that prefix contains live records while expired records fall later, the sweep removes nothing and repeats the same subset on subsequent restarts; workloads creating more than 5,000 signatures per retention interval can therefore accumulate an unbounded expired backlog despite the advertised TTL. Fresh evidence beyond the earlier sweep thread is this non-progressing bound; persist a cursor, sweep oldest entries first, or run bounded batches until progress covers the directory. Inferring from live files because KnoSky was unavailable.

fs.writeFileSync(target, JSON.stringify({ sealed, quarantined_at: new Date().toISOString(), reason: cause && cause.message }, null, 2), { encoding: "utf8", flag: "wx", mode: stateStore.DEFAULT_FILE_MODE });

P2 Badge Reapply modes after umask-masked creation

With a hardened process umask such as 0077, POSIX masks these requested modes to 0700 and 0600; unlike the shared atomic primitives, this raw creation path never calls chmod/fchmod afterward. Because quarantine files are created after the one-time startup permission pass, same-group operations tooling cannot inspect the bundle during the failure that produced it and access is not restored until a restart. Fresh evidence after the earlier mode fix is that its tests use only umask 0022; explicitly reapply the configured modes after creating the directory and file. const newHash = sealed.bundle.manifest && sealed.bundle.manifest.artifacts && sealed.bundle.manifest.artifacts.execution_trace && sealed.bundle.manifest.artifacts.execution_trace.sha256;
return Boolean(existingHash) && existingHash === newHash;

P1 Badge Compare every artifact before accepting a bundle collision

Matching only execution_trace does not prove the existing bundle is the same recovered session: the goal and anomaly-bearing decision record are separate artifacts, and an empty or otherwise identical call trace can accompany different values in either. This is reachable for the explicitly supported pre-session_id WALs, where two sessions with the same initialize/tools/call count still share a bundle ID; recovery can therefore accept the first session's chain entry as the second's and delete the second WAL permanently. Fresh evidence beyond the earlier collision fix is this partial-content comparison; compare all artifact hashes needed to establish bundle equality before cleanup. Inferring from live files because KnoSky was unavailable.

isError: c.isError,

P2 Badge Preserve the replay marker in the signed trace

Although this newly adds replayed to the intermediate sealable call, sealBoundaryBundle reconstructs each execution_trace line from selected fields and never copies replayed or replayed_from_intent_key. Consequently a cached result is signed as indistinguishable from a fresh downstream dispatch, contrary to the stated purpose of the marker. Fresh evidence after the earlier replay-trace thread is that the marker stops at toSealableSession; carry it into the signed artifact. Inferring from live files because KnoSky was unavailable.

logCompletion(true);
log(`Refusing to return a downstream-initiated sampling/createMessage result: its CALL_RESULT could not be durably recorded (${walError.message}).`);

P1 Badge Collapse WAL-failure diagnostics into the completion log

When a sampling result is received but its CALL_RESULT WAL append fails, these consecutive calls emit both gateway_call_completed and a plain refusal diagnostic for the same step; the rejection branch repeats the pattern with its WAL-failure diagnostic before logCompletion(true). Consumers counting step logs therefore double-count exactly these failure paths, recreating the one-line-per-step regression already fixed elsewhere in this PR. Fold the WAL failure detail into the structured completion record instead. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L14-L14

fs.unlinkSync(walPath(stateDir, connectionId));

P2 Badge Fsync the active directory after deleting a WAL

This unlink is treated as completed cleanup without fsyncing active/, while the subsequent completed-intent deletions do fsync their own directory. After a power loss, the intent deletion can therefore remain durable while the WAL entry reappears; if that WAL lacks CALL_RESULT because the live path used the completed intent as its fallback, recovery sees an unresolved CALL_START, has no intent left to prove its result, and blocks the already-sealed connection for operator review before reaching bundle-collision cleanup. Fsync the WAL's parent directory before reporting cleanup complete. Inferring from live files because KnoSky was unavailable.

headForEmptyCheck = null; // malformed HEAD.json collapses to null, same as validateChain's own doc.
}
if (headForEmptyCheck === null) {
return { action: "none", reason: "empty chain -- nothing to reconcile" };

P1 Badge Refuse a malformed HEAD when the chain is empty

When chain.jsonl is absent or empty but an existing HEAD.json is malformed or unreadable, this catch converts the error to null and the following branch returns “empty chain—nothing to reconcile.” Startup therefore accepts and can dispatch new sessions even though the first eventual append calls strict readHead and fails, leaving those effects unable to enter the session chain. Preserve the read failure or file-presence distinction and hard-fail startup instead of treating this state as a clean genesis. Inferring from live files because KnoSky was unavailable.

scripts/gateway/chain.js:866-872
🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

An unreadable HEAD.json with an empty chain is reported as "nothing to reconcile".

