Summary

The main-account hard lock (default on since #5694) stops opencodex from sending new main-account requests at 98% on either the 5-hour or the weekly window. That keeps opencodex-driven usage from reaching the 100% reading at which the Codex desktop app disables Send (#6196, #4878), but two gaps remain: a long turn on a small Plus 5-hour window can cross 98% before the next observation, and usage outside opencodex (ChatGPT web or mobile, Codex cloud, another machine) can exhaust the account where the lock cannot see it.

This PR narrows both gaps without changing the lock's shape:

  • Per-window thresholds. Each governing window now has its own threshold. The 5-hour window locks at 90% by default and the weekly/monthly window stays at 98%. Both are configurable with the new codexMainAccountHardLockThresholds: { short, long } (integers 80–100, short ≤ long). PUT /api/settings validates the object before writing, accepts a partial update, and restores it on rollback. A malformed value on disk falls back to the defaults.
  • Retention follows the lowest allowed threshold. The three policy-evidence retention guards in src/codex/quota.ts now keep any reading at or above 80%, so a configured threshold below 98 cannot be silently unlocked by an elapsed reset or a partial WHAM update. Retaining a reading below the active threshold only keeps the percentage visible; it never blocks.
  • Outside-usage warning. A small process-local detector (src/codex/main-account-external-usage.ts, no persistence and no timers) compares fresh main-account observations. If usage rises inside the same reset episode while opencodex has not used the main account, the dashboard shows "Possible usage outside opencodex. The lock may not prevent exhaustion.", and ocx status prints the same note. The warning expires after 6 hours or at the window reset, and an identity change clears it.
  • Copy. The blocked error, the dashboard setting, the main account card, and the ocx status protection line show the effective thresholds instead of a fixed 98% in all ten locales. ocx status --json gains a whitelisted mainAccountHardLock projection. The human report reuses the existing OAuth health read, so it does not query the accounts endpoint twice.

Default behaviour change to note in the release: an account whose 5-hour window reaches 90% is now held off the main account (other pool accounts and providers stay available). Set codexMainAccountHardLockThresholds.short to 98 to restore the previous behaviour.

This is the first item of the split roadmap in devlog/_plan/261001_quota_send_lock_split/ (included here). The app-server shim, the local-CA intercept, and the PAC fallback from #5947 are separate PRs and are not part of this change.

Main-account protection setting showing per-window thresholds

Verification

  • Hosted CI on the exact head 5d8ba39cb78a53571882de5201606787008b575f: 32 checks passed, 5 conditional matrix jobs skipped, none pending or failed (run 36831869526). The first head 96904e5758 failed structure gate (three structure docs over the 600-line budget) and one exact status assertion in codex-quota-auto-refresh-main-admission.test.ts; both are fixed in 5d8ba39cb7.
  • Local test suites and typecheck were not run, by maintainer instruction for this lane. This PR is verified by hosted CI on the exact pushed head.
  • An independent read-only review of the full diff against 010_hard_lock_hardening.md returned GO-WITH-FIXES with three P2 findings. All three are fixed in this head: the exact resetAt contract is restored in main-account-hard-lock-policy.test.ts, GUI tests now cover the outside-usage warning in both the setting and the main card, and the duplicate ocx status accounts fetch is removed.
  • Manual checks: no file in tests/fixtures/file-size-baseline.json exceeds its cap (src/cli/index.ts stays at 1979 lines), all ten locale catalogs carry the same new keys, and both new test files are registered in scripts/test-layout/layout.json and tests/fixtures/test-layout-expected.json.
  • Screenshot: dashboard served by this branch from an isolated OPENCODEX_HOME, Codex Set → Advanced settings.

New and updated tests: tests/codex-integration/main-account-hard-lock-thresholds.test.ts, tests/codex-integration/main-account-external-usage.test.ts, tests/config/settings-main-account-hard-lock.test.ts, gui/tests/main-account-hard-lock-setting.test.tsx, plus threshold updates in the existing hard-lock suites.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Summary by CodeRabbit

  • New Features
    • Main-account quota protection now blocks independently at configurable short- and long-window thresholds, defaulting to 90% and 98%. Monthly-only accounts use their monthly quota window.
    • Settings, account cards, and CLI status display effective thresholds and may show a warning when usage appears to have increased outside OpenCodex.
    • Configuration and status documentation now describe the thresholds and warning.
  • Bug Fixes
    • Protection now requires each blocking window to report usage below its own threshold before automatically unblocking.

…de-usage warning

The main-account hard lock now compares each governing window with its own
threshold: the 5-hour window locks at 90% and the weekly/monthly window at 98%
by default, both configurable through codexMainAccountHardLockThresholds
(integers 80..100, short <= long, malformed values fall back to defaults).
Evidence retention keeps any reading at or above 80% so a lower configured
threshold cannot be silently unlocked by an elapsed reset or partial update.

