Define the durable state update for the heartbeat observer.
run_or_resume_with_sender has no WorkflowContext in its heartbeat branch. Although WorkflowEngine::state_store().update(run_id, ...) can persist a mutation, the engine later assigns the step's cloned context back to s.context. That checkpoint can overwrite a concurrent observer update. Define a workflow-owned observer or an engine/store integration that atomically merges last_seen_comment_at and pending_corrections and preserves them across step checkpoints and crash recovery.

Frame item comments as untrusted content. The proposal prepends raw comment text to the role prompt, while existing item-description handling marks external content as untrusted. A comment can contain role-like instructions that the agent may follow. Wrap comments with the same external-content markers and state that comments are data, not higher-priority instructions.

Acknowledge corrections only after a successful role turn.
build_sdd_loop_step runs as a loop step, and execute_loop reuses the same WorkflowContext across retries. If pending_corrections is cleared before send(role_invocation) and that call fails, the next retry sees an empty list and builds a prompt without the correction. If that retry succeeds, execute_loop persists the cleared context, so the advanced cursor has lost the correction without delivering it successfully.
Keep pending_corrections until the role call succeeds and the loop iteration is durably persisted, or store a replayable acknowledgement state. The transactional retry and SQLite write do not prevent this: retries intentionally retain the cleared in-memory context, and the later successful context is what gets committed.

Perform an atomic correction check before completion.
The finalization branch only checks pending_corrections. After the observer's last poll, a new comment remains absent from that list. item_done only checks and refreshes claim ownership, then performs the push/PR work. Its normal completion path calls mark_completed, which checks only claim ownership before changing the item state. Neither path lists comments or handoffs, nor compares a comment/handoff revision.
A correction posted after the last observer poll can therefore remain unseen while item_done opens the PR or marks the item completed. Add a final comment and handoff check, and pass its revision into an atomic completion precondition that rejects completion when either value changed.

Resolve the handoff detection path before completing this design.
The item_id handoff branch updates assignee_agent and metadata on the existing item without creating a comment. The proposed comment poll cannot observe this ownership change. Because the design promises detection between claim and PR creation, this case is in scope. Define the handoff event or cursor, and specify whether detection releases, blocks, or reroutes the current run. Leaving both options open makes the design incomplete.

Correct Item #265’s timeline.
The paragraph states that PR #751 was posted as complete (1789568917), then describes the correction as targeting “still-open #265 work.” If that timestamp marks completion, this is a post-completion follow-up, not a mid-flight correction. Replace the example or cite evidence that #265 remained open at 1789569350.

Use a stable (created_at, comment_id) cursor.
The proposed timestamp-only cursor can omit a comment posted in the same second as the cursor. Comment timestamps have second resolution, and list_by_item permits multiple comments at that timestamp and orders them by created_at ASC, id ASC. Persist (created_at, comment_id) and advance it with the same lexicographic order.

…anism (item #269) Root-caused why a comment/handoff correction posted after an agent claims an item is never read before that agent opens/merges its PR: tasks are synthesized once from the item description at dispatch, every role prompt is built only from that frozen task plus prior review findings, and finalize calls item_done with no check of the item's live comment history. Reproduced live on items #265/#266 (PR #753 shipped mise which -t after a comment said to use mise env --json instead). Proposes piggybacking a comment/handoff poll on the existing claim heartbeat loop, feeding pending corrections into the next role prompt and gating finalize's item_done call the same way hold_reason already does. Committed. Status summary:
What I did: Investigated item #269's gap — dispatched agents never seeing a comment/handoff correction posted after they claim an item, before opening/merging their PR. Traced the full pipeline (
work_item_pipeline.rs,task_sourcing.rs,prompt_builders.rs,mcp_server/{comment,handoff,item}.rs) and confirmed the root cause: tasks are synthesized once from the item description at claim time, every role prompt is rebuilt only from that frozen task plus prior review findings, andfinalizecallsitem_done(which opens the PR) with no check of the item's live comment history —comment(action="list")exists but nothing in the automated pipeline ever calls it. Pulled the actual comment timelines for items #265/#266 as concrete evidence (a correction on #266 was posted mid-dispatch and still ignored three dispatch attempts and 23 minutes later when PR #753 shipped the disregarded approach).Wrote the deliverable doc at
docs/superpowers/plans/2026-09-17-mid-flight-item-correction-detection.md, proposing: piggyback a comment/handoff poll on the existing claim-heartbeat loop (work_item_pipeline.rs:1169, already survives crash-restarts), buffer new comments into apending_correctionsfield, inject them into the next role prompt at each task turn, and gatefinalize'sitem_donecall the same wayhold_reasonalready gates it if any correction is still unconsumed. Also covered why simpler alternatives (re-synthesizing tasks per-turn, relying on the agent to self-check comments) don't fit, and flagged open questions (handoff-onto-claimed doesn't write a comment at all, so it needs its own check; possible judge-reopen integration; poll cadence).Tests run: none — this is a doc-only deliverable per the task description, no code changed.
Concerns:
docs/is gitignored repo-wide, but the existing precedent doc (2026-08-17-sdd-loop-session-resume.md) is force-added and tracked, so I force-added and committed mine the same way. The proposal is a design, not an implementation — a follow-up item would need to actually build thepending_correctionsfield and the two checkpoint call sites.Opened by