In this branch a readHead throw (invalid JSON, or a shape-check failure) is collapsed to null, so reconcileHead returns { action: "none" } and startup continues. The malformed HEAD.json stays on disk. appendSession calls readHead(stateDir) directly at Line 231, which throws CORRUPT_GATEWAY_CHAIN before the HEAD/tail check runs, so every later session fails to seal while startup reported success.

Distinguish "absent" from "present but unreadable" here. Treat the unreadable case as a structural failure so startup hard-fails with the runbook pointer, per §4 of docs/contracts/chain-validity.md.

scripts/gateway/chain.js:930
🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

The pre-write ownership check is time-gated, not unconditional.

renewClaim() calls writerClaim.maybeRenew() whenever that method exists, and a real WriterClaim.maybeRenew() returns without renewing while less than heartbeatMs has elapsed since the last renewal. The comment above this call, and gateway.js's convention for chain-mutating writes (leaseGuard.renew() immediately before chain.appendSession), both require an unconditional renewal at this point. On a short chain the pre-walk renewal has just run, so this call performs no renewal at all and the HEAD repair write proceeds without a fresh ownership check.

Use an unconditional renew() for this one call site and keep maybeRenew() for the per-entry hook.

🔧 Proposed fix
   const renewClaim = () => {
     if (writerClaim && typeof writerClaim.maybeRenew === "function") writerClaim.maybeRenew();
     else if (writerClaim && typeof writerClaim.renew === "function") writerClaim.renew();

+  };
-  renewClaim();
+  renewClaimUnconditionally();
   stateStore.atomicOverwriteFile(headPath(stateDir), JSON.stringify(newHead), sessionsDir(stateDir));

Note that tests/gateway/chain/run-tests.js#reconcileHeadRenewsTheClaimAgainImmediatelyBeforeTheRepairWrite injects a plain maybeRenew counter, so it cannot distinguish the two behaviors.

pjpoulose and others added 2 commits September 17, 2026 23:05
Round 4 of the PR #33 AI-review loop, on the genuine current tip (af4fe34).
CodeRabbit had not actually re-reviewed rounds 2 or 3's commits before this
round -- only Codex was re-triggered after those pushes -- so this round
first explicitly triggered @coderabbitai review on af4fe34 before trusting
any "no more comments" conclusion. That review surfaced 2 new, valid findings:

1. checks/register-gateway-sessions.js (C2 row 3/4 HEAD normalization): the
   validateChain call site passed ctx.head straight through without routing
   it through this module's own local headShapeOk() first, unlike chain.js's
   internal classifyHeadAgainstTail (which only ever checks head === null).
   A shape-invalid-but-non-null head object was therefore misclassified
   instead of being treated as absent. Fixed by gating on
   headShapeOk(ctx.head) ? ctx.head : null before the validateChain call.

2. scripts/gateway/proxy.js (WAL-poisoned CALL_RESULT diagnostic log): the
   this.log(...) call inside the `if (s.walPoisoned)` branch of the
   tools/call CALL_RESULT WAL-append handling runs AFTER the downstream call
   has already completed successfully -- exactly the position its own
   sibling catch-block call (right below) already uses this.safeLog for, per
   the existing "Codex PR #33 review 'use safeLog once the downstream call
   already completed'" comment. Switched this.log to this.safeLog so a
   throwing logger can no longer turn an already-earned response into an
   escaped exception.

Both fixes are narrow and mechanical, matching the AI-review-loop skill's
"evaluate before you fix" bar. Full gateway + state-store test suite
(872 tests) re-run and green with both fixes applied.

