fix(cli): a public claude seat posts nothing unless its stream shows Commonly working (TASK-256) - #3202
Connectors TASK-256. A public seat must post nothing when its Commonly MCP isn't working. Before this PR, only the OpenCode adapter checked (#2374). On the claude adapter, the model narrated the failure and the run loop posted it to a stranger, as in the 10-21 hq-support canary. Public claude seats on this host: c4-smoke, quill, sam-claude-test.
What the stream already says
Measured with Claude Code 2.1.314, running -p … --output-format stream-json --verbose against a stub stdio MCP server:
The init event carries each server's state: mcp_servers: [{ name: 'commonly', status: 'connected' }]. When the server's command doesn't exist, the state is status: 'failed', and the model still answered HELLO with result.subtype: 'success'.
A failed call arrives as a tool_result with is_error: true, and its content is the server's own text. For the shipped MCP that's {"status":503,"body":"…","message":"HTTP 503"}, unwrapped. So isAnswerableToolError applies unchanged.
The change (cli/src/lib/adapters/claude.js)
claudePublicMcpWitness reads a public turn's stream before the reply is returned, on both the normal path and the fresh-session retry. The turn is withheld (thrown, event left unacked for retry) when:
public_mcp_not_ready: init doesn't show the shipped Commonly server connected. That includes a stream with no init event, or plain-text output. Ready is a positive witness.
public_tool_failed: a call to an mcp__<commonly>__* tool failed with anything isAnswerableToolError doesn't name. A 404 the person can be told about still posts.
Also measured: a server named commonly.me keeps that name in init, but its tools are mcp__commonly_me__*. The prefix is normalised to match. -p waits for a slow server: a 6 s initialize still reports connected, not pending.
Scope:
Only trust: 'public' seats are witnessed. A legacy internal record is read as public already (environment.js:73). Workspace seats are confined too, but they answer their own team, where narrating an outage is a useful signal. Unconfined seats are unchanged.
Only servers running the shipped Commonly MCP count, as in the OpenCode check.
The two error codes move beside the predicate in public-tool-error.js. opencode.js imports PUBLIC_TOOL_FAILED_CODE from there and re-exports it.
The catch path passes both codes straight to the run loop, never to the fresh-session retry.
✕ withholds the turn when the server failed to start
✕ withholds the turn when the stream carries no init event
✕ withholds the turn when the output is plain text
✕ withholds the turn when a Commonly call fails with an API 503
✕ withholds the turn when a Commonly call fails with a transport error with no status
✕ withholds the turn when a Commonly call fails with an empty error
✕ withholds the turn when a Commonly call fails with an error as text parts
✕ a legacy internal-trust seat is witnessed as public
Tests: 8 failed, 66 skipped, 6 passed, 80 total
Restored:
Tests: 66 skipped, 14 passed, 80 total
Full cli suite: 2282 passed and 3 failed. Those 3 also fail on main on this Mac: the Linux bwrap workspace test, seat-stop.process and seatbelt.commit-hooks.smoke. npm run lint is clean.
Not in this PR
A post made through the tool in the same turn. The witness gates the reply the run loop posts. A model can still call commonly_post_message before the witness runs. That reaches a stranger only in a partial outage, where reads fail with a non-answerable status and posting still works. In a full outage the tool post fails too, and a dead server leaves no tools. Such a turn is also retried, so its tool post can repeat. OpenCode's fix(cli): a public OpenCode seat withholds a turn whose Commonly tool failed in a way it may not narrate (0.1.119) #2374 check has the same gap. Closing it on every adapter is the MCP-side half of TASK-256, recorded there.
OpenCode's start check. That adapter's per-call check can't see a server that never started. TASK-256 stays open for it (hq-support runs OpenCode).
Adapters with no public seat (codex, pi, dsh) aren't witnessed. None of them runs a public seat today.
No version edits. The cli publishes @latest on merge.
…Commonly working (TASK-256)
A public claude seat whose Commonly MCP was down still posted: the model
narrated the failure to a stranger. Claude already runs stream-json, and that
stream says what is needed (measured on Claude Code 2.1.314 against a stub
server): the init event carries each MCP server's status, and a failed call is
a tool_result with is_error true whose content is the server's own text.
So a public turn is withheld when init does not show the shipped Commonly
server connected (including a stream with no init event), or when a Commonly
call failed with anything isAnswerableToolError does not name, the predicate
the OpenCode adapter already uses. The event stays unacked for retry. Workspace
and unconfined seats are not witnessed: they answer their own team. The two
codes live beside the predicate so both adapters share them.

Walkthrough
Claude public-seat turns now check whether the shipped Commonly MCP is ready and whether matching tool calls fail with unanswerable errors. The checks apply to initial and recovered-session replies. The changes also add shared error codes and classify those errors as runtime failures.

Summary
Shared public tool error handling cli/src/lib/public-tool-error.js, cli/src/lib/adapters/opencode.js, cli/src/lib/spawn-retry.js, cli/__tests__/spawn-retry.test.mjs
The shared module defines public_tool_failed and public_mcp_not_ready. The OpenCode adapter imports and re-exports public_tool_failed. Spawn failure classification treats both codes as runtime failures, with tests covering the classification.
Claude public MCP witness cli/src/lib/adapters/claude.js
The adapter checks the shipped Commonly MCP command. Connected init is ready; pending init requires a successful matching Commonly tool call. Failed matching calls are rejected unless their errors are answerable. Public trust enables these checks; workspace trust does not.
Turn handling and witness tests cli/src/lib/adapters/claude.js, cli/__tests__/adapters.claude.environment.test.mjs
Initial and recovered-session replies pass through the witness. MCP-not-ready and tool-failed errors bypass fresh-session recovery. Tests cover init states, tool failures, 404 refusals, server matching, retries, and trust levels.
Public-seat replies hit by a short Commonly rate limit can retry too fast and then wait many minutes for redelivery. The earlier namespace-matching concern in the Claude witness also remains open. Both should be resolved before merge.

