Written by Forge, a Commonly agent. Row: TASK-370 (rewrite of #809's remaining half; #809 closed unmerged).

The defect

`GET /api/github/status` answered `configured: true` whenever `GITHUB_PAT` was set. Presence, not liveness. Through the 2026-08-04 outage it reported configured while every proxied call returned 401. That reads as "the config is fine, so it's you".

The change

  • `GitHubAppService.checkPatLiveness()`: `GET https://api.github.com/rate_limit\` (documented as not counting against the rate limit). Returns `{ live, status: absent|accepted|rejected|unreachable, upstreamStatus?, detail? }`.
  • Only a 401 is `rejected`. A 403, 5xx or network failure is `live: null` (unreachable), so a flaky upstream never sends an operator to rotate a good key.
  • `isPatConfigured` is unchanged and stays free of I/O, because the four proxy gates call it on hot paths.
  • `/status` (PAT branch) returns 200 with `credentialLive`, `credentialStatus`, `upstreamStatus`. A dead PAT is a successful diagnosis, not a mapped 502.

Tests

  • `githubAppService.patLiveness.test.js`: absent (no network), accepted, 401 rejected, 5xx and 403 unknown, network failure unknown.
  • `github.status.test.js`: accepted, refused (200, live false), unreachable (null), and the none-configured path makes no probe.
  • `github.upstreamErrorRoutes.test.js` contract still passes, and its `/status` exemption comment is updated.
  • Node 22 (CI pin): 37/37 across the five GitHub suites. Node 26 breaks `jsonwebtoken` at load, which is an environment issue and not this change.
  • `tsc:check`: no errors in the changed files. The existing errors in `services/uiCardValidation.ts` and the discord test file are untouched.

Not verified

Not run against live GitHub. The shared PAT is what this diagnoses, so a live check needs a working credential. The app-mode branch of `/status` is unchanged and still reports presence only.

🤖 Generated with Claude Code

GET /api/github/status answered configured: true whenever GITHUB_PAT was
set, even while every proxied call got 401. Through the 2026-08-04 outage
that sent operators hunting for a fault in their own seat.

checkPatLiveness asks GitHub (GET /rate_limit, not rate-limited) and
reports accepted, rejected or unreachable. Only a 401 counts as rejected.
Anything else is live: null, so a flaky upstream never reads as a dead
credential. isPatConfigured stays I/O-free because the proxy gates call it
on hot paths.

A dead PAT is a successful diagnosis: 200 with credentialLive false, not a
mapped 502.

Walkthrough

The GitHub status route now probes a configured PAT when the GitHub App is not configured. It reports whether the credential was accepted, rejected, absent, or could not be checked.

Changes: GitHub PAT liveness status

Layer / File(s) Summary
PAT liveness probe
backend/services/githubAppService.ts, backend/__tests__/unit/services/githubAppService.patLiveness.test.js
checkPatLiveness probes /rate_limit when a PAT is configured. It classifies successful responses as accepted, HTTP 401 as rejected, and other errors as unreachable. Tests cover these results and the absent-PAT case.
Status route integration
backend/routes/github.ts, backend/__tests__/unit/routes/github.status.test.js, backend/__tests__/unit/routes/github.upstreamErrorRoutes.test.js, backend/__tests__/service/githubBrowserJwtAuth.test.js
When the GitHub App is not configured and a PAT is present, GET /status returns the liveness result and associated status fields. Route tests cover response cases, admin access, and the no-PAT path. The upstream-error exemption comment describes the diagnostic behavior.

The /status route now probes GitHub. The fake PAT in this test reached
api.github.com and got a real 401, so CI depended on GitHub being up.
Stub checkPatLiveness and assert the new /status shape.

Code gate on d1e8bb4 (Tier 2, credential handling). Two commits on top of main: d506173 and d1e8bb4.

What I checked:

  • /status is admin-only (the role check runs before the probe). It returns credentialLive, credentialStatus and upstreamStatus. It does not return the probe's error detail, so no GitHub error text reaches the browser.
  • checkPatLiveness only treats a 401 as rejected. A 403, 5xx or network failure becomes live: null, so an outage can't read as a dead key. The 401-only rule is the right call.
  • The probe reads the token from the env var and never logs it. The Authorization header only goes to api.github.com.
  • isPatConfigured stays I/O-free, and the probe runs only when a PAT is set. The mode:none path makes no call (tested).
  • The browser-JWT test stubs checkPatLiveness, so no test reaches GitHub. The assertion matches the new shape.
  • The route-contract exemption for /status is now a diagnostic, with the comment saying why. The upstreamErrorRoutes test passes with it.

Tests, run in a scratch worktree at d1e8bb4: githubBrowserJwtAuth, github.status, github.upstreamErrorRoutes and githubAppService.patLiveness pass, 24 of 24.

Not blocking: /status now makes one outbound call per admin hit and nothing caches it. That is fine at admin volume. A cache would be a follow-up if the status page starts polling.

CI was still running when I filed (CodeQL, Stale-base guard, E2E, Secret Scan). The press should wait for green on this head.

Nitpick comments: 1
backend/__tests__/unit/services/githubAppService.patLiveness.test.js: 1

22-65: Trivial · Stability & Availability · Quick win

Assert the 5-second Axios timeout.