A process-local detector compares fresh main-account quota observations and
flags a rise inside the same reset episode while opencodex has not used the
main account, so the dashboard and ocx status --json can warn that usage is
being consumed outside opencodex. The human ocx status line reuses the OAuth
health read instead of fetching the accounts endpoint a second time.

The hard lock now supports configurable short- and long-window quota thresholds, and status surfaces can report matching external-usage warnings. New planning documents outline quota-send work phases, their dependencies, and closeout steps.

Changes

Main-account hard-lock policy

Layer / File(s) Summary
Threshold configuration and admission policy
src/codex/quota-types.ts, src/config/schema/config-schema.ts, src/types/config.ts, src/codex/main-account-hard-lock.ts, src/codex/auth-context.ts, src/codex/quota.ts, tests/codex-integration/*, structure/config.md, structure/providers/openai-tiers.md
Short- and long-window thresholds default to 90% and 98%. Configuration accepts integer values from 80% through 100%, with the short threshold no greater than the long threshold. Status includes effective thresholds and the blocking window. Quota evidence is retained down to the minimum threshold.
Quota evidence and external-usage detection
src/codex/main-account-external-usage.ts, src/codex/quota.ts, src/codex/auth-context.ts, src/codex/auth-api/account-list.ts, tests/codex-integration/main-account-external-usage.test.ts, structure/providers/openai-accounts.md
Quota observations feed an identity-scoped detector. It records a warning for a same-reset increase of at least one percentage point when no recent main-account activity meets the quiet-period condition. Warnings expire at reset or after six hours.
Settings validation and persistence
src/server/management/config-routes.ts, tests/config/settings-main-account-hard-lock.test.ts, docs-site/src/content/docs/reference/configuration/server.md
The settings API validates threshold updates, checks partial updates against stored values, and restores previous values after a save failure. Settings responses include policy status and any matching external-usage warning.
Policy status in CLI and GUI
src/oauth/health.ts, src/cli/*, gui/src/components/*, gui/src/hooks/useCodexAccountPool.ts, gui/src/i18n/*, gui/tests/main-account-hard-lock-setting.test.tsx, tests/oauth/oauth-health.test.ts, docs-site/src/content/docs/ko/reference/cli/providers-accounts.md, structure/runtime.md, structure/dashboard-and-usage.md
The account-list and live-proxy health paths expose policy status and warnings. CLI and GUI status displays show effective thresholds and warnings when present.
Policy documentation and integration coverage

Merge Risk: 🔵 Low · up to ddb24

Some outside-usage warnings can be missed after cooldown refusals or an account identity change. The hard lock remains in place, but users should not rely on this advisory as a complete signal; the merge risk is bounded.

Security Architecture Review

Security architecture risk: 🔵 Low · up to ddb24

The lower default short-window threshold provides more headroom, while configuration changes retain existing access controls. Two activity-accounting gaps can hide outside-usage warnings after account changes or refused requests. Neither gap bypasses the hard lock.

Retained concerns

  • Low · reliability · inferred: The new detector scopes its baseline to an identity but keeps activity in an unkeyed process-global timestamp. Production account replacement clears the baseline without clearing that timestamp, so activity from account A can suppress an otherwise qualifying outside-usage warning for account B. This weakens exhaustion observability across account lifecycle transitions; it does not transfer credentials or bypass admission.
  • Low · reliability · inferred: The new activity timestamp advances when the hard-lock check passes, before later cooldown, entitlement, reserve, or profile-claim checks can refuse the request. Attempts that produce no upstream usage can therefore suppress qualifying outside-usage warnings. Repeated refused attempts can degrade the exhaustion advisory while the admission controls themselves continue to refuse traffic.
Security review details

Security Blast Radius

  • inferred — The supported exposure is the existing management settings boundary and the process's main-account admission, quota, and advisory state. The identified defects affect warning accuracy for the observed main identity, including sequential account replacements; they do not demonstrate access to another account's credentials or broader service authority.

Security Findings and Attack Paths

  • inferred — An attempt reaching a main-account policy check can advance advisory activity even if a later local control refuses it. Such attempts can mask externally driven quota increases, but the evidence does not establish unauthenticated reachability, additional upstream consumption, or a hard-lock bypass.

Trust Boundaries and Controls

  • observed — The settings route remains behind the existing centralized management-authentication gate. Threshold input is explicitly validated at that route, and the shared resolver normalizes values again before admission and status consumption. No alternate threshold-writing route or authorization bypass was evidenced.

Resilience and Maintainability Implications

  • observed — Quota publication retains identity and writer-generation checks, while main-account probe retries reject obsolete dispatches or credentials before publishing. Confirmed identity transitions purge main quota and policy state. These controls contain stale-response effects even though advisory activity cleanup is incomplete.

Hardening Proposals

  • proposed — Associate activity metadata with the observed account identity, reset it at the corresponding lifecycle boundary, and advance it at an explicitly defined usage boundary after local refusals. Validate account replacement and refused-request sequences against the advisory's externally visible contract.

