New Revision (2026-10-01, v1.217.0) records the cause, the change (MMCA.Common #465) and the seven host call sites (ADC #234, Store #179). ADR-115 point 10: the transformers apply to every document the host registers. Site re-rendered. ADR-016. MMCA.Store #180 (merged, e3f9d166) rewrote the emulator-tier comment that ADR-016 recorded as stale (it named 8.5.10 against an 8.5.11 entry). This updates the Decision parenthetical and the Revision (2026-10-01) to match.
