シェルの外部コマンド化（/bin）とフェーズ2（ハンドル・IPC・proc_spawn） #69

シェルをディスパッチャにし（組み込みは cd/set/exit/help）、clear/echo/free/ps/pwd/uname を /bin/*.wax（WAL fnd 1.0 を import する WAX v3）に移した。カーネルにハンドル、IPC、svc_register/lookup、proc_spawn（libs）、wait、mem_grow。console サービス、暫定の読み取り専用 fs（bootfs）、init。
