feat: add L-HEVC (Layered HEVC) codec string support - #12

Parse and generate lhv1/lhe1 codec strings (SHVC / MV-HEVC, ISO/IEC 14496-15). The profile-tier-level part uses the HEVC syntax and is decoded by reusing H265Info (exposed via .ptl); the optional trailing scalability field (.S<mask>) is preserved verbatim in .scalability, since its byte encoding is not freely specified. Wired into codecInfoFactory via the lhv/lhe prefixes.

H265Info(exposed via.ptl). The optional trailing scalability field.S<mask>is real but its byte encoding is not freely specified (ISO 14496-15 is paywalled; paulhiggs/codec-string only labels lhv1 without parsing), so it is preserved verbatim in.scalabilityrather than guessed.15 tests; full suite 225 passing.
