Diagnosis and review now identify the smallest permitted evidence needed when customer access is limited. This keeps useful work moving without requesting broad access or overstating what a partial trace or diff establishes.
Adds three fictional evaluation cases covering uncertain timeout outcomes, partial-diff review and a page-specific source denial. Existing reference routing and source fallback behavior remain unchanged. Regenerates standalone packages and updates release metadata to 5.1.21.
Validation: structural checks, routing/CLI smoke and fixture preparation tests passed. The local full suite passed 480/481; the remaining localhost-listener test was sandbox-blocked and passed on an authorized rerun. Three single-run agent diagnostics and independent method review found no actionable issues; these are not comparative performance claims. Required CI must pass before merge.