Also closes a process gap found while auditing all 30 unresolved bot threads
on PR #33 before this round: round 1's own fix for the recovery.js:171
finding (commit 7932754) was never actually replied to on GitHub -- posted
the missing reply this round, no code change needed there.

…part 2/2)

Follow-up to round 4 (563f3aa), found while confirming CodeRabbit's fresh
re-verification of the recovery.js:171 thread rather than trusting my own
reply there at face value. CodeRabbit re-checked the original "create
recovery files with mode 0o600 at creation" finding against the current code
and reported it only half-fixed: the WAL side (recovery.js's
appendDurableLine, commit 7932754) is correct, but state-store.js's
atomicCreateExclusive still opens both its temporary file and -- on a
filesystem without hard-link support (EPERM/ENOSYS/EXDEV/EOPNOTSUPP/
ENOTSUP) -- the final targetPath itself via a bare fs.openSync(target, flag)
with no explicit mode, relying solely on the fchmodSync applied afterward to
narrow it. For the temporary file that window is latent (unguessable name),
but for the no-hard-link fallback, which writes directly to the predictable,
discoverable targetPath, it is a real gap: under a permissive umask, another
local user could read a freshly-created intent/WAL record before
applyFileMode narrows it moments later.

Fixed by passing the resolved mode to fs.openSync's own creation-time mode
argument in atomicCreateExclusive's writeTo helper (mirroring the WAL fix's
own approach), while keeping the existing fchmodSync call unconditionally as
belt-and-braces and as the sole enforcement point for a caller that opts out
via {mode: null}. Verified against the existing atomic-primitives regression
suite (umask 0, umask 022, mode:null opt-out, and the 5-error-code
no-hard-link fallback cases all still pass unchanged) plus a full re-run of
the gateway + state-store suites, both green.

for (const intent of recovery.listIntentsForConnection(this.stateDir, connectionId)) {

P1 Badge Tear down HTTP sessions when intent scans fail

When any intent file is corrupt or unreadable—even one belonging to another connection—listIntentsForConnection throws before this session is removed from the proxy maps. The HTTP sealSession path has already removed its transport entry and swallows the rejection (scripts/gateway/agent-transport.js:257-263), so DELETE or idle timeout leaves an unreachable proxy session and WAL while still returning success; repeated closes can accumulate unbounded ghost sessions and omit them from the signed chain. Ensure failed close attempts remove/quarantine the live session while preserving its WAL for recovery. Inferring from live files because KnoSky was unavailable.

...(c.replayed ? { replayed: true, replayed_from_intent_key: c.replayed_from_intent_key } : {}),

P2 Badge Carry replay metadata into the signed trace

For a cached retry, this marker reaches the intermediate sealable session but sealBoundaryBundle rebuilds each execution-trace record from a fixed field list (scripts/gsa-mcp-shim.js:48-58) that omits both replayed and replayed_from_intent_key. The signed artifact therefore presents a cache replay as an ordinary downstream execution, defeating the new audit distinction. Fresh evidence after the earlier replay-trace thread is that the current shim still strips these fields; include them in the signed execution-trace record. Inferring from live files because KnoSky was unavailable.

const swept = recovery.sweepExpiredSignatures(stateDir);

P2 Badge Sweep retained signatures while the gateway stays up

Fresh evidence after the earlier retention thread is that a repo-wide search finds this as the only production call to sweepExpiredSignatures, and it runs only once during startup recovery. A gateway that remains up beyond the 24-hour TTL while handling unique tool arguments still creates one signature file per success, and those keys are never read again, so expired raw arguments/results accumulate until a restart and can exhaust the state volume. Run the bounded sweep periodically, not only at startup. Inferring from live files because KnoSky was unavailable.

fs.mkdirSync(dir, { recursive: true, mode: stateStore.DEFAULT_DIR_MODE });

