ADR-0016 third amendment (proposed) and specs/managed-session-hooks-prd.md: FastSkill hooks four agents, the timer reaches the rest, existing Gemini hooks are removed on apply, Pi gets an aikit-owned extension file, and `managed hooks --system` prints Codex's entry as TOML.
