What a user gets

A seat can give a slow refocus trace more time, and a slow attempt is reported honestly. This is a budget-and-reporting
change. It is not full refocus parity: overlapping hook runs are still not serialized (see "Not resolved").

  • Before:
    • A seat with a slow trace had no supported way to give it more time. Even a larger inner timeout was killed by the
      registrations' 5 s.
    • A default fire whose lookups (the trace, rig queue whoami, the content REF) each stayed under 2 s but together
      passed 5 s was killed with nothing delivered and nothing recorded.
  • Now:
    • A selectable budget: OPENRIG_REFOCUS_BUDGET_MS in a seat's environment, from 4,500 ms (the default) to
      52,000 ms. With a budget selected, each lookup's cap scales with it (the trace gets about 23 s at 52 s) and every
      lookup is clipped to what's left, so a slow attempt ends as a reported gap. A single 45 s trace call is not provided.
    • The default lookups are main's: fixed 2 s caps, unclipped, with the old-CLI whoami retry.
    • The refocus.cjs registrations allow 60 s (both harnesses). Every other hook keeps its registration, including
      Claude's PreCompact at 30 s.
    • The failure report gets its own 1.5 s after the attempt's lookups, measured from the actual elapsed time, so a
      slow default attempt's third failure still files the issue. An unconfirmed report keeps its id for the next attempt,
      as before.

Not resolved

Two hook runs for the same seat and occupant are still not serialized, as on main. If they overlap, a PostCompact
recorded during a delivery can be lost, and one refocus can be delivered twice. This is reproduced between hook processes
(the tests characterize it). Whether a harness overlaps these events for one session is not established, so the gap is
open, not shown to be safe. A per-occupant lock was tried in this PR and removed: a stale-lock takeover with path-based
file operations can't guarantee one holder.

Changed results

  • The 60 s registration: a refocus hook that hangs outside its lookups can hold a prompt for up to 60 s, where
    shipped main stopped it at 5 s.
  • A default fire whose lookups together pass 5 s now completes (about 6 s in the measured case) where shipped main
    stopped it with nothing delivered. Timing is path-specific: it depends on which lookups are slow and on the old-CLI
    retry.
  • With no budget set, the delivered payload equals main's code under the same registration. That was measured
    byte for byte in eight cases with a harness-kill runner:
    • lookups fast;
    • a 2.5 s trace;
    • partial output, then a timeout;
    • a failing trace;
    • a fast and a slow content REF;
    • a content-REF seat with a slow whoami;
    • and the 5 s-passing case above.
  • openrig-core is now 0.1.13 (both manifests), the next version after main's 0.1.12. Installed copies older than that advance at the next daemon start,
    which is how this hook change reaches them. Installs at an equal or newer
    version, external copies, and already-running harnesses don't change.
  • Codex: hooks/codex.json gets the same 60 s for parity. A Codex seat doesn't run refocus.cjs today, so nothing
    changes for Codex seats now.

How I verified it

  • packages/daemon/test/refocus-budget-concurrency.test.ts (12 tests). Each runs the real hook as a harness does,
    with JSON on stdin and a kill at the outer bound, against a stub rig and a stub trace with set delays.
    • The registrations: refocus.cjs 60 s on both harnesses, every other hook unchanged.
    • A selected budget:
      • a slow but successful trace is delivered;
      • a never-ending trace ends as a gap inside the budget;
      • a budget above the limit is capped at 52 s;
      • lookups that pass the budget end as a gap, and three attempts file one confirmed report.
    • The default:
      • the 2 s trace cap;
      • main's results, including the content-REF seat with a slow whoami;
      • three slow failed attempts that run past 4.5 s still file one confirmed issue report (red before this change's
        report fix).
    • Overlap, characterized (UNRESOLVED):
      • a PostCompact landing during a delivery can be lost;
      • two overlapping prompts can both deliver;
      • two occupants each still get theirs.
    • Leftover state: a delivery claim, pendingSeq or lock file left on disk by an earlier candidate doesn't stop a
      hook delivering what is due.
  • The existing refocus, core-plugin, plugin-load and vendoring suites pass.
  • Lint passes.
  • Revision: 01b74ee5 (main merged in, then the version bump). 823fb043 removed the lock and claim from
    7da32d1e and kept the budget and report changes.