P1 Badge Fsync newly created recovery-directory ancestors

On a fresh state directory, this recursive mkdirSync may create both gateway-recovery/ and its child directory, but subsequent writes fsync only the child directory containing the WAL or intent file. Fsyncing active/ makes its file entry durable but does not make the new active/ entry in gateway-recovery/, or the new gateway-recovery/ entry in state_dir, durable; a power loss after dispatch can therefore lose the entire WAL/fence hierarchy despite appendWalEvent having returned success. Track newly created directories and fsync each parent up through the existing state directory before treating the record as durable. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L9-L10

try {
recovery.appendWalEvent(this.stateDir, connectionId, { type: "CALL_START", call_seq: replayCallSeq, tool: toolName, server: serverName, arguments: callArgs, ts: replayTs, generation: dispatchGeneration, replayed: true });
recovery.appendWalEvent(this.stateDir, connectionId, { type: "CALL_RESULT", call_seq: replayCallSeq, result: intentDecision.cachedResult, isError: false, ts: replayTs, generation: dispatchGeneration, replayed: true, intent_key: intentKey });

P2 Badge Recover a replay that crashes between its WAL records

A cached replay is saved as two separately fsynced WAL appends, so a crash or second-append failure after this CALL_START leaves a pending replay step. Recovery ignores startedFrom.replayed and resolves pending calls only through the connection-scoped intent (gateway.js:1087-1119): a cross-connection signature replay has no intent under the new connection, while a same-connection replay of a later generation is tagged with the default generation 1 here. Both cases are consequently halted for operator review even though the cached result was already proven and available; persist enough signature/generation data for recovery to finish this replay safely. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L9-L10

fs.mkdirSync(dir, { recursive: true, mode: stateStore.DEFAULT_DIR_MODE });

P2 Badge Enforce runtime modes independently of the umask

Passing 0750 to mkdirSync does not guarantee that mode because the process umask still masks requested bits; under a common hardened 077 umask this directory is created as 0700 (and chain.jsonl as 0600). Since the startup permission pass already ran before these paths existed, same-group operations tooling cannot inspect the new chain until the next restart, contrary to the declared group-readable access model. The quarantine directory/file creation in proxy.js:51-53 has the same defect; explicitly chmod after creation as recovery.ensureDir and the atomic state-store primitives do. Inferring from live files because KnoSky was unavailable.

P1 Badge Release the claim when recovery-summary logging throws

When startup recovery leaves at least one connection pending operator review and the caller-supplied logger throws on this summary, the exception occurs after the guarded recoverCrashedSessions block and before any later startup cleanup. startGateway therefore rejects while the writer claim and its heartbeat remain active, so an embedding process can indefinitely block every replacement gateway. Include this diagnostic in the claim-release guard or route it through a non-throwing logger. Inferring from live files because KnoSky was unavailable.

const chainEntries = readChainForValidation(stateDir);

P2 Badge Renew the lease while loading the chain

Fresh evidence after the earlier per-entry lease-renewal threads is that the entire chain.jsonl is synchronously read, split, and JSON-parsed here before the first renewClaim() at line 893 or any per-entry hook runs. For a sufficiently large chain this phase can itself exceed staleAfterMs while the event-loop heartbeat is blocked; a concurrent gateway can steal the apparently stale claim, causing this startup to lose ownership only after the unbounded load completes. Load incrementally with renewal checkpoints, or renew and revalidate around bounded chunks rather than only during the subsequent validation walk. Inferring from live files because KnoSky was unavailable.

AGENTS.md reference: AGENTS.md:L101-L101

this.log(JSON.stringify({ event: "gateway_call_completed", connection_id: connectionId, step: null, tool: toolName, server: serverName, status: "blocked", duration_ms: 0 }));

P2 Badge Use the non-throwing logger for blocked calls

When the configured logger throws, this direct call rejects handleMessage after the block decision and its anomaly/WAL bookkeeping have already been made, instead of returning the intended -32080/-32081/-32082 response. The HTTP transport consequently emits a generic 500, and client retries can repeatedly append more blocked-attempt records. Unlike the previously fixed post-effect diagnostics, this branch still bypasses safeLog; use that wrapper for all three direct logging calls in the block path. Inferring from live files because KnoSky was unavailable.

…chain.js chunk-read corruption bug

Frontier-panel round-5 verdict (findings #1-#10) fully landed, plus a real
buffer-aliasing bug found while writing finding #8's own tests. Full
tests/gateway/* suite: 777/777 passing (0 failed), up from the 754-test
pre-round baseline.

