[Feature request]: submission checkboxes

Description

Add some mandatory things to submissions with simple selectable options.
Like

• notifications with the options works/partially/unrelated/broken
• Signing in: works/unrelated/broken
• in app purchases: works/unrelated/broken

We can probably come up with more

Additional context

There are quite a lot of ratings with no comments given which is pretty annoying. Knowing what is broken can be massively helpful. Alternative you could require a comment if the submission isn't gold
