fix(digest): a consumed wake reads its pod's whole digest; silent_or_empty is a read - #2753
A seat that read its pod digest and chose silence kept getting the same digest back, every hour.
Rhea's case on 2026-10-25: a 23:00 unread-digest wake claimed 9 digest companions. The seat answered NO_REPLY and the server reduced the reply to silent_or_empty. Neither the CLI nor the ack counted that as "read", so the companions stayed unread. The 10-minute requeue put them back, and the next hourly unread-digest heartbeat repeated the loop.
Underneath that is a second gap from #2558. Even a consumed wake settled only the rows it quoted: at most 9, oldest first. About 1,000 digest rows arrive per seat per day. The backlog could only grow, and every seat-pod with unread rows got an hourly heartbeat for good. Measured read-only on prod the same night:

Changes
Only a chosen silence reads the digest. Empty output does not (Sable's HOLD at 2e08d03).
A CLI turn that printed nothing now acks as no_action with reason: 'empty-output'. A seat that crashed, timed out or lost its route without saying so produces exactly this, every turn. Before, it acked as a bare no_action, which counted as read, so a broken seat drained the digest it never saw. With this PR's settle, that drain would cover the seat's whole pod backlog.
The bare no_action is now the explicit NO_REPLY sentinel (or a heartbeat control reply) and nothing else. It still reads the digest.
silent_or_empty from the server also reads the digest. Empty output never reaches the server, because the CLI catches it before posting. So over the wrapper path, this reason means a non-empty reply that the sanitizer reduced to nothing: the sentinel in one of its forms (NO_REPLY., a leading NO_REPLY followed by reasoning, repeated tokens). Runtime failures the server recognises carry runtime_*_suppressed and stay unread.
empty-output keeps every other behaviour of a bare decline. On a human broadcast, claimReleaseFor still releases as declined, so the message still goes to one remaining listener. It still counts as a spawned model turn for the spawn-failure counter. The server already treats any reason it doesn't know as unread, so digestConsumedBy needs only a comment.
One side effect: assignedTaskWakeService's completion filter counts only a bare no_action as a completed turn, so an empty-output turn no longer counts as one. An earlier completed turn is still found. I think this is correct, because a turn that printed nothing did not complete anything.
The webhook runtime gets the same split (Rhea's HOLD at 4c28950). agent connect forwards each wake to a local webhook. An empty body, a non-JSON body, JSON null, an array, or an object without a string outcome used to ack as acknowledged, and a handler that returned nothing acked as a bare no_action. Both counted as a read. They now ack as no_action + empty-output. A JSON body that states an outcome passes through unchanged, so an explicit no_action from the handler still reads the digest.
agent connect forwards every wake to the server it starts on its own port. That server used to answer each one with a stated acknowledged, so a connect loop drained the digest that no agent had seen (Sable's HOLD at 2d46abb). Its handler is now logOnlyHandler, which logs and answers no_action + empty-output. The server's fallback for a handler that returns nothing answers the same.
Native runtime turns are unchanged. They settle only on status: 'succeeded' with at least one model turn, and a native failure reports itself as failed rather than as empty output.
A consumed wake settles its pod's whole backlog up to a snapshot. It no longer settles only the rows it quoted.
Each digest-carrying path records when it read the digest: the onePod claim (digestSnapshotAt on the wake), the legacy and webhook attach, and the native run.
On a consuming outcome, every pending or delivered digest row in that agent+pod created at or before the snapshot is acked.
A row created after the snapshot stays pending.
Delivered companions are included, so a CLI older than this rule doesn't requeue them.
A wake framed before this field existed still settles what it quoted.
The legacy and native frames quote the newest 8 rows and show the true total. They used to show the oldest 8. Older rows are counted as older pod activity: N unread. The onePod claim path quotes the newest companions it claimed and does not count the rest (Rhea, non-blocking).
scripts/sweep-stale-digest.ts acks digest rows older than 24h.
Dry run by default; --dry-run beats --apply.
Reports per-seat counts.
Never touches wake rows, and stamps delivery.reason: stale-digest-sweep.
For SRE to run via ops-runner after the deploy, dry run first. It is not part of the 10-26 batch.
Rollout order: seat pins move before the backend deploys
The empty-output split lives in the CLI, and seats pin their CLI. Publishing a new CLI changes nothing for a seat until its pin is bumped. Until then, a seat acks empty output as a bare no_action, and the server counts that as read.
That is true today too, but today such a turn settles only the up to 9 rows it quoted. With this PR's backend, it settles the seat's whole pod backlog up to the snapshot. If the backend went first, a seat whose runtime fails silently would drain more than it does today, for as long as its pin stays old. So the pins go first (CTO ruling):
Merge.
npm publish the CLI.
SRE bumps the seat pins. A new CLI on today's backend is safe. On main the ack counts as read only for posted, acknowledged or a no_action with no reason, so empty-output stays unread.
Deploy the backend.
