Ranked only while live; optional per-stream pair limit

What changes for chat

• Live duels are always ranked, with no pair limit by default (the owner asked for unlimited). Mods can set pairRankedPerStream (0 to 50, 0 = no limit) in the balance editor; with a limit the pair gets that many ranked duels per live stream. Before this it was a fixed 5 per rolling 24 h.
• Past the limit a duel is unrated and pays half the dollars (floor: $2 win, $1 loss).
• Offline duels are unrated and pay nothing. If the live check fails, the last answer stands; with no answer yet, duels stay ranked.
• The last ranked duel's result line ends with "Last ranked duel between them this stream."
• !stats @name adds the ranked duels left between the sender and @name.
• Mods reset one pair with !resetpair @a @b in chat or "Reset pair count" on the mod page.
• Pair plays saved before this change count toward the current stream when they fall inside it.

Tests

bun run check: 399 pass. New cases in tests/game.test.ts (per-stream cap, half pay, offline, legacy plays, 0 = no limit, resetPair) and tests/channel.test.ts (!stats @name, !resetpair, admin resetPair).
