In the ai_guardrails filter's nemo-outbound chain, traffic from Praxis to NeMo is unauthenticated. However, the TrustyAI NeMoGuardrails server deployment adds a kube-rbac-proxy side-car to the NeMoGuardrails server pod which requires users to authenticate to NeMo using their OpenShift bearer token. Lack of authentication in Praxis callouts to NeMo cause the requests to be rejected.
This PR partially resolvess this issue by adding a new filter, service_account_token that reads a mounted serviceAccountToken file on every callout and injects it as a bearer Authorization header. It depends on updates to the extproc pod to add a ServiceAccount token and a RBAC for said ServiceAccount matching what the what the NeMo kube-rbac-proxy sidecar checks.
NeMo Guardrails now supports optional token-file authentication and provider-specific TLS trust. Response screening applies only to successful responses to requests with bodies. The pull request adds a service-account example and updates tests and documentation for these behaviors.

Exports credential_authority and adds a shared Bearer-header transform. Metering uses the shared transform.
NeMo token and TLS configuration filters/src/guardrails/providers/nemo.rs, filters/src/guardrails/config.rs
Adds optional token-file and CA-bundle settings. NeMo callouts stage credentials for the configured endpoint authority and apply provider-specific TLS trust.
Screens only 2xx responses to requests with bodies. Bodyless requests and non-2xx responses are forwarded without screening. Tests cover these response paths.

Tests service-account credential forwarding, token-file rotation, response forwarding, and configuration construction. Updates an existing authorization-header assertion.
praxis-proxy/ai#1286: Changes response-phase verdict handling and redaction, which depend on the response-screening selection contract.
praxis-proxy/ai#1773: Changes the ai_guardrails configuration contract used by the updated examples.
praxis-proxy/ai#1839: Changes handling of GET /v1/models, which interacts with the response-screening behavior tested here.

Merge Risk: Moderate · previous review c7824
The service-account example sends NeMo a token intended for the Kubernetes API; its practical reuse depends on the account’s permissions. Give the token a NeMo-specific audience and correct the failure-mode guidance before merging.
Security Architecture Review
Security architecture risk:🟡 Moderate · up to de4df
The configured callout path replaces caller credentials with a service-account token and rejects requests after detected token-read failures. However, refresh-monitor failures can leave cached credentials in use without a freshness bound. Production transport protection and the service account’s effective permissions remain unverified.
Retained concerns
Medium · reliability · inferred: Credential invalidation does not cover post-startup watcher failure. The callback discards watcher errors, while requests continue using the last available snapshot. If subsequent rotation observation is interrupted, all callouts sharing that filter can continue sending stale credentials without a local freshness bound or recovery mechanism, potentially stranding the authenticated guardrails path. Ordinary read failures do invalidate the snapshot; downstream acceptance of expired or revoked credentials is not established.
Security review details
Security Blast Radius
inferred — Credential-refresh failure affects requests sharing the configured filter instance, rather than only the initiating client. The configured callout authenticates as the mounted service account, not as the caller. Its maximum tenant, service, or data-store authority cannot be bounded from the token-mount example because effective RBAC and audience enforcement are not supplied.
Security Findings and Attack Paths
inferred — The supported failure path is loss of refresh observation followed by continued use of the cached token. The inspected implementation provides neither watcher-error invalidation nor a credential-age bound. This establishes a freshness and authenticated-path resilience concern, not a demonstrated request-only attack or downstream acceptance of expired credentials.
Trust Boundaries and Controls
observed — The inspected NeMo path separates client identity from outbound pod identity through a configured endpoint, prepared-target binding, and credential replacement. Backend assertions confirm that the service-account header reaches the mock destination and the client credential does not. Binding is supplied by the provider and chain configuration; the token filter itself has no destination or audience field.
Resilience and Maintainability Implications
observed — Ordinary invalid-token and deletion transitions clear cached authority, and later successfully processed reloads can restore it. Unit tests cover startup reread, eventual rotation, deletion rejection, and invalid-header invalidation. These controls do not exercise or repair the separate watcher-error transition.
Hardening Proposals
proposed — Make watcher health part of credential availability, with explicit invalidation and recovery or an independently bounded refresh mechanism. Before production rollout, verify server identity and redirect credential handling, and document the narrowest supported token audience and RBAC permissions.

