Closes #15.

Password change and "sign out everywhere" both invalidate every token including this device's, and both hand back a replacement. Adopting it is what keeps you signed in to the account you just secured — there is a test pinning the old token being dead and the new one working.

The current-password field appears only once the email address is actually edited, because that is the single case the server demands it.

The email toggle is disabled and explained where the instance has no SMTP configured. Leaving it live would mean a switch that silently does nothing.

Switching instance clears the stored URL and the session, and that is the whole of it — the base URL is resolved per request, so nothing is rebuilt or restarted.

The current-password field appears only once the email address is actually
edited, because that is the single case the server demands it; asking for it
unconditionally would imply the whole form needs it.

Username validation mirrors the backend's pattern so the form answers rather
than a round trip.
