Creation of new users

Describe the bug
User is unable to create new user

To Reproduce
Steps to reproduce the behavior:

1. Start at login screen
2. Click on the "first responder" or "civilian" button under request access
3. Fill out form and submit
4. See error

Expected behavior
Request of new user created.