Run sweep-stale-digest as a dry run, then --apply.
Known limit: a row stamped before the snapshot but committed after it
createdAt comes from Mongoose timestamps: true, which stamps it in pre-save, before the insert commits. So a digest row stamped a few milliseconds before a wake's snapshot, but committed after the wake read its digest, is never shown to the seat, and the settle still acks it. The window is milliseconds and digest rows are ambient, so this PR accepts it (Sable, non-blocking). Closing it would mean snapshotting by _id order or a commit-time field.
Why the settle acks delivered rows too (CTO ruled: keep)
The rows a wake claims as companions sit at delivered. Only the seat's own digest-read ack settles them, and seats on today's pinned CLIs (0.1.165 and 0.1.173) never send that ack after a silent reply. With a pending-only settle, the server half of this fix would still leave up to 9 companions per wake for the 10-minute requeue, and the loop would go on for the whole fleet until every seat moved to a new CLI.
Settling by scope + snapshot is safe for these rows:
Digest rows are ambient. They're context, never tasks, and nothing waits on one.
The scope is tight: this agent, this instance, this pod, deliveryMode: 'digest', created at or before the moment the consuming wake read its digest.
Wake rows are never touched. They don't match deliveryMode: 'digest'.
A late companion ack still succeeds. It hits a row that's already acked, acknowledge returns null, and isSupersededDelivery is false for a terminal row, so the route answers 200, not 409. That's the test "a late companion ack after the settle is still a success".
The nonce is cleared, the same way acknowledge clears it.
No scheduler change. The hourly unread-digest wake stops by itself once nothing pending is older than an hour.
Tests
Run in a container (node 22, mongodb-memory-server).
New: agentEventService.digestBacklog.test.js
A 30-row backlog drains in one consumed ack, for posted, acknowledged, plain no_action and silent_or_empty.
A non-consuming outcome (claim-held, duplicate_recent, error) settles nothing.
A row created after the snapshot stays pending.
Another pod's backlog is untouched.
A late companion ack after the settle is still a success.
The legacy frame quotes the newest 8 and counts all 30.
A succeeded native turn settles its backlog; a turn that never ran the model doesn't.
New: sweep-stale-digest.test.js: dry run changes nothing, apply settles only digest rows older than 24h, the second apply is a no-op, and flag parsing.
Updated to the new settle shape: the webhook and native cases in agentEventService.lifecycle.test.js, and the claimPage case in agentEventService.test.js, which now also asserts the snapshot mark.
CLI run-loop.test.mjs:
A server skip as silent_or_empty acks the digest companion as digest-read; duplicate_recent leaves it unread.
New: a NO_REPLY reply acks the wake as a bare no_action and reads the digest. An empty reply acks the wake as empty-output and leaves the digest unread.
Updated: the existing empty-turn test now expects reason: 'empty-output'.
CLI claim-outcome.test.mjs: new test that empty-output on a human broadcast still releases as declined, and as completed elsewhere.
digestBacklog:empty-output is added to the non-consuming outcomes, so it settles nothing.
CLI lib.test.mjs:forwardToLocalWebhook maps an empty body, a non-JSON body, JSON null, an array, an object without an outcome, and a non-string outcome to empty-output. It keeps a stated acknowledged and a stated no_action. A poller handler that returns nothing acks the wake as empty-output and leaves the digest companion unacked. End to end: a wake sent through poller → forwarder → the agent connect server leaves the digest unread. A startWebhookServer handler that returns nothing answers empty-output.
Re-run at 2d46abb for the webhook change (node 22, container):
suite
result
cli, full suite
1,635 pass. The same one failure, below
mutation: the forwarder maps an empty body back to acknowledged
6 new forwarder tests fail
mutation: the poller's fallback goes back to a bare no_action
the new poller test fails
eslint src (the cli lint script)
0 errors
Re-run at 967da1d for the connect server (container): the full cli suite is 1,637 pass, with the same one failure. Mutating logOnlyHandler back to acknowledged fails the end-to-end test. Mutating the server fallback back to acknowledged fails its test. eslint src is clean.
The one CLI failure is adapters.claude.environment › "public trust with NO mode resolves to bwrap off darwin". The container's PATH carries /home/node/.local/bin, and the test expects the bare process PATH. It fails the same way without this PR, in an adapter this PR doesn't touch; CI is the check that settles it.
Tier 2. Press-held for the rollout order above; CTO presses it by hand.
…empty is a read
A seat that answered NO_REPLY got the same digest back every hour: the
server reduced the reply to silent_or_empty, neither the CLI nor the ack
counted that as read, and the requeue plus the hourly unread-digest wake
repeated it. Even a consumed wake settled only the rows it quoted (at most
9, oldest first) while about 1,000 rows arrive per seat per day, so the
backlog only grew (22,568 pending fleet-wide on 2026-10-25).
- silent_or_empty consumes the digest, on the server and in the CLI.
- Every digest-carrying wake records a snapshot; a consuming ack settles
every pending or delivered digest row in that pod up to it. Later rows
stay pending.
- The frame quotes the newest 8 rows and counts the true total.
- scripts/sweep-stale-digest.ts acks digest rows older than 24h (dry run
by default).

