For a successful command such as cargo test && sed -i 's/old/new/' src/main.rs, the start and end fingerprints differ. This branch returns None. src/hook_completion_gate.rs still records passed: true, and evidence_fresh at Line 285 accepts that evidence for 30 minutes. The completion gate therefore accepts verification of an untested tree.
Keep a known mismatch distinct from an unavailable fingerprint. When the fingerprints differ, discard the verification evidence or mark it as failed. Add a test that checks has_fresh_passing_verification after this mismatch, not only the helper result.