🤖 Generated with Claude Code

…rness kill, and one delivery per occupant

Three problems in the refocus hook, each reproduced first by running the
real hook under the harness's kill with stub lookups:

- No supported way to give a slow trace more time. A larger inner
  timeout would still be killed at the registrations' 5 s.
  OPENRIG_REFOCUS_BUDGET_MS (4,500 to 55,000 ms) now selects the
  hook's budget. Each lookup's cap scales with it, and the refocus.cjs
  registrations allow 60 s on both harnesses. The other hooks stay at
  5 s.
- Lookups each under their own 2 s cap could together pass 5 s. The
  harness killed the hook, nothing was delivered, and no failure was
  recorded, so three in a row raised no issue. Every lookup, the first
  `rig queue whoami` included, is now cut to what's left of the budget.
  A lookup with no time left reports its gap, and the attempt delivers
  what it has and records the failure.
- Overlapping hook runs for one occupant lost a PostCompact that landed
  during a delivery, and two overlapping prompts delivered the same
  refocus twice. State reads and writes now take a per-occupant lock
  file, held for milliseconds, never across the trace. A prompt claims
  the delivery, and an overlapping prompt leaves it alone. At the end
  the state is re-read, and only the pending the delivery claimed is
  cleared. A claim left by a killed hook lapses after the budget plus
  2 s.

With no budget selected, the delivered payload is byte-identical to
before in every measured case that finished. The one changed default is
a fire that used to be killed: it now ends as a reported gap. A hook
that hangs outside its lookups can now hold a prompt for up to 60 s
instead of 5 s. openrig-core goes to 0.1.12 so installed copies pick
this up.

📝 Walkthrough

The refocus hook now uses configurable execution budgets, with runtime registrations allowing up to 60 seconds. Lookup and trace work use remaining-budget limits, and failure reporting receives an extended deadline. Per-occupant locks and delivery claims coordinate concurrent events. Documentation and integration tests cover these changes.

The plugin manifests advance to version 0.1.12. Claude and Codex refocus registrations use 60-second timeouts. The hook applies configurable budgets to lookups and trace execution, and health reporting accepts a deadline. Documentation and tests describe and check these limits.

Per-occupant state updates use an exclusive lock and delivery claim. Completion removes only its own claim and clears pending state only if no newer event arrived. Documentation and tests cover overlapping prompts, lock recovery, fallback behavior, and interrupted delivery.

Two narrow edge cases can briefly let overlapping refocus hooks run without exclusive state access. Both have bounded effects and recover on their own, so the change is mergeable with minor follow-up.

Retry lock acquisition instead of proceeding without ownership.

lock() returns after one second without setting lockHeld, but both callers continue to read and persist state. A concurrent hook can therefore overwrite a pending event or delivery claim without owning the lock.

An immediate exit is not a safe fix for the non-prompt branch. That branch persists PostCompact and other pending events only after the lock call. The hook registrations provide no repository-owned retry or durable handoff for an event skipped by that exit. Keep retrying lock acquisition, or add an equivalent durable fallback, before any state read-modify-write.

…t, and never write state without the lock

Two gaps in the previous commit:

- The issue report after a third failed attempt still got
  min(1.5 s, 4.5 s - uptime). An attempt that used its whole budget
  therefore spawned the report with a ~1 ms timeout, every time, so that
  episode was never reported. Hosted CI caught it. The report now gets
  its own 1.5 s after the hook's budget, which still ends well inside
  the 60 s outer bound.
- A hook that waited 1 s for an occupant's lock went on without it,
  reopening the race the lock closes. It now waits past the stale age.
  A live holder keeps the lock for milliseconds, and a killed one's lock
  is broken at 5 s, so the wait always ends holding the lock. Only a
  lock file that can't be created at all (an unwritable state folder)
  leaves the hook going on without it, rather than failing the turn.

The expiry test's stub now answers an issue report after 200 ms, so a
near-zero report timeout fails it on every run instead of sometimes.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

Keep the 60-second registration contract and reserve cleanup time.