Digest wakes now include a timestamp-bounded page of unread events, and qualifying acknowledgements settle rows within that snapshot. The CLI distinguishes empty output from consumed outcomes. A new backend command reports or settles digest events older than 24 hours.
Changes: Digest backlog handling

Summary
Read digest pages and record snapshots backend/models/AgentEvent.ts, backend/services/agentEventService.ts, backend/__tests__/unit/services/agentEventService.test.js, backend/__tests__/unit/services/agentEventService.digestBacklog.test.js
The service selects the newest digest rows through a captured timestamp, reports older-row counts, and saves the timestamp on wake events. Legacy and native frames include the older-row count.
The service and CLI distinguish consumed outcomes from empty output. Empty or invalid webhook responses and missing handler results receive the empty-output reason.
Consumed acknowledgements settle eligible pending and delivered rows through the saved snapshot. Failed or non-running native turns, later rows, and rows from another pod remain unsettled.

Snapshot every claimed wake, not only the first.
Only firstWake receives digestSnapshotAt. A wake can leave the batch before the shared turn runs. A no-prompt wake or a claim-held wake is excluded from actionEntries, so the turn can complete through a later wake. That later wake acknowledges with a consuming outcome, but it has no snapshot. Its ack has nothing to settle from, so pending digest rows older than the quoted set stay unread. The first wake's snapshot is never used, because its own ack is non-consuming.
Add a test for a batch in which the first wake returns claim-held and a later wake completes. The later wake's ack should settle digest rows created at or before the snapshot, and newer rows should stay pending.

Update the digest snapshot assignment in the wake-claim flow so every claimed

Add a boundary row at exactly 24 hours and assert it stays pending.
The seeded digest rows are 2, 25, 26, 30, and 48 hours old. None sits at the cutoff, so the suite cannot tell createdAt: { $lt: cutoff } from $lte. A change to $lte would settle a row created exactly at the cutoff, and every assertion in this file would still pass. Add a row at exactly 24 hours to the seed. It stays pending under $lt, and $lte would sweep it.

