Remove the newly added visible Details controls from account and key cards. Clicking a card still opens usage details, and focused cards support Enter/Space with a visible focus outline.
Only show Reauthorize on Codex/Claude accounts when the credential is expired/unknown or the last refresh status is not success. A disabled account with otherwise healthy credentials does not require reauthorization simply because it is disabled.
