Bumping a preset with a versioned @fetch dest stacked the old path next to the new one in instructions and skills.paths, so opencode loaded every planify version's rules and skill (0.2.1 through 0.4.0 seen in a real config).

Append now treats a path under the cache dir that differs only in its X.Y.Z token as the same entry and supersedes it in place, collapsing already-stacked copies on the next install. Paths outside the cache dir (prompted dirs, hand-added entries) stay plain append. remove is unchanged.

Not covered: entries from before the planify-* → opencode-planify-german-* rename have a different family name and must be removed by hand.

Every preset bump writes a new versioned @fetch dest, and append kept the
old one: instructions and skills.paths stacked opencode-planify-german
0.2.1 through 0.4.0, so opencode loaded every version's rules and skill.

An append entry under the cache dir now shares an identity with any entry
that differs only in its X.Y.Z token, and supersedes it in place like a
name@spec plugin does. Paths outside the cache dir keep plain append.

Master carries one fix since v0.29.0: append now supersedes versioned
cache paths, so a preset bump no longer stacks the old @fetch dest in
instructions and skills.paths (#136). A fix, so this is a patch.