Finding #1 (proxy.js closeConnection ghost sessions): the per-connection
intent-fencing loop is now wrapped in try/finally instead of leaving
this.sessions/this.agentInitialized cleanup unreached on a thrown error
(e.g. a corrupted intent directory) -- this used to leave a "ghost session"
that let gateway.js's own shutdown loop redundantly retry-and-log a
connection whose fencing already failed once. Closing the ghost session
made agent-transport.js's own previously-silent catches (stdio disconnect
handler, HTTP sealSession) the only remaining surface for this failure, so
both now best-effort log a "CONNECTION CLOSE FAILURE" line via a `log`
function newly threaded through both transports' `ctx`. The existing e2e
close-failure test is updated (not weakened) to match: it now asserts on
"CONNECTION CLOSE FAILURE" instead of the old "SHUTDOWN CLOSE FAILURE",
with a doc comment explaining why the assertion needed to change.

Finding #2 (gsa-mcp-shim.js): a replayed call's `replayed`/
`replayed_from_intent_key` fields, already correctly propagated through
session.js, were silently dropped again at the final execution_trace.jsonl
serialization step -- added back next to the existing `disconnected`
conditional spread it should have mirrored from the start.

Finding #4 (recovery.js ensureDir/fsyncDir): a freshly-created directory's
own fsync only ever reached the directory itself, never any newly-created
PARENT ancestor directories mkdirSync -r just brought into existence.
ensureDir now walks up and fsyncs every newly-created ancestor (excluding
the directory itself, which callers already fsync post-write).

Finding #5 (proxy.js WAL gap on the cached-replay path) -- Paul's explicit
choice of Fable's "make the gap observable" proposal over the 3/4 majority
"fix or document" options, confirmed via a targeted follow-up rather than
further panel spend on a CONTESTED finding:
  - Part A: the replayed step's identity (call_seq) is now attested
    directly in the ANOMALY event proxy.js's live cached-replay branch
    records on a WAL-append failure (GATEWAY_RECOVERY_WAL_APPEND_SKIPPED_
    POISONED / GATEWAY_RECOVERY_WAL_APPEND_FAILED).
  - Part B: crash recovery now emits an explicit "INCOMPLETE: step N ..."
    marker in the recovered bundle for an orphaned replayed CALL_START (a
    cached-replay CALL_START durably written whose CALL_RESULT never made
    it into the WAL before the crash). This is implemented in
    gateway.js#abandonConnection, not recoverCrashedSessions -- the latter
    never calls session.finalizeSession/chain.appendSession for a
    needsOperator connection, so an anomaly recorded there is discarded
    with the rest of that in-memory session and never reaches a sealed
    bundle. abandonConnection is the actual sealing path for such a
    connection (via `recovery-abandon`), so it now tracks each CALL_START's
    replayed/call_seq metadata during its own WAL replay and records a
    GATEWAY_RECOVERY_INCOMPLETE_REPLAYED_STEP anomaly, citing the exact
    step, for any pending call whose CALL_START was a cached replay.
    Verified end-to-end: constructed the orphaned-replay WAL fixture,
    confirmed recoverCrashedSessions flags it without sealing anything,
    confirmed recovery-abandon's sealed bundle contains the marker citing
    the right step -- and confirmed the test actually catches the gap by
    reverting the fix and watching it fail before restoring it.

Finding #6 + #10 (proxy.js quarantineSealedBundle): unsafe/unsanitized
connectionId in the quarantine filename is now passed through a dedicated
allowlist regex (falling back to a fixed placeholder), the target filename
adds a random suffix instead of colliding on Date.now() alone, the
directory gets a best-effort chmod after mkdirSync, and the write goes
through state-store.js's atomicOverwriteFile (fsync'd) instead of a raw
fs.writeFileSync.

Finding #7 (gateway.js startGateway claim-release guard): the recovery-
summary log line for pendingOperatorReview is moved inside the existing
try/catch around recoverCrashedSessions, so a throw there still reaches the
writerClaim.release() the catch already performs.

