fix(item): make plan approval immutable against stale metadata writes (#300) - #891
Conversation
…data writes (#300) crud::update now keeps plan_status/plan_approved_at/plan_approved_by when a wholesale metadata write would revert an approval, unless it is an explicit reject, resubmit, or fresh approval. …rites (#300) Treat pending without a new plan_asset_id as non-resubmit, and cover the stale pending+same-asset shape through item::update (the path used by merge_item_metadata / orphan reconcile / handoff). …s no agent identity (#300) CLI approve-plan and Telegram channel approve used AgentflareMcp with agent=None, so serde_json wrote plan_approved_by:null and the audit trail looked empty after a successful human approval. Split plan-transition strip/restore out of item.rs to stay under the frozen LOC gate. Priority: ➖ Normal Estimated code review effort: 3 (Moderate) | ~20 minutes Change: Bug fix Merge Risk: 🔵 Low · up to This fix prevents most stale writes from reverting an approved plan. A writer holding metadata from an earlier plan cycle can still reopen the plan gate. That is a narrow edge case worth owner awareness or a follow-up, and it should not block the merge. 🚥 Pre-merge checks | ✅ 5✅ Passed checks (5 passed)

A stale snapshot from an earlier plan cycle can still revert an approval.
explicit_transition decides from the shape of the incoming metadata, not from what the writer meant to do. Here is a realistic sequence:
- A plan is submitted with
plan_asset_id: "a1"and rejected. - It is resubmitted with
"a2"and approved. - A writer that read the metadata during cycle 1 writes it back with
plan_status: "pending"(or"rejected") andplan_asset_id: "a1".
In step 3, "a1" != "a2" matches the resubmit arm. A stale "rejected" matches the reject arm. Either way, the stored approval reverts and the gate closes again. This is the same stale read-modify-write problem that live #281 showed, now in the multi-cycle case.
The real transition writers are known: item_submit_plan and set_plan_status. Give them an explicit channel instead of inferring intent from content. Two options:
- Add a dedicated backend function or
UpdateItemflag for plan transitions. With that in place,preserve_plan_approvalcan always preserve the stored transition fields for ordinary metadata writes. - Use a compare-and-set on a monotonic
plan_revisioncounter stored in the metadata.
