Fix existing Markdown lint failures #67
Description
Question
Make the repository Markdown lint gate pass. The current failure is in the existing docs/plans/2026-08-30-always-prefix-appset-apps-design.md document, which reports 18 heading, list-spacing, fence-spacing, and blank-line violations. Fix only the lint issues and rerun the gate.

Resolved in release PR #65. Fixed the existing Markdown structure violations in the design plan. Markdown lint now passes with 0 issues.
