feat: ingest Codex sessions through provider registry

Summary

• add a read-only Codex SQLite/rollout provider and provider registry
• hydrate transcript-detected Codex children when their local thread remains available
• preserve unavailable child transcripts and Codex stores as explicit incomplete data
• route the React index worker through provider references

duration_ms 4561.463575 currently has one failing server test: its empty-Claude-corpus fixture also discovers the host Codex store, so indexing does not become empty/complete within its expected timeout. The failure is documented for follow-up before merge.