25-31: Set the audience of the projected token in the example.
The Pod-spec snippet does not set audience. Without it, the token uses the API server's default audience. A token with that audience can be replayed against the Kubernetes API itself. Set an audience that matches the --auth-token-audiences value of the kube-rbac-proxy. A previous review raised the same audience mismatch.
The watcher thread repeats the read, assemble, and validate sequence from reload_token and from_config. Add one helper that returns Result<Zeroizing<String>, FilterError>. Use it in all three places so the validation rules stay the same.
⚠️Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Restrict the event-path match to the token file and its atomic-swap entries.
p.parent() == path.parent() matches every path in the watched directory. The watch is NonRecursive on that directory, so this condition is true for every event that arrives. A change to any sibling file then triggers a token reload, and the path == p check does nothing. The reload is harmless, but the filter does not actually decide whether the event affects the token file. Kubernetes projected volumes rotate the token through ..data and ..<timestamp> entries. Match those entries and the token path, or remove the dead filter so the code states its real behavior.

Document request and response failure modes separately.
The statement that failure_mode: closed rejects the chat and failure_mode: open forwards it unscreened applies to request-phase errors only. In the response phase, on_response_body catches provider errors, replaces the response body with an evaluation_failed document, and returns Continue under both modes. Adversarial Review (v2) — fix(guardrails): destination-bound service account token
This supersedes my earlier review (5621032284). The implementation has been substantially reworked since then.
The refactoring is a significant improvement over the version I first reviewed. Key changes:
My H1 (token file re-read on every request) — FIXED. Now uses credential_source::build_one() with inotify-based file watching and in-memory SecretHandle.current() cache. Token reads are now O(1) atomic pointer swaps, not filesystem I/O.
My M1 (http:// example) — FIXED.require_token_transport() rejects plaintext non-loopback endpoints at startup. The host_is_loopback function correctly handles IPv4 (127.0.0.0/8), IPv6 (::1), IPv4-mapped IPv6 (::ffff:127.0.0.1), and localhost.
The DeferredCredential approach is an excellent security design. The token is staged into request extensions and only injected by the executor after dial resolution confirms the destination matches the configured authority. This means: (a) chain filters cannot see or strip the token, (b) a DNS rebind or redirect to a different host does not leak the token, (c) the token is never on the wire unless the actual TCP destination matches the configured endpoint. This is stronger than the caching-with-TTL approach I suggested.
Remaining findings
M1. debug! log level on token unavailability may be too quiet for production.
When snapshot.payload.as_deref() returns None (file configured but no usable token), the error is logged at debug! and the call is denied. Fail-closed is correct, but an operator debugging "why are guardrails suddenly broken" needs debug-level logging to see this. Consider warn! — this is an actionable operational condition, not a diagnostic detail.
L1. The _watch: Option<WatchHandle> field keeps the file watcher alive for the NemoProvider's lifetime. On config reload, the old provider (and its watcher) is dropped, and the new one starts a fresh watch. This is correct lifecycle management. 👍
L2. CodeQL flagged cleartext logging of a secret in the test file. Worth addressing even in test code to avoid CI noise.
Overall Assessment
The destination-binding design is well above typical service-account-token implementations. The security model is clean: startup-time transport validation, runtime authority binding, fail-closed on unavailability. The refactoring to use the shared credential_source infrastructure eliminates the file-I/O concerns from my first review.
Note: the author mentioned they are still testing on OpenShift and the PR is not yet RFR.
Verdict: Address the debug! → warn! on token unavailability (M1). Otherwise recommend merge when ready.
Replace the ServiceAccountToken watcher with the shared
credential_source module, and add credential_source::bearer_header so
NeMo and external metering share one Bearer transform. Invalid tokens
are now rejected at startup.

Let’s move forward with this PR using the shared helpers. The credential source, rotation logic, and credential authority helper are generic enough to belong in Core. We can move those in a separate refactor and keep the NeMo-specific configuration and guardrail behavior in AI.