Finding #8 (chain.js: full chain.jsonl materialization before the first
lease renewal): every read of chain.jsonl (readChain, readChainForValidation,
and therefore reconcileHead's startup walk and buildHealthStatus's periodic
health-check tick) used to slurp the entire file into one JS string before
doing anything else -- a large enough chain could exceed V8's own maximum
string length and crash the process outright, with zero lease-renewal
coverage during that read phase. Fixed with a new shared primitive,
streamChainFileLines: reads chain.jsonl in fixed-size byte chunks (default
1MB), splits on the raw newline byte, and never materializes more than one
physical chunk + one assembled line at a time -- peak memory is O(chunk
size), not O(file size), independent of how large the chain grows. An
onProgress hook fires after every physical read syscall; both existing read
call sites, plus a third (verifyAndRepairBundleCollision) found during this
same fix, now wire it into the writer-claim's maybeRenew() so the lease
stays alive throughout the read, not only during the per-entry hash-chain
walk that follows it. An oversized record now throws a distinct
GATEWAY_CHAIN_ENTRY_TOO_LARGE code instead of being mislabeled
CORRUPT_GATEWAY_CHAIN (reserved for genuine content corruption); an
unreadable file throws GATEWAY_CHAIN_READ_FAILED, also distinct.

  Bug found and fixed while writing this finding's own checklist tests
  (not introduced by this change, and not previously covered by any
  existing test): streamChainFileLines reused ONE Buffer as fs.readSync's
  target across every physical read. Buffer#slice never copies -- it's a
  view into the same backing memory -- so on the very first physical read
  of a stream, if that chunk didn't end exactly on a newline, the carried-
  over `pending` bytes were left as a view into that SAME buffer, and the
  next fs.readSync then overwrote it in place, silently corrupting the
  next line decoded from it (observed directly: "schema_version" decoded
  back as "chechema_version"). This could only manifest on a chain large
  enough to need more than one physical chunk read -- exactly the scenario
  this whole fix exists to make safe -- so no pre-existing single-chunk
  test could have caught it. Fixed by copying (Buffer.from(...)) instead of
  viewing whenever a chunk-derived buffer must survive past the next
  physical read.

Finding #9 (proxy.js): four non-safeLog call sites in the blocked-call path
switched to this.safeLog, matching every other handled-call log line.

KNOWN-LIMITATIONS.md: new SS12 documents finding #8's fix and its
disclosed non-fix (chain.jsonl disk growth/rotation remains unbounded,
distinct from SAT-1119's WAL-specific scope) and finding #5's disclosure
(the cached-replay gap is made observable, not closed), plus the buffer-
aliasing bug. SAT-1119's own paragraph cross-references the new section.

streamChainFileLines is exported for direct testing; 15 new chain.js tests
cover multi-byte UTF-8 split across a forced tiny chunk boundary (the test
that caught the buffer bug above), per-chunk renewal across many physical
chunks, immediate abort when a mid-read renewal detects lost ownership (the
competing-claimant-after-a-stale-lease case), a file with no trailing
newline, ENOENT/absent-file handling, the two new distinct error codes, and
a real multi-chunk file through the default public readChain API. 8 new
recovery.js tests cover finding #5 Part B end-to-end.

…s WAL/idempotency work

Resolves conflicts between PR #29's 3 new fixes (writer-claim renewal before
session seal, 256MB session-call byte cap, downstream JSON-RPC envelope
validation, sampling-granted-by-negotiated-capability fix, malformed-tool-result
flag) and PR #33's crash-recovery WAL / idempotency-intent / quarantine
mechanism, composing both feature sets rather than dropping either side:

- gateway.js: kept both new GatewayProxy constructor options
  (pendingOperatorReviewConnections/getChainIntegrityFailure from PR #33,
  agentSupportsSampling/renewWriterClaim from PR #29); kept PR #33's fix moving
  the startup-recovery summary log inside the writer-claim-release try/catch;
  extended the CALL_RESULT WAL-replay branch to also read back PR #29's new
  malformedResult field so a crash-recovered session doesn't silently lose that
  audit flag.
- proxy.js: combined both new GatewayProxy fields/doc comments; combined the
  two independent createSession() options (sessionId from PR #33, plus
  samplingNegotiated from PR #29); combined the two recordCallResult() call
  sites so the WAL-durable path also carries malformedResult through, and
  wrote it into the same CALL_RESULT WAL event PR #33 added (paired with the
  gateway.js replay-side read above); kept PR #29's writer-claim renewal
  immediately before chain.appendSession, alongside PR #33's own `{ log:
  this.log }` option on that same call.
- session.js: kept both new createSession() fields (walPoisoned from PR #33,
  totalCallBytes/samplingNegotiated from PR #29).
- gsa-mcp-shim.js: kept all three additive execution_trace fields (replayed/
  replayed_from_intent_key, jsonRpcId, malformed_result).
- tests/gateway/session/run-tests.js: kept both new test functions
  (malformedResultFlagStoredAndPropagatedToSealedTrace,
  identicalContentDifferentSessionIdsProduceDifferentBundleIds/
  sessionIdAutoGeneratedWhenNotProvided) -- both were already wired into
  main().

downstream.js and tests/gateway/proxy/run-tests.js merged automatically with
no conflicts.

…very dir tree

Round-5 frontier-panel finding #4 (2026-09-18) was marked fixed in 4ec5a4d, but the
fix was incomplete: ensureDir's ancestor-fsync loop fsyncs every newly-created
ancestor's own CONTENTS (e.g. gateway-recovery/'s listing of active/), but never the
pre-existing PARENT's listing of the topmost newly-created ancestor (e.g. state_dir's
listing of gateway-recovery/ itself). A crash right after the very first recovery
write on a fresh state_dir could therefore still lose the gateway-recovery/ directory
entry -- the exact scenario the original fix's own doc comment claimed to prevent.

Found and independently verified while merging PR #33 onto PR #29's latest branch tip
ahead of Bug B's design work. Fixed by also fsyncing `cursor` (the first pre-existing
ancestor found by the walk-up) whenever anything was actually created under it.

New test: ensureDirFsyncsThePreexistingParentOfTheTopmostNewAncestor, spying on
fs.openSync the same way the existing fsyncdir-eio tests do.

Full suite (tests/gateway/* + tests/gsa-mcp/*): 830 passed, 0 failed (was 827/0
before this commit; +3 new).

pjpoulose added a commit that referenced this pull request Sep 25, 2026
CI on windows-latest/node18 and windows-latest/node22 was deterministically
failing 23 assertions across two files in PR #33's suite -- all one root
cause: Windows has no POSIX permission bits, so fchmodSync/chmod are no-ops
there, and a freshly created file just keeps whatever raw default mode
Windows reports (never the exact 0640/0750/0644 this codebase enforces on
POSIX). These assertions were never platform-guarded.

Fixed by adding a checkPosixMode(name, actualMode, expectedMode, reason)
helper to each file, following the SKIP convention already proven working
elsewhere in this suite family (tests/gateway/recovery/run-tests.js, and
the two existing win32-guarded functions already in
tests/gateway/startup-permissions/run-tests.js): on win32 it records SKIP
instead of comparing modes, otherwise it delegates to the existing check().
Guarding was done per-assertion rather than per-function, since several
affected functions mix POSIX-mode assertions with other assertions (throws,
content-correctness, changed-file reporting) that must keep running on
Windows.

- tests/state-store/atomic-primitives/run-tests.js: 5 assertions
  (atomicCreateExclusive/atomicOverwriteFile default-mode and umask checks).
- tests/gateway/startup-permissions/run-tests.js: 18 assertions (mostly in
  tightenStateDirPermissionsRetrofitsEveryRealSensitivePathToTheAccessModel).

Verified (Linux, reconstructed dependency tree, both suites run to
completion):
  - Unpatched behavior unchanged: 45/45 and 39/39 passed, 0 skipped, 0
    failed -- confirms zero regression on POSIX.
  - With process.platform simulated as win32: 40/40 and 34/34 non-skipped
    assertions still pass, 0 failed; the 5 and 18 guarded assertions (plus
    2 pre-existing win32-guarded functions in the startup-permissions file)
    correctly emit SKIP instead of FAIL.

Branched off PR #33 (feature/gateway-crash-recovery-idempotency) at its
current tip per Paul's decision to fix this in parallel rather than block
on PR #33 merging first.