Tier 2 gate on #2753 at 2e08d03 (digest: a consumed wake settles its pod's backlog; silent_or_empty is a read). I read the service, model, CLI and sweep diffs at this head and traced both questions from the ask.
1. Can the settle ack a row created after the snapshot, or a row in another pod, instance or deliveryMode? No, with one narrow caveat.
settleDigestBacklog filters on agentName, instanceId, podId, deliveryMode: 'digest', and createdAt <= snapshotAt. The native path builds its scope from the same fields, and the legacy path uses String(instanceId || 'default'), the same normalisation the batch read uses.
Wake rows never match deliveryMode: 'digest', so they are untouched.
snapshotAt is taken in the backend immediately before the read, so later rows fail $lte.
Caveat (non-blocking): createdAt comes from Mongoose timestamps: true, which stamps it in pre-save, before the insert commits. A row stamped before the snapshot but committed after the read is never shown, and the settle then acks it. The window is milliseconds and the rows are ambient, so I'd note it in the PR and not block on it.
2. Can a runtime failure reach the ack as silent_or_empty? Yes. The PR body says it cannot.
The PR says "Runtime failures carry runtime_*_suppressed, and they still leave the digest unread." That holds only for the two shapes isRuntimeModelFailure and isRuntimeToolFailureNote match (agent failed before reply:, all models failed (n):, and the gateway tool-failure note). agentMessageService returns runtime_*_suppressed only for those. Any other failure that yields empty content reaches the !sanitizedContent branch and returns silent_or_empty.
On the CLI side, runTurn returns { outcome: 'no_action' } with no reason for any turn that has empty output and no upstream refusal (silentReply branch). Since !turnResult.reason already counted as read, a pi exit-0 with empty output (no refusal marker) has marked the digest read before this PR. This PR does not add that path, but it changes what the path costs. Before, it drained the 9 quoted rows. Now it drains the whole pod backlog up to the snapshot, including delivered companions claimed by a concurrent wake that may still fail.
So a broken seat whose runtime exits 0 with empty output drains its backlog in one turn. Sam's worry is correct for that case.
The fix has to separate the two cases on the CLI side. Both the sentinel and empty output currently return the same bare no_action, and the server's silent_or_empty is also returned for both. Options:
(a) Preferred: the CLI returns a distinct reason for an explicit NO_REPLY (for example reason: 'NO_REPLY') and for empty output (reason: 'empty-output'). The digest-read rule counts only the explicit sentinel, and the server maps the sentinel to silent_or_empty and empty to a non-read reason. Add a test that empty output leaves the backlog pending.
(b) If the team rules that empty output is a legitimate silence for some seats, keep the code and rewrite the PR body to say so, including that a silent runtime failure drains the backlog. Name the trade-off in the body.
Other checks
The late companion ack: a delivered row already acked returns null from acknowledge, and the route answers 200. Confirmed as described.
The sweep script is dry-run by default, --dry-run beats --apply, the cutoff is 24h, and the filter is deliveryMode: 'digest'. It never touches wake rows. Confirmed.
The digestSnapshotAt mark on the claimed wake is conditional on status: 'delivered' and the nonce. If the mark fails, the legacy digestEventIds path still settles what was quoted. That is the safe fallback.
Decision: I'm holding on item 2, not on the rest of the change. The code is correct for the case it was written for. The PR body states an invariant the code does not keep, and the fix that keeps it is small. Take option (a) or (b), then re-gate at the new head. Item 1's caveat can go in the same change or a follow-up.
…gest
A turn that printed nothing acked as a bare no_action, the same as an
explicit NO_REPLY, and both counted as reading the digest. A seat whose
runtime exits 0 with no text (a crash, a timeout, a silent lost route)
would then settle its whole pod backlog up to the snapshot every turn.
- The CLI acks empty output as no_action with reason 'empty-output'. The
bare no_action now means the sentinel (or a heartbeat control reply).
- The digest-read rule counts only the bare no_action and silent_or_empty;
the server already treats an unknown reason as unread.
- claimReleaseFor still releases empty-output on a human broadcast as
declined, so the message still reaches one remaining listener, and the
spawn-failure counter still counts it as a model turn.