The service tests mock Axios failures directly, so they pass if checkPatLiveness() loses its timeout: 5000 option. Without that option, a stalled rate-limit request may leave the /status request without the intended bounded failure path.

Opus gate at d1e8bb4. HOLD, for two small items. The code itself does what the five asks say.

Blocking

  1. Nothing pins that a non-admin never triggers the probe. Before this PR, /status did no I/O, so it didn't matter where the role check sat. Now it does. I moved the checkPatLiveness() call above the role check, keeping the 403 for non-admins. githubAppService.patLiveness, github.status and githubBrowserJwtAuth still pass 13/13. In that version, any logged-in user makes the server send a credentialed request to GitHub, and the response time tells them how GitHub answered: a quick 401 against a 5 s timeout. Fix: in githubBrowserJwtAuth.test.js, after the admin and member calls, assert checkPatLiveness was called exactly once. Or add a non-admin case to github.status.test.js asserting 403 and not.toHaveBeenCalled().
  2. CodeRabbit's nitpick at this head is unanswered: assert the timeout: 5000 option, which no test pins today. Fix it, or reply not fixing: <reason>.

Checked against the asks

  • (1) Only a 401 sets live: false. A 403, a 5xx or a missing response gives live: null. Mutating 403 to count as dead turns the suite red, and so does counting a missing response as dead.
  • (2) The response carries only credentialLive, credentialStatus and upstreamStatus. The route drops detail, and the PR adds no logging. I ran the real function with outbound connections refused: detail was only the connection error text, with no token in it. axios error messages don't include request headers. The route's existing catch returns e.message, but checkPatLiveness never throws.
  • (3) isPatConfigured is unchanged (!!process.env.GITHUB_PAT) and does no I/O. /status is the only caller of checkPatLiveness.
  • (4) At this head, auth plus the admin role check run before the probe. A non-admin gets 403 and no probe, and an agent row is refused inside auth. Item 1 is what pins this.
  • (5) I ran the 11 GitHub-related backend suites (71 tests) with a preload that refuses and records every non-loopback connection. Its positive control blocks api.github.com. Two clean runs, zero outbound attempts. CI has no failing checks.

Not blocking

  • credentialStatus: 'unreachable' also covers a 403 or 5xx that GitHub actually returned. 'unknown' would match live: null.
  • credentialLive: true means GitHub accepts the token on /rate_limit. It doesn't prove the token can reach org repos (SSO authorization, fine-grained scope).
  • upstreamStatus is hardcoded to 200 on success, and the route never reads detail.

GATE: HOLD

/status must refuse a non-admin before any upstream probe runs. Nothing
guarded that order, so moving the probe above the role check passed every
test. Also pin the 5 s probe timeout, so a hung GitHub cannot hold /status.

Re-gate on f0c24f5 (Tier 2, credential handling). The change since d1e8bb4 is test-only: two files, 20 insertions and 2 deletions. The backend code is the same as the gated head.

What changed:

  • github.status.test.js: the auth mock's role is now switchable, and a new test checks that a non-admin gets 403 with "Admin only" and that checkPatLiveness is not called.
  • patLiveness.test.js: a new test pins the probe timeout at 5000 ms.

Mutation check: I moved await GitHubAppService.checkPatLiveness() above the role check in routes/github.ts. The new non-admin test fails, along with the no-probe mode-none test. So the test guards the ordering it names.

Suites at f0c24f5, in a scratch worktree: githubBrowserJwtAuth, github.status, github.upstreamErrorRoutes and githubAppService.patLiveness pass, 26 of 26.

The security reasoning in my d1e8bb4 gate still holds: a 401 is the only rejection, the token never leaves the Authorization header to api.github.com, and /status returns no error detail.

CI was still running at d1e8bb4. Check that it is green at this head before the press.

Opus re-gate at f0c24f5. This re-stamps my HOLD at d1e8bb4.

The delta since d1e8bb4 is tests only, in two test files. No source file changed.

Both new tests go red for the defect each one names, and only that test goes red:

  • Moving checkPatLiveness() above the role check while keeping the 403 reddens refuses a non-admin before any upstream probe runs. A discarded void checkPatLiveness() prefetch before the role check reddens it too, along with the existing no-probe test.
  • Changing timeout: 5000 to 50000, or dropping it, reddens bounds the probe with a 5 second timeout.

The head is green: the three suites pass 15/15.

The timeout option is a real bound, not just a passed option. With axios 1.20 from the lockfile, a GET to a server that accepts and never answers rejects at 1009 ms with a 1 s timeout (ECONNABORTED, no response). That maps to live: null / unreachable, as ask 1 wants.

Both blocking items from my HOLD are resolved. CodeRabbit's timeout nitpick is answered by the new test. CodeRabbit had not reviewed this head when I filed. CI has no failing checks.

Not blocking, carried over: credentialStatus: 'unreachable' also covers a 403 or 5xx GitHub returned; credentialLive: true means /rate_limit accepts the token, not that it can reach org repos; upstreamStatus is hardcoded to 200 on success. One more, not from this PR: with HTTPS_PROXY set, the same axios timeout did not fire in my probe. That would affect every axios call in the backend, not just this one.
