Confirmed: per-lex, and named with the citation itself. Book 41 alone carries 613 named anchors.

One page per book, zero-padded: d-01.htm … d-50.htm. The compiler already derives {book02} from a numeric capture group, so no url_by map is needed.
Decide the principium form before minting any reference. The anchors spell it 41.2.3.pr. — a dot before and after pr. #25 proposes the locator D.41.2.3pr. These do not agree. Either adopt .pr. as the canonical locator form, or keep pr and carry a vars: mapping on the resolver. Changing this later re-mints every affected reference UUID (ADR-0002), so settle it in review.
Note also that the paragraph is optional: 41.2.3 and 41.2.3.pr. and 41.2.3.1 are all real anchors, so the locator regex must admit a bare lex, a pr form, and a numbered paragraph.

Anchors are the citation. Corpus/d-41.htm (270 KB) contains 613 <a name="…"> anchors, spelled exactly as the citation: 41.1.1, 41.1.1.pr., 41.1.1.1, 41.2.3, 41.2.3.pr., 41.2.3.1 … 41.2.3.19, and so on.
#41.2.3 resolves to:

Correct — this is #25's own example locator.

A bad anchor is a soft failure — the reader lands at the top of the book rather than on a wrong lex. That is acceptable; it is not the silent-wrong-passage failure of #22.
Edition confirmed on the page itself: "Digesta Iustiniani : Liber 41 ( Mommsen & Krueger )" and "Based upon the Latin text of Mommsen's edition". This matches what #25 expected.