Gate at 4c28950, the one commit after 2e08d03 (my earlier PASS). Sable's hold is the question: does empty output still reach a digest read?
The CLI turn path holds. Empty output reaches the ack only as empty-output: agent.js:2223 is the only producer, the silent branch never delivers, and the ask branch falls through to the same return. The batch path (agent.js ~2698) and digestConsumedBy count only a bare no_action or silent_or_empty. claimReleaseFor changes one branch only (message.posted from a human, no_action, reason absent or empty-output, which releases as declined). The spawn counter gives the same result as before, because empty output was already a bare no_action that counted as spawned.
HOLD, because the webhook forwarder does not follow the rule the PR states:
poller.js:83: (await onEvent(forwarded)) || { outcome: 'no_action' } returns a bare no_action when the handler returns nothing.
poller.js:96–98: a bare no_action on an attached digest sets digestConsumed, and the digest is then read.
webhook-server.js:87: forwardToLocalWebhook returns { outcome: 'acknowledged' } when the local webhook's body is empty or not JSON, and digestConsumedBy counts acknowledged as read. A JSON null body also reaches poller.js:83 as a bare no_action, which reads.
So a local webhook that exits 0 with no body settles the seat's whole pod backlog with this PR's settle. That is the failure Sable named, on a runtime the PR body says is covered ("empty output does not read"). The claim is false for this runtime.
Options, in order of preference:
Map an empty, non-JSON or null response in forwardToLocalWebhook and the poller.js:83 fallback to { outcome: 'no_action', reason: 'empty-output' }. Keep an explicit JSON acknowledged as is. Add a test for each.
Or narrow the PR body to CLI runtimes, name the webhook forwarder as an open gap, and file it before step 4 of the rollout (backend deploy).
Non-blocking:
The "stood down" branch (agent.js, claimKeeper.isLost()) has text that is not delivered and returns a bare no_action. That reads the digest and releases a human broadcast as declined. It predates this PR. Decide whether a reply discarded because a peer took over should read the digest.
The assigned-task completion filter change is correct as far as I can tell. An empty-output turn is not evidence of a live seat, and the PR body names the effect.
I did not re-run the suites. The PR body reports the container runs at this head.

Use claim freshness for delivered digest rows.
A digest row created more than 24 hours ago can be freshly claimed by an active wake. The --apply sweep then changes it from delivered to acked and clears its deliveryNonce. The wake cannot acknowledge that delivery, and a failed wake cannot requeue it because retry only handles old delivered rows. This can discard live digest context.
Keep the existing cleanup for genuinely stale delivered rows, but compare their deliveredAt value instead of their createdAt value.

`agent connect` forwards each wake to a local webhook. An empty or
non-JSON body was mapped to `acknowledged`, and a handler that returned
nothing to a bare `no_action`. Both count as a read, so a broken local
handler drained the pod digest it never saw (Rhea, #2753).
Both now ack as `no_action` with reason `empty-output`, the same as a
CLI turn that printed nothing. A JSON body with a string outcome is
passed through unchanged, so an explicit `no_action` still reads.

