fix(cli): hard timeout on every CAP request, and exit when the inbox loop stalls (TASK-338) - #3116
Before this change, no CAP request from the CLI carried a timeout or an AbortSignal, in api.js, poller.js or inbox.js. So one request that a proxy or the network accepted and never answered left the agent run inbox loop alive and idle for good. On 10-31 two seats each got an HTTP 504 and then an unanswered request, and sat idle for 8+ hours until someone killed them. Their keepalives (tmux, launchd KeepAlive) act only on exit, so they never saw it. Outside users' commonly agent run has the same flaw.
What changes
1. A hard timeout on every CAP request (cli/src/lib/api.js, cli/src/lib/inbox.js)
Reads time out after 30s, writes after 60s and uploads after 120s. COMMONLY_CAP_TIMEOUT_MS sets the base, and createClient({ timeoutMs }) overrides it.
The signal covers the body read as well as the headers. A server that sends headers and then goes silent is cut off too.
A timeout throws code: 'cap_timeout' with no status. So the run loop treats it as an ordinary failure and sends it through pollRetryPolicy's backoff (TASK-025). It is never an ack.
When a caller passes its own signal, the hard timeout runs beside it, not instead of it. Doctor and the 5s identity and skills reads keep their own shorter timeouts and their own error.
login() is covered too.
2. Poll liveness (cli/src/lib/poll-liveness.js, wired into performRun and startPoller)
If no poll settles for COMMONLY_POLL_LIVENESS_MS (default 10 minutes), the loop stops and logs one line: poll stalled: no inbox poll settled for …. agent run and agent connect then exit 1, so the keepalive starts a clean process.
The clock runs while the loop polls, acks or holds work. It pauses while a model turn runs (both turn sites in the tick) and during the loop's own scheduled waits, including backoff up to 15 minutes. A long turn or a long backoff can't trip it.
The clock uses monotonic time, so a laptop that sleeps doesn't read as a stall.
A stall clears held-retry renewals the same way stop() does. A loop that already stopped itself (auth, a superseded delivery) never reports a stall.
3. 5xx errors back off, never stop
The run loop already handled this. The new tests now pin it.
docs/cli/README.md documents both env vars and adds a troubleshooting entry for the stall exit.
Tests
cli/__tests__/cap-timeout-liveness.test.mjs has 10 tests against a real HTTP server, not a mocked client, because the defect is in the socket:
504, then a hang, then healthy: the loop backs off and recovers, with no stall exit.
A poll the timeout doesn't catch: with the timeout set longer than the liveness limit, the loop stops, asks for an exit, and sends no second poll.
A long turn: a 2.5s turn under a 1s liveness limit causes no stall.
A hung ack after a turn: the stall still fires, because the clock covers more than the poll request.
stop() during an in-flight poll: a stop, not a stall. The page is left to requeue, as before.
The timeout itself: writes get twice the read timeout, headers followed by silence is cut off, and a caller's own shorter timeout keeps its own error.
agent connect: the poller has the same stall exit.
Mutation checks:
With the signal removed from timedFetch, 3 tests fail.
With the clock never started, the stall test fails.
With the turn not pausing the clock, the long-turn test fails.
The full CLI suite ran in Docker on Node 22: 2185 passed and 3 failed. The 3 failures are adapters.claude.environment and mention-event-types.contract, which fail the same way on main in a Linux container. eslint src is clean.
Review
This is Tier 1: it changes CLI loop behaviour and touches no auth or data. Its one review round was the writer's own Opus subagent review. It found 0 blocking and 7 non-blocking issues, and all but one are addressed here:
The clock now covers the whole tick outside turns, not just the request.
Only the cap's own timeout is renamed.
A stall clears held renewals.
The clock uses monotonic time.
The docs say to keep the timeout below the liveness limit.
New tests cover headers followed by silence, the stop() race, a hung ack and a caller's own timeout.
The long-turn test now uses a 1s limit.
Left as is: webhook-server.js forwarding to a local handler with no timeout. That isn't a CAP request, and it's out of this row's scope.
CodeRabbit and CI come next. This changes cli/, so it's a hand press with a connector-ops lane review. cli/package.json is unchanged, because the version is set at publish.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
CAP requests use method-specific timeout limits. Agent polling tracks unsettled polls and stops with an error after the configured liveness limit. Tests and CLI documentation cover these behaviors.

Summary
CAP request timeouts cli/src/lib/api.js, cli/src/lib/inbox.js, cli/__tests__/cap-timeout-liveness.test.mjs, cli/__tests__/inbox.test.mjs
CAP client and inbox requests use configured read or write timeouts. Tests cover HTTP 504 responses, request and response-body timeouts, and caller-provided aborts.
Poll-liveness controller and poller cli/src/lib/poll-liveness.js, cli/src/lib/poller.js, cli/__tests__/cap-timeout-liveness.test.mjs
The poller tracks unsettled requests and stops with an error and callback when the liveness limit is reached. Tests cover stalled-poll behavior.
Agent-loop liveness and exit handling cli/src/commands/agent.js, cli/__tests__/cap-timeout-liveness.test.mjs, docs/cli/README.md
The agent loop pauses monitoring during turns and scheduled waits. A stall stops the loop, invokes the callback, and causes agent connect or agent run to exit with status 1. Tests cover recovery and stop cases. Documentation describes timeout settings and exit behavior.
Early Node 20 users can see generic timeout errors and accumulating listeners. The change is mergeable with awareness of these limited issues.

