Markdown heading sanitization could expose another HTML tag after a single replacement, leaving incorrect anchors and a CodeQL finding. Strip innermost tags until the text stops changing, matching the frontend and CLI checkers.
Add regressions for nested, reintroduced and incomplete tags and duplicate heading anchors. Schema definitions and generated model types are unchanged.
Validation on 3c71ddd: Node 24 documentation tests (6 passed) and documentation check (4 Markdown files, zero problems).
Dependency order: merge this PR before CLI PR #49, which already pins this schema commit; frontend PR #65 consumes that combined CLI revision. If squash merging changes the retained commit, update downstream submodule pins accordingly.