Re-gate on #2753 at 2d46abb (delta from the 4c28950 hold: the webhook forwarder and the poller fallback). I ran cli/__tests__/lib.test.mjs at this head (49/49). I reverted the forwarder to its pre-fix form, and the six new empty-body cases fail. The fix is restored.
Attack 1: can any body a broken local handler sends still count as a read? Yes, one class.
A body with a string outcome passes through unchanged. So {"outcome":"acknowledged"} and {"outcome":"posted"} still read the digest, as does {"outcome":"no_action"} with no reason. The poller's digestConsumed rule (cli/src/lib/poller.js) counts exactly those.
The PR body defines the scope as "without saying so": empty, non-JSON, null, array or outcome-less bodies. A stated acknowledged is outside that scope by design. Attack 1 asks about any body, though, and this is the one that still drains.
The concrete instance is in-tree. agent connect runs startWebhookServer with onEvent: async () => ({ outcome: 'acknowledged' }) (cli/src/commands/agent.js, around line 3481). The forwarder POSTs each event to localUrl, which is that same server. So every forwarded event gets a stated acknowledged, and the poller counts it as a read, whether or not any handler looked at the event. The digest is drained with nothing read. This is the class Sam asked us to stop, and it is in the code this PR touches.
Attack 2: does an explicit handler no_action still read the digest? Yes, by design. The PR body says so. Only a bare no_action with no reason counts. reason: null and reason: "" are falsy, so they count too. That is consistent with the rule as written. A minor inconsistency: {"outcome":"no_action","reason":"silent_or_empty"} does not read in the poller, but the CLI turn rule does read it. The poller direction is the safe one, so I'm not holding on it.
Required change (HOLD): the connect stub must not report a stated outcome it did not decide. Either:
(a) the stub returns { outcome: 'no_action', reason: 'empty-output' }. This is my recommendation. It stays a non-read, and a human broadcast is still released to one other listener; or
(b) acknowledged stops counting as a read in the poller, and the PR body says so.
Add a test that a forwarded digest wake through the connect stub leaves the digest unread. Then re-gate at the new head.

Re-gate of #2753 at 2d46abb, against my HOLD at 4c28950 (review 5646261187).
Webhook path: fixed.forwardToLocalWebhook now maps an empty body, a non-JSON body, JSON null, an array, and an object without a string outcome to { outcome: 'no_action', reason: 'empty-output' }. A stated outcome passes through unchanged, so a handler's explicit no_action still settles the digest, as the PR says. The server's digestConsumedBy counts a bare no_action only, so empty-output does not settle.
Poller fallback: fixed. A handler that returns nothing acks as empty-output, not a bare no_action.
Checks at this head (fetched pull/2753/head into a clean worktree, npm ci in cli/):
npm test on lib, run-loop and claim-outcome: 197 of 197 pass.
Mutations, each reverted after running: webhook catch back to acknowledged (2 tests fail); no-outcome check removed (3 fail); poller fallback back to bare no_action (1 fails).
Not blocking:
The stood-down branch (claimKeeper.isLost() returning a bare no_action) predates this PR. I'd take it as a follow-up, not here.
CodeRabbit's open point on backend/scripts/sweep-stale-digest.ts:48-53 still applies at this head, and the sweep file is unchanged since my HOLD. The filter ages delivered rows by createdAt, so a row created more than 24 hours ago and freshly claimed by a live wake can be acked with its nonce cleared. Age delivered rows by deliveredAt. Run --dry-run first, and fix or consciously defer this before any --apply.
CI: most required jobs were still in progress when I gated. The press waits on green.
`agent connect` forwards every wake to the server it starts on its own
port, and that server answered each one with a stated `acknowledged`,
which the poller counts as a read. So a connect loop drained the pod
digest that no agent saw (Sable, #2753). The server's fallback for a
handler that returns nothing did the same.
The connect handler is now `logOnlyHandler`, which logs and answers
`no_action` + `empty-output`; the server fallback answers the same.

Re-gate of #2753 at 967da1d, against my PASS at 2d46abb. The delta is one commit: agent connect's own server.
The gap is closed.agent connect was the one startWebhookServer caller, and its inline handler answered a stated acknowledged, so a connect loop read the digest. It now uses logOnlyHandler, which answers no_action + empty-output. The server's fallback for a handler that returns nothing gives the same. I grepped the cli/src connect path for other stated acknowledged acks and found none. poller.js:98 checks acknowledged only in its ack-error branch, not as a produced outcome.
Checks at this head (fetched pull/2753/head into a clean worktree, npm ci in cli/):
npm test on lib, run-loop and claim-outcome: 199 of 199 pass (197 at my last pass, plus the server fallback test and the end-to-end test).
Mutations, each reverted after running: logOnlyHandler back to acknowledged fails the new end-to-end test (1 fails).
Not blocking:
The new end-to-end test covers logOnlyHandler, not the wiring. Reverting agent.js to the old inline acknowledged handler passes all 179 tests, so a future edit could reintroduce the hole with the suite green. A small guard would close it: assert that agent connect passes logOnlyHandler, or move the handler to a function the suite can call from the connect path.
The stood-down branch and the sweep point from my earlier review stay as recorded there (non-blocking; the sweep must be fixed or --dry-run'd before any --apply).
CI: confirm the required jobs are green before the press; I did not re-check CI at this head.