At the 55-second budget, process.stdout.write emits the refocus before its callback runs. The callback can then spend 1.5 seconds in recordRefocusResult and up to 6 seconds in lock(). The 60-second registration can terminate the hook before state.delivering and pending state are persisted, so a later prompt can deliver the refocus again.

Do not raise the registration timeout to 65 seconds. Keep the documented 60-second contract and reduce the maximum work budget to leave time for reporting and state completion.

…orded inside the 60 s bound

After the budget, a delivery can still spend up to 1.5 s on the failure
report and up to 6 s waiting for the occupant's lock to record its
outcome. At a 55 s budget that could pass the registrations' 60 s, and a
hook killed there would leave its claim and pending state behind, so the
refocus could be delivered again. The longest budget is now derived:
60 s - 1.5 s - 6 s - 0.5 s margin = 52 s. The registrations stay at
60 s. The default (4.5 s) is unchanged.

The trace gap is what tells the 52 s limit from 55 s. The elapsed-time
bound only checks that the hook ended, so it now allows 30 s instead of
leaving under 400 ms for start-up and post-trace handling.

…s no owner; main's default results

Round 2 of the review:

- The state lock moves to refocus-lock.cjs and is owner-safe:
  - A lock holds its owner's token and is released only by that owner.
  - A stale lock (its owner killed) is taken over by moving it aside and
    checking it is the one seen stale. A competitor's fresh lock moved
    by mistake is put back, so two hooks never both hold it.
  - Every path ends by its deadline. Something at the lock path that
    can't be read or moved (a directory, a dangling link, an unwritable
    folder) fails at once instead of spinning to the 60 s kill.
  - A hook that can't take the lock goes on as it did before there was
    a lock, and says so on stderr.
- A delivery claim now lasts as long as its owner can still be running
  (budget + failure report + closing lock + 0.5 s), so no second prompt
  delivers while the first is alive. After a killed hook, re-delivery
  waits that long (12.5 s by default).
- With no budget selected, every lookup keeps its fixed 2 s cap,
  unclipped, so a default fire gives main's result, content-REF seats
  included. A selected budget still scales and clips the lookups.

…, not taken for a dangling link

After a read found the lock gone, any lstat hit was reported as a
dangling link. If the holder released and another hook took a fresh
lock in between, the waiter gave up and went on without the lock while
it was held. Now only a symbolic link at the path is a dangling link;
anything else is waited for, within the deadline.

The default-parity test's lookups now take 1.7 s rather than 1.9 s each.
Together they still pass the old 5 s bound, but each leaves room under
its 2 s cap for a slow runner's process start-up. Hosted CI's macOS
daemon run timed out the content REF at 1.9 s.

…p the budget and the report window

The lock could not be made exclusive with path-based file operations: a
stale takeover acts on a path, not on the lock it inspected. So this
change no longer claims to serialize overlapping hook runs. Removed:
refocus-lock.cjs, the delivery claim, pendingSeq, and their tests. The
hook is main's again, plus:
- OPENRIG_REFOCUS_BUDGET_MS, from 4,500 to 52,000 ms (pinned, not
  derived), scaling and clipping the lookups only when selected;
- default lookups keeping main's fixed 2 s caps and old-CLI retry;
- the failure report getting its own 1.5 s after the attempt's lookups,
  measured from the actual elapsed time, so a slow default attempt's
  third failure is still reported;
- the 60 s refocus.cjs registrations (every other hook unchanged).

Overlapping hook runs for one occupant keep main's behaviour: a
PostCompact can be lost, or a refocus delivered twice. That gap stays
open. The tests now characterize it instead of asserting a fix, and
check that a claim or pendingSeq left on disk by the earlier candidate
doesn't stop a hook delivering what is due.

@mvschwarz mvschwarz changed the title fix(refocus): a selectable time budget, honest gaps instead of the harness kill, and one delivery per occupant fix(refocus): a selectable time budget, and a failure report that survives slow attempts Oct 18, 2026

Approved at 01b74ee. A seat can give a slow refocus trace more time with OPENRIG_REFOCUS_BUDGET_MS (default 4,500 ms, the current behaviour; up to 52,000 ms), and a slow attempt's failure report now gets its own window. Both change-only reviews and the required CI (8/8) pass at this head, and the merge with current main is clean.
