`first_time(&sid, "stop")` branch so a blocking reminder leaves the session busy
when it starts a continuation turn; either account for the reminder before
marking the session idle or restore the busy state when emitting the blocking
decision.
Review comments at @src/memory_nudge.rs:
- Line 73: Update save to report whether the ledger was persisted, and make
claim_fresh return None when that update fails; only return fresh keys after a
successful ledger write.
- Line 58: Update the ledger key used by `map.entry` to include both the agent
identity and session ID, following the existing runtime session-tracking
pattern, so entries from agents sharing a session ID remain separate.
- Around line 154-155: Update the flow around `claim_fresh` to select and record
only up to `TOP_K` observations that can be emitted, leaving unselected hits
eligible for later recall. Ensure the emitted observations and ledger entries
correspond to the same selected keys.
- Around line 145-146: Resolve the active project at the hook boundary and pass
its scope to recall_with for both PreEdit and TestFailed instead of passing
None, which leaves FTS and vector searches unfiltered. Keep global observations
governed by an explicit scope policy.

🟡 Minor · Apply project-or-global scope before candidate limits. · memory_nudge.rs:163-178

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick winApply project-or-global scope before candidate limits.
The pre-edit and test-failure hooks call
recall_for, which retrieves an unscoped window and filters it afterward. Other-project observations can consume the FTS and vector candidate windows first. A current-project or global observation can then be unavailable to the nudge.Add an explicit project-or-global scope to both retrieval branches before their limits. Do not pass the current project to the existing project-only filter, because that excludes global observations. Keep the post-filter as a defense against other-project results.

…claim no longer hides in silence (#946) An item whose `metadata.pr` record was lost -- a redispatch or a worktree cleanup strips it -- became invisible to the review sweep: the self-heal reads the PR number out of that metadata, and the PR discovery path skipped the PR outright because agentflare had opened it itself. The item sat outside "in_review" with an open, approved, CI-green PR that nothing would ever merge, and nothing said so (item #392, seen on PR #926). Two changes: - `discover_untracked_prs` now lets an agentflare-stamped PR reach the attach path when a local item owns its branch, so the item regains its PR number and the sweep sees the PR again. The stamp is only a "this one is ours" marker; the branch ownership is what decides where it goes. A stamped PR with no local owner is still never adopted, so no duplicate tracking item is minted. - The sweep no longer drops a stray item in silence when a live claim holds it. The claim still keeps the sweep from restoring the item -- somebody is working on it -- but the sweep now names the PR, the item and the claim holder, once per item, the same way the dispatch-gate notice does (item #341). The count is on `ReviewSweepResult` so an operator can see the sweep holding something back.