…status in admission tests

structure/config.md, runtime.md and dashboard-and-usage.md fold the new notes into existing paragraphs so each stays at the 600-line budget; the threshold contract stays in providers/openai-accounts.md. The native-main admission test now asserts the complete status with thresholds and the blocking window.

🟡 Minor · Clear the activity timestamp when replacing main-account… · main-account-external-usage.ts:13-37

src/codex/main-account-external-usage.ts:13-37
🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Clear the activity timestamp when replacing main-account runtime state.

A confirmed A-to-B switch calls purgeMainCodexAccountRuntimeState() and forgetMainAccountUsage(). That function clears baseline but leaves the module-global lastActivity unchanged. If B receives a baseline reading and then a same-reset increase within 30 minutes of A's activity, the quiet-period check suppresses B's warning even though B had no recent activity. This leaks A's activity into B's identity-scoped detector state.

Suggested fix
 export function forgetMainAccountUsage(): void {
   baseline = undefined;
+  lastActivity = -Infinity;
 }

 export function resetMainAccountExternalUsageForTests(): void {
   forgetMainAccountUsage();
-  lastActivity = -Infinity;
 }

Both hard-lock surfaces now pass the management payload's thresholds through hardLockThresholds, which falls back to the server defaults unless both values are finite percentages. Adds focused tests for projectMainAccountPolicyHealth's rejection branches and external-usage whitelisting.
A hard-lock refusal used to note main-account activity before the policy check, so repeated refused requests suppressed the outside-usage warning exactly while the lock held. Activity is now noted after the request is admitted. The settings route also rejects an empty codexMainAccountHardLockThresholds object instead of storing {}. Both found by the independent security review of a747130.

On macOS tmpdir() is under /var, a symlink to /private/var. The no-follow writer resolves the registry's parent, so the icacls stub saw /private/var paths, failed its root-prefix assertion inside the swallowed registration and left the registry empty. #6375's CI never ran the macOS shards; this PR does, and macos 1/2 failed at owner-registry-acl.test.ts:72. Test-only fix.

🟡 Minor · Clear activity when the main identity changes. · main-account-external-usage.ts:16-24

src/codex/main-account-external-usage.ts:16-24
🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Clear activity when the main identity changes.

lastActivity is global, but baseline is identity-scoped. After the A→B transition, activity from A can remain within the quiet window when B records its baseline. A same-reset increase of at least one point for B can then suppress the warning, even when B has no recent activity.

purgeMainCodexAccountRuntimeState() already clears the main usage baseline through forgetMainAccountUsage(). Clear lastActivity there as well. Do not address this only by excluding cooldown refusals from activity recording; that does not remove activity recorded for A.

Suggested fix
 export function forgetMainAccountUsage(): void {
   baseline = undefined;
+  lastActivity = -Infinity;
 }

Update forgetMainAccountUsage to also reset lastActivity when clearing baseline,
so activity from a previous main identity cannot affect the next identity’s
usage checks.

Owner Author

Maintainer integration record (MAINTAINERS.md, dev-only)

  • Actor: @lidge-jun (admin). I'm integrating this maintainer-authored PR into dev without a second maintainer approval, as the project owner authorized on 2026-10-01.
  • Exact head: ddb24e9b2fecc5d5cd0d347e6021ca0863f56335. It merges dev 06cc3815c0. The current dev (328ce95814, three newer commits) merges with it cleanly, and GitHub reports it MERGEABLE.
  • Hosted CI at this head: Cross-platform CI 36898487664 passed on attempt 1 (pull_request). The jobs that ran and passed were test 1/4–test 4/4, macos 1/2, macos 2/2, macos widget + bundle, desktop shell, gates, structure gate, storage policy, docker smoke, api usage, docs site build, npm-global ×3, keyring ×3 and ci. React Doctor, Service lifecycle, hygiene and enforce-target also passed.
  • Local suites: not run, on the owner's explicit instruction. Hosted CI is the only execution evidence.
  • Security review: an independent reviewer returned PASS at a747130875 with two low-severity notes. Both were fixed in 64f95b6224: activity is now counted only after admission, and an empty thresholds object is rejected. The reviewer re-attested PASS at bacd1ab26f, 876f18b509 and ddb24e9b2fecc5d5cd0d347e6021ca0863f56335.
  • Review threads: both CodeRabbit findings were fixed in f2cb1db824. These were the threshold shape guard and the projectMainAccountPolicyHealth tests. A third, about cooldown-refusal activity, was answered with the reasoning for keeping the current behaviour. All threads are resolved.
  • Earlier CI on this head line:
    • macos 1/2 at bacd1ab26f failed in the dev test owner-registry-acl.test.ts (macOS /var symlink). That was later fixed on dev by test(config): canonicalize the owner-registry ACL test root (macOS) #6411, and dev's version was taken here.
    • desktop shell at 876f18b509 hit the 45-minute job limit while installing Tauri apt dependencies. It was rerun and passed.
    • enforce-target was cancelled by a gate concurrency group and passed on rerun.
  • Maintainer objections: none. assert-mergeable-review.sh --maintainer-integration 6363 exited 0.
