Summary

Refs #2999 (the native-main publication issue; this closes the token-loss half raised in the #3000 review).

A successful grant_type=refresh_token exchange retires the refresh token it sent. Reviewing #3000, the maintainer wrote: "a caller that cancels late discards the only live refresh token and the account needs manual reauth. A cancelled wait must not decide the fate of a refresh that already succeeded." Current dev still loses the rotated token in two ways inside resolveMainAccountToken (src/codex/main-account.ts):

  1. Abort after the exchange. After refresh(...) resolved, if (dependencies.signal?.aborted) throw ran before persistRefreshedMainAuthJson. It was added in fix(codex, usage): bind account-scoped evidence to credential generation and add privacy-bounded cache diagnostics #5268 and pinned by the test a caller cancelled during the refresh does not commit the late result.
  2. Abort during the exchange. The caller's signal went into the exchange: AbortSignal.any([caller, refreshTimeout]) was passed to fetch and to the body read. A disconnect after the POST was sent aborted a request the provider may already have completed.

In both cases auth.json kept the retired token, and the next refresh got refresh_token_reused.

Changes:

  • Caller cancellation is checked before the exchange starts, and can still stop either lock wait.
  • Once started, the exchange observes only refreshTimeout (30 s).
  • On success the result is always published through the unchanged snapshot/identity guards. The rejection and reauth state are then cleared, and only after that is the caller's abort reason thrown. The request path still sees a cancellation.
  • An external writer that replaced auth.json during the exchange still wins: the publication is refused, as before.
  • withNativeMainExclusiveClaim (native-main-claim.ts:195) and withCodexRefreshFileLock (account-store.ts:833) await the callback and release in finally. A cancelled wait therefore cannot free an acquired lock mid-exchange. No helper changes were needed.
  • The test from fix(codex, usage): bind account-scoped evidence to credential generation and add privacy-bounded cache diagnostics #5268 is inverted, and two tests are added. responses-alternate-main-cancellation's cancel-success case now expects the rotated grant to be saved, with no follow-up inference. cancel-terminal still expects no state change.
  • structure/providers/openai-accounts.md gains a "Native-main refresh cancellation" section, structure/codex-home.md links to it, and docs-site/.../guides/providers.md describes the behavior.

Out of scope:

  • The check-then-rename(2) window from [Bug]: Native-main refresh publication can overwrite external Codex writers #2999. No FFI or new replacement primitive is added.
  • Other refresh paths that can lose a rotated token when a caller passes its own signal. They are listed for follow-up, not changed: src/oauth/chatgpt.ts:338, src/oauth/xai.ts:351 (local Grok import), and src/oauth/nous.ts:718. The stored-pool flight in src/oauth/index.ts and src/codex/account-store.ts:1252 already detach the exchange from caller cancellation.

This is an authentication surface, so it needs maintainer-sponsored.

Verification

Test runner: Bun 1.4.0 (OCX_TEST_RUNNER_BUN).

  • bun run test -- tests/codex-integration/codex-main-account-refresh.test.ts tests/responses/responses-alternate-main-cancellation.test.ts tests/responses/responses-native-main-refresh.test.ts: all pass (12 + 6 + 19).
  • Before the fix, on 93ed1a40b: the main-account file had 9 pass / 3 fail and the alternate-main file 4 pass / 2 fail. Each changed cancellation case failed.
  • bun run test -- tests/codex-integration/codex-main-account-refresh.test.ts tests/responses/... tests/ci-workflows/ --parallel=1: 1562 pass, 1 fail. The failure is bun-runtime.test.ts "PATH-selected process provenance", which fails the same way on unmodified dev on this host (PATH has an older Bun).
  • bun run test -- tests/codex-integration/ tests/responses/ tests/codex/, then a serial rerun of the files that failed under load: 8 still fail. They are in codex-shim-fnm, codex-shim-standalone, codex-shim-readiness and codex-catalog-sync-hardening. Every one also fails on unmodified dev on this host, and none imports the refresh path.
  • bun run typecheck, bun run structure:check, bun run privacy:scan: pass.
  • I did not run the full bun run test: the host's load average was ~33 on 4 cores, causing suite-wide 5 s timeouts. Remaining coverage is left to CI.
  • docs-site build not run locally.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • Bug Fixes

    • Refresh cancellation now stops a refresh before token exchange begins. If cancellation occurs after the exchange starts, a successful refresh can still save updated credentials before cancellation is reported.
    • External changes to credentials continue to take precedence, preventing a refresh from overwriting them. Cancelled requests do not proceed to inference.
  • Documentation

    • Clarified refresh cancellation behavior and how device reauthentication differs from credential refresh.

A successful refresh_token exchange retires the token it sent. Two paths in
resolveMainAccountToken still lost the replacement when the caller went
away:

- the caller's signal was passed into the exchange, so a disconnect after
  the POST was sent aborted the request or its body read;
- after the exchange resolved, a caller-abort check threw before
  persistRefreshedMainAuthJson ran (added in lidge-jun#5268).

Either way auth.json kept the retired token and the account needed a
manual re-login. Caller cancellation now stops only the waits before the
exchange. The exchange observes the refresh's own 30-second timeout, the
result is published through the unchanged snapshot/identity guards, and the
caller's abort reason is thrown after the commit. An external writer that
replaced auth.json during the exchange still wins.

The claim and refresh-lock helpers await the callback through completion,
so a cancelled wait cannot release an acquired lock mid-exchange.

Native-main credential refresh now checks cancellation before starting. Once token exchange starts, caller cancellation does not abort the exchange. Successful credentials are published before cancellation is reported, unless an external auth-file change prevents publication.

Refresh cancellation and publication
src/codex/main-account.ts, structure/providers/openai-accounts.md, structure/codex-home.md, docs-site/src/content/docs/guides/providers.md
The resolver checks for cancellation before refresh and uses the refresh timeout during token exchange. On success, it publishes credentials and updates rejection state before returning a caller abort reason. The documentation describes cancellation, locking, publication, and device reauth behavior.
Cancellation and external-writer tests
tests/codex-integration/codex-main-account-refresh.test.ts, tests/responses/responses-alternate-main-cancellation.test.ts
Tests verify successful publication before cancellation is returned, continued exchange after caller cancellation, rejection of publication after an external auth-file change, and prevention of subsequent inference in cancelled scenarios.

The change is mergeable with a small test follow-up: assert the saved account identity so these tests catch an identity mismatch alongside token rotation.

For the native main Codex credential in `CODEX_HOME/auth.json`, cancelling a request stops a

P2 Badge Allocate the timeout when the exchange starts

When either lock wait consumes the existing deadline, this advertised 30-second exchange window does not exist: resolveMainAccountToken creates refreshTimeout before withNativeMainExclusiveClaim and withCodexRefreshFileLock (src/codex/main-account.ts:474-495), then passes that already-aging signal to refresh at line 515. For example, reclaiming a roughly 31-second-old crashed lock can take almost 29 seconds, leaving the token POST about one second; the provider can rotate the token before the client aborts and auth.json then keeps the retired grant. Use separate wait and freshly-created exchange timeouts, or state that this is a shared end-to-end deadline.
