fix(native): every failed ask in a shared pod gets its own couldn't-finish line - #3299
When a hosted turn that someone was waiting on fails and posts nothing, the agent posts a fixed line: "I couldn't finish a reply to this, and it didn't count against today's turns…". The text is identical every time. In a shared pod, the duplicate guard in AgentMessageService.postMessage skips any message identical to one the same agent posted in the last 30 minutes, so every repeat of the line after the first was dropped.
Production, 2026-11-01. In a chat pod with 3 members, five mention turns failed between 22:36:48 and 22:41:51Z. Only the first got the line, at 22:36:49Z, and the other four asks got silence. A single failed ask in a 1:1 agent-room got its line, because 1:1 pods were already exempt from the guard (the 2026-10-09 hq-support case).
The guard was built for heartbeat and curation re-posts crowding a shared room. It was never meant to throttle the failure line.
Change
postMessage takes a new top-level option, failureNotice, and skips the duplicate guard when it is set. Every other check still runs: sanitization, NO_REPLY handling, error routing and the consecutive-run cap.
The native runtime sets it only on the failed-turn post, which covers both the fixed line and the Model X could not answer (…) variant.
It is a top-level option on purpose. Both runtime HTTP routes pass the caller's metadata through unchanged, so a metadata flag would let any agent opt out of the guard. The routes build their postMessage arguments field by field and never forward failureNotice, and a test pins that metadata.failureNotice gets no exemption.
This doesn't add spam. The line only goes out for a directly addressed ask (chat.mention, thread.mention, dm.message, or a reply to the agent) whose turn failed with nothing posted, so there is at most one line per ask. It carries no @mention, so it can't start a cascade.
Tests
agentMessageService.summary.test.js: a failed-turn notice posts in a shared pod even when the same line went out minutes ago, and a caller's metadata.runFailed or metadata.failureNotice is still skipped as duplicate_recent.
nativeRuntimeService.guardStop.test.js: the fixed line and the model-failure line both carry failureNotice: true, and the agent's own reply does not.
Reverse check: taking failureNotice out of the guard's condition fails the shared-pod test, and dropping it from the native post fails the guard-stop assertions.
466/466 across the agentMessageService* and nativeRuntimeService* unit suites.
Review
One round (D218), a subagent review: PASS, with no must-fix items. Its one nit, pinning the model-failure variant in a test, is taken in 45c194f.
…finish' line
The fixed line a hosted turn posts when it fails with nothing posted is
identical every time, so in a shared pod the 30-minute duplicate guard
skipped every repeat. On 2026-11-01 five asks in one chat pod failed
within five minutes and only the first got the line; four got silence.
1:1 pods were already exempt. The native runtime now marks the line
failureNotice, a server-only option the HTTP routes never forward, and
the duplicate guard skips it. A caller's metadata cannot claim it.

One review round (D218) by a subagent. It passed 62a1f1d with no must-fix findings. Its one nit is fixed in 45c194f, which adds the model-failure variant's failureNotice assertion.
failureNotice is set in exactly one place, the native failed-turn post. All three postMessage calls in agentsRuntime.ts build their arguments field by field. Nothing spreads a request body into it, and setting it through metadata is refused (tested).
It bypasses only findRecentDuplicate. Sanitization, NO_REPLY handling, error routing and the consecutive-run cap all still apply.
At most one line per directly addressed ask. The line has no @mention, so it can't start a cascade.
Reverting either side turns a test red. Unit tests: 466/466.
The message service now skips recent-duplicate suppression for messages marked as failure notices. Native runtime failure notices set this flag. Tests cover duplicate handling and the flag on guardrail, model-failure, and fallback notices.

Summary
Failure notice option and duplicate suppression backend/services/agentMessageService.ts, backend/__tests__/unit/services/agentMessageService.summary.test.js
PostMessageOptions adds failureNotice. When it is true, postMessage skips recent-duplicate lookup. Tests cover this behavior and caller-supplied metadata.
Native runtime failure notice posts set failureNotice: true. Tests check the flag on guardrail and model-failure notices, and distinguish the fallback notice from the agent reply.
Team-Commonly/commonly#1012: The issue describes failed turns that produce no output. This change addresses the shared-pod duplicate-suppression case, but not the broader behavior.
The change lets repeated failure notices bypass the duplicate guard. A fourth consecutive failed ask from the same agent in a pod can still receive no failure line because the run cap refuses it silently. Exempt failure notices from the cap before merging, or the fix only partly resolves the reported problem.

Exempt failure notices from the consecutive-run cap.
failureNotice bypasses only duplicate suppression. It does not bypass the consecutive-run cap. With the default cap of three, the fourth failure notice can be refused when no human or other agent message appears between the failed turns. postMessage returns success: false with refused: true; nativeRuntimeService ignores that result, so the asker receives no notice. Successful reply posts also count toward the same run.

Update the consecutive-run cap check in the `postMessage` flow so
`failureNotice` messages bypass the cap while ordinary messages remain subject
to it; preserve the existing duplicate-suppression behavior. Add a regression
test confirming four queued failed asks in the same shared pod all deliver their
failure notices.

Reply to the point in CodeRabbit's review body at 45c194f: "Exempt failure notices from the consecutive-run cap" (agentMessageService.ts:1571).
Not taken, on purpose. The scenario is real: four asks queued with nothing posted between them could produce four failed turns, and the cap of 3 would refuse the fourth line. But the cap does the same to four ordinary replies in that position. It exists to stop one agent posting run after run of its own messages, and four identical "couldn't finish" lines in a row are exactly that.
This PR fixes a different failure. There, the line was dropped even though the person had spoken since the last one. Each ask was a separate human message, and the duplicate guard still swallowed it, so the person got nothing back. With this change, a person who asks again gets the line again. A burst of queued asks gets up to three consecutive lines, which is enough to say the agent is failing. The same review round looked at this case and kept the cap. Both suites pass at this head.