From the review. Claude Code keeps a server's declared name in init but turns
characters outside [A-Za-z0-9_-] into `_` in its tool names (`commonly.me`
becomes `mcp__commonly_me__`, measured on 2.1.314), so a failed call from such a
server passed. The fresh-session retry path is now tested as well. Also
measured: `-p` waits for a slow server (a 6 s initialize) before init, which
then reports it connected, not pending.

Claude-Session: https://claude.ai/code/session_015AKgpQ2pr6xRMhN5mcfoHF
One round, by a fresh-context Opus subagent. At 795fb21 it gave HOLD on three points:
Name normalisation: fixed in 90b0159. Measured on 2.1.314: init reports commonly.me, and the tools are mcp__commonly_me__*. The prefix now matches, and a test covers it.
pending at init: measured, not a risk.-p waits for a 6 s initialize, then reports connected.
A post through the tool in the same turn: recorded as a residual, not fixed here. It reaches a stranger only in a partial outage. The OpenCode check has the same gap, and the MCP-side gate in TASK-256 closes it for every adapter. The reviewer agreed it shouldn't block, and asked that the duplicate post on retry be recorded too. Both are now in the TASK-256 note.
Re-review at 90b0159: PASS. This is a strict improvement that closes the two measured failures (a dead server, and a read that returns 5xx) and doesn't widen any path. The run loop (agent.js:3407-3440) leaves the thrown turn unacked and posts nothing.

Add tests for the resume rethrow and the fresh-session recovery witness.
Both changed paths have no test:
Line 1420 now rethrows PUBLIC_MCP_NOT_READY_CODE and PUBLIC_TOOL_FAILED_CODE.
Lines 1478-1482 apply the witness to the recovered turn.
Every new test in adapters.claude.environment.test.mjs runs without sessionId, so isResume is false. Reverting line 1420 leaves all tests green.
The revert matters. publicWitnessError embeds up to 160 characters of the tool error in err.message. A tool error that contains "no session" or "already in use" would match the recovery regex at line 1425 and start a fresh-session retry. The retry would bypass the withhold contract.
Add two tests with sessionId set:
The first spawn returns a stream with a failed Commonly call whose text contains "no session". Assert public_tool_failed is thrown and spawn was called once.
The first spawn fails with "already in use". The recovery spawn returns init('failed'). Assert public_mcp_not_ready is thrown.
As per path instructions: "A test that would still pass with the fix reverted is not a test (rule 1)", and comment on "missing tests for changed behaviour".

Scrub credentials from the tool error text before it enters err.message.
turnReply receives credentials and passes them to claudeResultText. publicWitnessError ignores them. The error text comes from the MCP tool result and goes into the thrown message, up to 160 characters. The run loop and logs then receive that message.
Other failure paths in this adapter (adapterFailure) exact-match redact the seat's credentials. This path does not. If a Commonly API error body or transport error echoes the bearer token, it reaches the logs.
Pass credentials into publicWitnessError and redact before slicing. Reuse the redaction that adapterFailure uses in packages/seats/src/upstream-refusal.js.

Match the complete MCP server namespace.

Match the complete MCP server namespace.
If the shipped server is commonly.me, this prefix also matches a different server named commonly_me__audit. Resolve tool names against the complete declared server namespaces before attributing a failure. (raw.githubusercontent.com)

Classify public tool failures before matching refusal text.
593-602: Major · Stability & Availability · Quick win
Classify public tool failures before matching refusal text.
A Commonly tool error can contain This content was flagged while remaining unanswerable. toolResultText preserves that text, and isAnswerableToolError rejects it.
publicWitnessError copies the text into Error.message. classifySpawnFailure matches CONTENT_REFUSED_RE before it checks error codes. The terminal handler then acknowledges the event as refused instead of retrying it.
The normal MCP-not-ready reason is only status ... or no init event, so this collision applies to the arbitrary tool-error text, not the normal witness message.

Every turn in the TASK-256 witness describe kicked a detached `npm install`
warm: one test declared a bare `commonly-mcp` command, which resolves through
the MCP home with no version. The warm outlived the suite and wrote into the
CLI temp-cleanup guard's probe root after Jest ended, so its final rmdir
failed with ENOTEMPTY. The describe now seeds a fresh registry answer, and the
unshipped case is a hand-run copy (node <path>/src/index.js), as the staging

Keep public witness failures on the retry path.
For example, {"status":500,"message":"content_policy_violation"} is unanswerable, but publicWitnessError copies that text into Error.message. classifySpawnFailure then classifies the error as CONTENT_REFUSED. …tool's text (TASK-256)
publicWitnessError quotes up to 160 chars of the tool's own error, and
classifySpawnFailure tested that text for refusal, quota and auth words

classifySpawnFailure now returns runtime for public_tool_failed and public_mcp_not_ready before it reads any text. The event stays unacked and gets retried. A vendor's "content_policy_violation", "insufficient_quota" or "403 forbidden" no longer makes the turn an acked refusal or a cooldown. Five arms pin that, with a positive control: the same text without the code still classes as a refusal. Before the fix, all five arms failed. The previous commit forced every withheld public turn to runtime, which
also turned a Commonly 429 into a 5s retry that meets the same 429.
A throttle (status 429 or rate-limit wording) now keeps RATE_LIMIT,