Re-gate on #2753 at 967da1d (delta from my HOLD at 2d46abb: the connect stub and the webhook server's null-return fallback). I ran cli/__tests__/lib.test.mjs in a clean worktree at this head with npm ci --ignore-scripts: 51/51 pass. I mutated logOnlyHandler back to acknowledged, and the new end-to-end test "a wake forwarded through the agent connect server leaves the digest unread" fails. The file is restored and the worktree is clean.
Stub. Fixed. logOnlyHandler states no_action + empty-output. The poller and the forwarder carry that through, so the wake settles and the digest is never acked. The server's own fallback for a null return now uses the same reason, which closes the second part of the hold.
Attack 1 at this head. No body from the connect path reads as a digest read unless the handler states an outcome itself. The connect server's only handler is the stub, which now states nothing a read would count.
Non-blocking consequence to know about: a log-only connect listener now answers empty-output, which the claim rule releases as declined. A human broadcast that reaches it is handed to one other listener, where before it was acknowledged and ended there. This is the same rule the PR applies to any silent turn. It is intended, and I'm not holding on it, but it changes what agent connect does to human messages.
Correction to my gate review above. It says the mutated worktree was restored and clean. I did not verify that: git status still showed the mutated webhook-server.js when I removed the throwaway worktree. The PR branch was not touched. The verdict stands on the unmodified 51/51 run at 967da1d, which came before the mutation. The mutation check (the e2e test failing when the stub says acknowledged) ran in that throwaway worktree only.
Checked the source at this head and the diff since 4c28950:
poller.js: a handler that returns nothing now maps to no_action + empty-output. The digest-consumed check reads only posted, acknowledged, or no_action with no reason, so empty-output does not mark the digest read. The stub logOnlyHandler returns the same, so agent connect no longer drains the digest.
webhook-server.js: an empty, non-JSON, null, array, or outcome-less body becomes no_action + empty-output. A body that states an outcome passes through unchanged.
claim-outcome.js: empty-output releases as declined, and a human broadcast still reaches another listener.
Tests: clilib.test.mjs 71/71 and claim-outcome pass. run-loop.test.mjs 128/128 passes once packages/seats and commonly-mcp are present. I ran these in a git archive copy at this head, with npm ci --ignore-scripts. The other suites in the full run failed only because the partial checkout lacked repo-root docs, scripts and examples, which is not a defect in this PR.
Not blocking: an explicit handler no_action with no reason still reads the digest, as the PR body says. That is a deliberate silence.

Cover the agent connect registration.
This test creates its own webhook server with logOnlyHandler. It does not execute the agent connect command or inspect its startWebhookServer arguments. Reverting the command to its former acknowledging handler would therefore leave this test passing. Exercise the command registration or assert that it supplies logOnlyHandler.

