docs(review): CodeRabbit threads can be answered in the thread - #2609
Follow-up to #2605. The seat gh guard now allows one write: an in-thread reply to a review comment. This documents the exact form and how it fails. A seat added --jq today and was refused, which looks like a missing permission.
Docs only, Tier 0: every CodeRabbit thread answered, plus green CI.
The seat gh guard now allows one gh api write: an in-thread reply to a PR
review comment. Document the exact six-argument form, note that an extra
flag makes the guard refuse it (one seat hit this with --jq), and keep the
single PR comment quoting discussion_r links as the fallback.

The external review instructions allow one restricted gh api reply to a review comment. They retain a PR comment as a fallback and as the format for answering multiple threads. Both methods count equally.
The instructions permit one gh api review-comment reply with specified equivalent call spellings. They refuse added headers, extra fields, inline bodies, or another path. PR comments remain the fallback and the multi-thread response method. Both methods count equally.

Document the required root comment ID.
Line 32 must state that <id> is the top-level CodeRabbit review-comment ID, not a reply ID. GitHub does not support replies to replies, so using a reply ID makes this command fail. (docs.github.com)
As per path instructions, this documentation must state the API precondition accurately.

Update the `gh api` example in the external-review instructions to clarify that
`<id>` must be the top-level CodeRabbit review-comment ID, not a reply ID, so
the documented API precondition is accurate.

Correct the documented gh api alternatives.
-f sends @<file> as literal text, so it does not preserve -F body=@<file>. Keep -F in the canonical command and remove -f from the list of equivalent spellings. A bare --jq is also not a complete invocation, because the flag requires a jq expression.
