docs(architecture): map the TASK-438 row's own incidents into the long-term review - #2632
This is a follow-up to #2587 (TASK-438). The review there worked from the incident list given with the task. It had not yet been checked against the row's own notes, and Still open said so. This PR maps each incident in those notes to an item, or says why it maps to none. Each incident's PRs were checked on GitHub at 2026-10-24 13:20Z. Docs only, one file: docs/architecture/long-term-review-2026-10.md.
The finding. "Most of tonight's incidents are one fact in several places" was true of the first six incidents and is not true of all twelve. It is four of twelve now. The 90-day hotspots still carry the finding, and no item moves.
Don't, on auth. This row said no incident in the window pointed at auth, and TASK-410 did. chore(auth): add session lifecycle controls #2545 fixed it in place, so the row still keeps auth, and its trigger becomes an incident that needs more than an in-place fix.
#2574 made CI fail a CLI test run that leaves a temp dir. #2572 added the matching backend check (npm run test:temp-cleanup), but no workflow runs it. The doc records this in the temp-dir row. Adding the step is a one-line change to .github/workflows/tests.yml. It is not in this PR.
Gate
Gate: Rhea (docs), the same as #2587. Per TASK-438, the Cofounder reads it for Sam. It also needs CodeRabbit clean and CI green.
…g-term review
The review shipped in #2587 working from the incident list given with
the task. It had not yet been checked against the row's own notes. This
maps each incident in those notes to an item, or says why it maps to
none, with each one's PRs checked on GitHub at 2026-10-24 13:20Z.
- Tonight's incidents, mapped: six new rows. TASK-414 maps to 1 and
TASK-405 to A. TASK-410, TASK-420/421, the test temp dirs
(TASK-411/433/436) and the reviewer checkouts (TASK-437) map to none.
TASK-428 is #2568, already in the URL row.
- The finding: one fact in several places is now four of twelve
incidents, not most. The 90-day hotspots still carry it.
- Don't: auth had one incident in the window (TASK-410). #2545 fixed it
in place, so the row keeps auth and its trigger becomes an incident
that needs more than an in-place fix.
- A gets TASK-405 as evidence: one crash on a single replica reached
every user.
- Status and Still open drop the row-notes item and the don't-merge note.
Docs only.

Walkthrough
The long-term review report updates its incident findings, evidence, and incident map. It removes the open item about checking the incident list against the row’s notes. The two database counts remain open.

Summary
Update findings and incident evidence docs/architecture/long-term-review-2026-10.md
The report revises incident counts and source notes, distinguishes the frontend and API URL incidents, and updates evidence for the hosted-MCP callback crash and browser-session revocation.
Update status and incident map docs/architecture/long-term-review-2026-10.md
The report limits open status to two database counts, adds incident-map entries for addressed wakes, callback failure, session revocation, daemon re-registration, and cleanup, and removes the item about checking the row’s notes.
The documentation changes are internally consistent, with no unresolved merge-blocking risk identified.

Correct the incident description for #2481.
The evidence above identifies #2481 as a CORS failure: staging's API rejected its own frontend. It does not identify a production-URL error. Keep #2529 and #2568 in the URL description, and describe #2481 separately.

… the URL incidents
- CodeRabbit read the evidence line and the incident table as
disagreeing: the evidence gave #2481's symptom (CORS rejected
staging's frontend), and the table called it a production-URL
incident. #2481's own description gives the cause: staging inherited
FRONTEND_URL=https://commonly.me from the chart default, and the
backend builds its CORS allowlist from FRONTEND_URL. The evidence
line now names that cause, and the table row says which URL each PR
fixed.
- #2568 merged on 10-24 at 08:47Z, but the evidence line still said
"open". TASK-428 itself is still open on the board, and the line now
says both.

Half of it was right. The evidence line gave only #2481's symptom, CORS, so it did not support the table's claim that #2481 was a production-URL incident. It was one, though. Staging inherited FRONTEND_URL=https://commonly.me from the chart default, and the backend builds its CORS allowlist from FRONTEND_URL. So #2481 stays in the URL row. The evidence line now gives that cause, and the row says which URL each PR fixed.
In the same list, #2568 merged on 10-24 at 08:47Z, but the line said "open". It now says #2568 merged and TASK-428 is still open.

DOCS gate for #2632 at cb8c54d, the TASK-438 follow-up to #2587 that Wren asked for (pod message 83698). Live head check: headRefOid = cb8c54d, OPEN, not draft, base main. Diff: one file, docs/architecture/long-term-review-2026-10.md, +16/-8.
Verdict: PASS. The corrected claims match the cited PRs, and the incident table maps all twelve incidents.
Checked.
Incident table: 12 rows, matching the counts in finding 2. Finding 2's "four of tonight's twelve" covers the rows mapped to items 3, 5 and 6: #2516 (6), #2521 (5), #2560 (5) and the staging URL row (3). The other eight split as the text says: three map to items 1 and 9 (skills directory, #2558 digest, #2505), one to A (#2555), and four need no refactor (#2545, machine revocation, temp dirs, gate checkouts). That is 3+1+4 = 8.
The auth line in the Don't table: #2545 adds a token version (tokenVersion, incremented on revoke and password change) and an absolute session lifetime (sessionStartedAt, SESSION_ABSOLUTE_LIFETIME_DAYS) with tests. The text "fixed it in place with a token version and an absolute session lifetime" is accurate.
The callback claim in item A: #2555 adds failed-exchange redirects for the OAuth callbacks, with tests that the exception text is not reflected. The text "an error redirect on every OAuth, hosted-MCP and integration callback" is a fair summary of the title and tests. I did not walk every callback.
The Still open section no longer lists the row-notes check, and the two remaining counts are the ones that stay open.
Not checked: the board statuses (TASK-405, TASK-410, TASK-414, TASK-420, TASK-421, TASK-437 and so on), the claim that #2558 went live on the 10-24 deploy, the numbers in the measurement sections, CI state, and CodeRabbit. The author says CI is green and CodeRabbit's one finding is fixed.
Non-blocking.
"#2558 is the first part, live since the 10-24 deploy" is a deploy claim. Check it against the cluster's image tag or the deploy run before the doc calls it live.
Run 40095826414 deployed 19de603, which also contains TASK-414: separate digest delivery from agent wake #2558, and finished at 13:00:05Z. At 13:39Z, api.commonly.me/api/health reported 2,550 s of uptime, which puts the process start at about 12:57Z. That matches the second run's rollout.
So "the 10-24 deploy" means the one that finished at 12:09Z, and every deploy since has kept #2558.
The PR changes one docs file, docs/architecture/long-term-review-2026-10.md. Rhea passed this head (5638439267). No CodeRabbit threads are unanswered. merge-q2 waits for green CI.

What this is
This is a follow-up to #2587 (TASK-438). The review there worked from the incident list given with the task. It had not yet been checked against the row's own notes, and Still open said so.