Add caller-path tests for both timeout conversions.
cli/__tests__/inbox.test.mjs reaches createInboxRequest, but its fetch and text() promises settle immediately. Add assertions that TimeoutError from fetchImpl and res.text() becomes InboxRequestError with null status. These assertions fail if the conversion is reverted.
A non-timeout-error assertion remains useful for unchanged propagation, but it can pass before and after the reversion. It does not detect the new timeout behavior.

CodeRabbit's one grouped comment (inbox.js 417-419, caller-path tests for the timeout conversion) is valid, and 6767e35 fixes it. cli/__tests__/inbox.test.mjs now drives createInboxRequest with a fetchImpl that rejects with TimeoutError, and with a res.text() that rejects the same way. Both assert InboxRequestError with a null status and the named message. One more test checks that every request carries a signal, and another that a non-timeout failure passes through unchanged. With the conversion reverted, the two timeout tests fail (checked). Nothing was skipped.

The current inbox tests only assert that createInboxRequest receives an AbortSignal; their timeout cases inject TimeoutError into fetch and res.text(). The real-server tests exercise the separate createClient path.
Add tests named a hanging inbox request is aborted and a stalled inbox body read is aborted, using a short configured timeout. These tests cover a material, distinct inbox path rather than duplicating the existing createClient coverage.

api.jsnameTimeout now renames only when the caught error iscapTimeout.reason. Checking that capTimeout had aborted could claim a caller's own TimeoutError if both signals fired. The cap_timeout tests still pass, which confirms fetch rejects with the signal's own reason object. The caller-timeout test still passes too.
inbox.js now uses the same formatLimit as api.js, so a sub-second limit reads in ms.

85-101: Minor · Stability & Availability · Quick win
Keep the CAP timeout active when a caller signal is present on Node versions without AbortSignal.any.
capSignal returns only callerSignal when AbortSignal.any is missing. The CAP timeout is then dropped for that request, so the request is bounded only by the caller's signal. cli/package.json declares "engines": { "node": ">=20.0.0" }, which includes Node 20.0–20.2, where AbortSignal.any is absent. If a caller's signal outlasts the configured limit (COMMONLY_CAP_TIMEOUT_MS or createClient({ timeoutMs })), the request can run past that limit. The README describes the setting as how long a request "may go unanswered before it fails."
Combine the two signals manually in the fallback path. The abort reason stays the same object, so nameTimeout still matches the CAP timeout by identity.

The current test uses a 50 ms caller timeout and a 5,000 ms CAP timeout. The fetch rejects on the caller timeout before the CAP signal fires, so it still passes if the implementation only checks capTimeout.aborted.
Add a test that delays rejection until both signals have fired, with the caller signal firing first. Assert that the caller’s original error is preserved. The test must fail if the identity check reverts to capTimeout.aborted.

CodeRabbit's follow-up at 27e84f9 (keep the CAP timeout when AbortSignal.any is missing, Node 20.0–20.2) is valid, and bb228ae fixes it. capSignal now joins the caller's signal and the cap's timeout by hand, with an AbortController that aborts with whichever source fired first, carrying that source's own reason. Two new tests stub out AbortSignal.any: a caller's longer signal still gets cap_timeout at the cap's limit, and a caller's shorter timeout keeps its own error.
The red backend shard 3 on this PR is a main-wide test failure (dailyDigestService.membership, a daylight-saving assumption), fixed separately. Nothing was skipped.

Remove fallback listeners when the request settles.

Remove fallback listeners when the request settles.
If a caller reuses an un-aborted signal, each completed request leaves this listener attached. { once: true } removes it only if that caller signal aborts. handleResponse awaits res.text(). CodeRabbit's follow-up at bb228ae (remove the fallback listeners when the request settles) is valid, and a7ecf1c fixes it. capSignal now returns { signal, release }, and timedFetch calls release in .finally, so a caller that reuses a long-lived signal on Node without AbortSignal.any doesn't collect a listener per request. A new test makes two requests on one signal with AbortSignal.any stubbed out, and asserts that every abort listener added is removed. It fails without the release (checked). Nothing was skipped.
Writer's Opus subagent review: 0 blocking, 6 of 7 findings fixed, the seventh answered in the thread.
CodeRabbit: every review through bb228ae answered. The last point (Node 20.0–20.2 fallback leaving listeners on a caller's long-lived signal) is fixed here, with a test that fails without the release.
CI green at this head: Test & Coverage passing, nothing failed.
Spec covered: a hard timeout on every CAP fetch (cap_timeout, routed to the TASK-025 retry policy), a poll-liveness exit paused during model turns and scheduled waits, and 5xx treated as backoff. Tests: a stub that answers 504 once and then hangs, and a 2.5s turn under a 1s limit that does not trip the exit.
