Review tier: Tier 1, docs only, one reviewer: Rhea, as requested by connector-ops in message 83391. D18 remains Proposed pending review and ratification.
An agent should retain its Commonly identity when it acts through a shared external account. ADR-025 D18 proposes that every outbound connector action carry the agent and on-behalf-of human pair while each connection selects one external principal. It specifies Slack name/avatar customization, Discord webhook overrides, Telegram prefixes, and GitHub body credit plus commit trailers. Provider limitations fall back to explicit text attribution.
The scope notes distinguish D15's inbound attribution and ADR-022's persona/runtime separation. RoomGrant targets and authorization stay unchanged. The first rollout is the separate internal writer and reviewer GitHub Apps from D52. Platform capabilities are linked to official documentation; this PR implements no adapter or credential change.
Validation: node scripts/verify-adr-numbering.js --dir docs/adr passes (30 distinct ADRs); added relative links and anchors resolve; git diff --check passes. No runtime tests are needed for this documentation change.
Record the agent and on-behalf-of human pair, one external principal per connection, platform rendering limits, and D52 writer/reviewer App rollout. Preserve D15 inbound attribution, ADR-022 personas, and RoomGrant scope.

Use precise D15 attribution wording.
D15 defines the human identity attributed to supported inbound posts. It does not identify the actual sender of every inbound message. Group messages still lack per-sender Commonly identity mapping. Rewrite this sentence to avoid implying sender-level attribution.
As per path instructions, docs/** claims must match the documented repository behavior.

Clarify whether Co-authored-by must provide GitHub contribution credit.
A syntactically valid Co-authored-by: name <email> trailer preserves textual attribution, but GitHub contribution credit requires an email associated with the co-author’s GitHub account. D18 does not define that mapping. Its statement that the Apps do not require a GitHub account per agent makes the intended behavior ambiguous.
If GitHub contribution credit is required, define the verified agent-to-GitHub email mapping and the fallback when no mapping exists. Otherwise, state that the trailer is Commonly metadata only and use Agent-Model as the canonical agent identity.

Rhea's D15-link finding: ADR-022 now points directly to #channel-routing--user-scoped-connectors-d8d16, the section containing D15. The heading and link were checked together.
CodeRabbit review 5638147581, other comment at lines 547–549: fixed. D15 is described as attribution for supported inbound posts; the lack of group-message per-sender mapping stays explicit.
Same review, other comment at line 584: fixed. The proposal requires textual Co-authored-by attribution and promises no GitHub contribution credit or per-agent account. A future account/email mapping must be verified before claiming credit. I did not adopt the suggestion to make Agent-Model the canonical agent identity: the authenticated (agentName, instanceId) is the identity, while Agent-Model records model provenance.
git diff --check, ADR numbering (30 distinct numbers), and the D15 destination/anchor check pass. D18 remains Proposed.

Tier 1 docs re-review at 5c1c63f6: PASS.
ADR-022 #decisions is fixed: the link now targets #channel-routing--user-scoped-connectors-d8d16, which resolves to the D8–D16 section that holds D15.
The D18 anchor #relayed-outbound-identity-d18-proposed resolves.
D18 is still Proposed. The new Co-authored-by and Agent-Model paragraph makes no GitHub credit promise, and git diff --check is clean on both ADR files.
The new scope note in ADR-025 says group messages still lack per-sender mapping. D15 does not say that in those words, but its owner-stamped authorship implies it. Not blocking.

Define the human’s GitHub commit attribution.
The clarification correctly prevents treating the delegated human as a GitHub co-author. It does not define how that human appears in a GitHub commit message. The GitHub row specifies only the agent’s Co-authored-by and Agent-Model trailers, while D18 requires human identification on message-bearing actions.
Define a fixed, non-coauthor text format for the human, such as On-behalf-of: <human>, and include it in the rollout demonstration. Do not imply GitHub contribution credit.

ADR-025 D18, proposed in #2620, makes every outbound action carry the
agent and the human it acts for. Say where that lands in this contract,
so either can be built first.
