docs(ax-audit): native-runtime HTTP 401 named the model, not the guardrail (entry 86) - #3083
AX-audit entry #86, from the 2026-10-31 Scout-staging incident (fixed in #3077 + #3079).
The trap: a native-runtime AgentRun failed with errorMessage="Model deepseek-v4-flash: HTTP 401" — which reads as a bad DeepSeek key. The 401 actually came from the openai-moderation-enforce guardrail's during_call request to https://api.openai.com/v1/moderations, using staging's 11-byte placeholder OPENAI_API_KEY, before the model call. The real URL was visible only in litellm's own pod logs; the DeepSeek key was valid throughout.
Captures two lessons for the fleet: (1) a native-runtime error names the model it was reaching, not the dependency that failed — read litellm's logs for the failing URL before blaming the model key; (2) "litellm is up" is necessary but not sufficient for a native agent to reply — the guardrail is an independent failure surface on the same turn.
Docs-only; no code. The fix PRs (#3077, #3079) are already merged and verified live (Scout now replies in the onboarding pod).
…ardrail (entry 86)
From the Scout-staging incident (#3077 + #3079). The AgentRun error
"Model deepseek-v4-flash: HTTP 401" attributed the failure to DeepSeek, but the
401 came from the openai-moderation-enforce guardrail's upstream call to
https://api.openai.com/v1/moderations with staging's placeholder OPENAI key —
visible only in litellm's own pod logs. Captures the trap so the next agent
debugging a silent native agent reads the guardrail path, not just the model key,
and knows "litellm is up" is necessary but not sufficient.

The development audit adds an entry about a staging native-runtime failure. It attributes the HTTP 401 to the moderation guardrail’s invalid OpenAI key, records the staging change to injection-guard, and advises checking LiteLLM logs.

Summary
Document the staging incident docs/development/agent-experience-audit.md
The audit records the failure’s guardrail cause, the staging configuration change to injection-guard, and guidance to check LiteLLM logs. It identifies changing the service default as follow-up work.
… not its code default (review fix)
Addresses the writer's-review finding on #3083: the entry implied the sibling
llmService path's code default is already injection-guard. It is not — only the
Helm env (backend-deployment.yaml:573) hardcodes it; llmService.ts's code default
is the same unsafe pair. Reworded so a reader copies the deployment override, not
the sibling's code default.

Writer's-subagent review (D218, medium): done. All load-bearing claims verified against the codebase (NATIVE_RUNTIME_GUARDRAILS default, the openai_moderation pre-call, values-dev:131, backend-deployment.yaml:573 hardcode, the guard test's existence on main, entry numbering). One imprecision it found — the entry implied the sibling llmService code default is already injection-guard — is fixed at head 410d4ff: the text now says that path is safe only via its Helm-env hardcode, and names the deployment override (not the code default) as the thing to copy. Docs-only.

Correct the LLMSERVICEGUARDRAILS claim.

Correct the LLMSERVICE_GUARDRAILS claim.
backend/services/llmService.ts defaults LLMSERVICE_GUARDRAILS to openai-moderation-enforce,injection-guard. The injection-guard value comes from the backend.env.nativeRuntimeGuardrails override in values-dev.yaml, which is a different setting.
Proposed correction
- Root-cause follow-up (tracked, not in these PRs): flip the service default in `nativeRuntimeService.ts` to `injection-guard` so a new/unconfigured env does not inherit the keyless 401, matching the already-hardcoded `LLMSERVICE_GUARDRAILS: injection-guard` on the sibling path.+ Root-cause follow-up (tracked, not in these PRs): flip the service default in `nativeRuntimeService.ts` to `injection-guard` so a new/unconfigured env does not inherit the keyless 401, matching the `backend.env.nativeRuntimeGuardrails: "injection-guard"` deployment override in `values-dev.yaml`.

Review comment at @docs/development/agent-experience-audit.md at line 4230:
Correct the root-cause follow-up in the audit: distinguish the
`LLMSERVICE_GUARDRAILS` default in `llmService.ts` from the
`backend.env.nativeRuntimeGuardrails` deployment override in `values-dev.yaml`.
Update the comparison to reference the deployment override rather than claiming
the sibling path hardcodes `LLMSERVICE_GUARDRAILS: injection-guard`.

Use the configured during_call phase in the audit entry.
The repository configures openai-moderation-enforce as during_call. The entry incorrectly calls it a “pre-call guardrail” and asserts that it ran before the model call. This can direct operators to the wrong phase during troubleshooting. Remove the unsupported ordering claim and identify the configured guardrail as during_call.
