Troubleshooting and FAQ #552
Description
Write docs/troubleshooting.md for the failures contributors actually hit: pnpm install failing on a bad lockfile/package.json mismatch, CI passing locally but failing on a floating toolchain, migrations silently not applying because the journal omits them, tests interfering through shared in-process counters, and the web app failing to build after a dependency drift.
Acceptance criteria:
- Each entry states the symptom, the cause, and the fix
- Includes the "my migration didn't run" case, since the journal makes this silent rather than loud
- Includes the "works locally, fails in CI" cases with their known causes in this repo
- Grows from real incidents rather than hypotheticals — cite the actual failure text where it is distinctive