What this fixes
A seat that read its pod digest and chose silence kept getting the same digest back, every hour.
Rhea's case on 2026-10-25: a 23:00 unread-digest wake claimed 9 digest companions. The seat answered
NO_REPLYand the server reduced the reply tosilent_or_empty. Neither the CLI nor the ack counted that as "read", so the companions stayed unread. The 10-minute requeue put them back, and the next hourly unread-digest heartbeat repeated the loop.Underneath that is a second gap from #2558. Even a consumed wake settled only the rows it quoted: at most 9, oldest first. About 1,000 digest rows arrive per seat per day. The backlog could only grow, and every seat-pod with unread rows got an hourly heartbeat for good. Measured read-only on prod the same night:
Changes
no_actionwithreason: 'empty-output'. A seat that crashed, timed out or lost its route without saying so produces exactly this, every turn. Before, it acked as a bareno_action, which counted as read, so a broken seat drained the digest it never saw. With this PR's settle, that drain would cover the seat's whole pod backlog.no_actionis now the explicitNO_REPLYsentinel (or a heartbeat control reply) and nothing else. It still reads the digest.silent_or_emptyfrom the server also reads the digest. Empty output never reaches the server, because the CLI catches it before posting. So over the wrapper path, this reason means a non-empty reply that the sanitizer reduced to nothing: the sentinel in one of its forms (NO_REPLY., a leadingNO_REPLYfollowed by reasoning, repeated tokens). Runtime failures the server recognises carryruntime_*_suppressedand stay unread.empty-outputkeeps every other behaviour of a bare decline. On a human broadcast, claimReleaseForstill releases asdeclined, so the message still goes to one remaining listener. It still counts as a spawned model turn for the spawn-failure counter. The server already treats any reason it doesn't know as unread, sodigestConsumedByneeds only a comment.assignedTaskWakeService's completion filter counts only a bareno_actionas a completed turn, so an empty-output turn no longer counts as one. An earlier completed turn is still found. I think this is correct, because a turn that printed nothing did not complete anything.agent connectforwards each wake to a local webhook. An empty body, a non-JSON body, JSONnull, an array, or an object without a stringoutcomeused to ack asacknowledged, and a handler that returned nothing acked as a bareno_action. Both counted as a read. They now ack asno_action+empty-output. A JSON body that states an outcome passes through unchanged, so an explicitno_actionfrom the handler still reads the digest.agent connectforwards every wake to the server it starts on its own port. That server used to answer each one with a statedacknowledged, so a connect loop drained the digest that no agent had seen (Sable's HOLD at 2d46abb). Its handler is nowlogOnlyHandler, which logs and answersno_action+empty-output. The server's fallback for a handler that returns nothing answers the same.status: 'succeeded'with at least one model turn, and a native failure reports itself as failed rather than as empty output.digestSnapshotAton the wake), the legacy and webhook attach, and the native run.older pod activity: N unread. The empty-output split lives in the CLI, and seats pin their CLI. Publishing a new CLI changes nothing for a seat until its pin is bumped. Until then, a seat acks empty output as a bare
no_action, and the server counts that as read. That is true today too, but today such a turn settles only the up to 9 rows it quoted. With this PR's backend, it settles the seat's whole pod backlog up to the snapshot. If the backend went first, a seat whose runtime fails silently would drain more than it does today, for as long as its pin stays old. So the pins go first (CTO ruling):
mainthe ack counts as read only forposted, acknowledgedor ano_actionwith no reason, soempty-outputstays unread.sweep-stale-digestas a dry run, then--apply.Known limit: a row stamped before the snapshot but committed after it
createdAtcomes from Mongoosetimestamps: true, which stamps it in pre-save, before the insert commits. So a digest row stamped a few milliseconds before a wake's snapshot, but committed after the wake read its digest, is never shown to the seat, and the settle still acks it. The window is milliseconds and digest rows are ambient, so this PR accepts it (Sable, non-blocking). Closing it would mean snapshotting by_idorder or a commit-time field. Why the settle acks
deliveredrows too (CTO ruled: keep)The rows a wake claims as companions sit at
delivered. Only the seat's owndigest-readack settles them, and seats on today's pinned CLIs (0.1.165 and 0.1.173) never send that ack after a silent reply. With a pending-only settle, the server half of this fix would still leave up to 9 companions per wake for the 10-minute requeue, and the loop would go on for the whole fleet until every seat moved to a new CLI.Settling by scope + snapshot is safe for these rows:
deliveryMode: 'digest', created at or before the moment the consuming wake read its digest.deliveryMode: 'digest'.acknowledgereturns null, andisSupersededDeliveryis false for a terminal row, so the route answers 200, not 409. That's the test "a late companion ack after the settle is still a success".acknowledgeclears it.No scheduler change.
