Check layered environment/.env overrides before automatically selecting MaaS from an available API key. Preserve explicit constructor-mode priority and the existing automatic MaaS behavior when no environment mode is present.
Validation: 138 tests pass and 22 optional tests skip across the new precedence tests and existing unit suite. Both environment and explicit .env self-hosted cases fail on upstream; the constructor-override control already passes. All pre-commit checks pass. Pipeline creation is mocked; no cloud OCR requests, model weights or local inference are used.
