fix(cli): read context tokens under the physical cwd; bound a stalled turn at window plus grace - #2991
Context tokens under the physical cwd. Claude names its transcript folder after process.cwd(), which macOS returns as the physical path (/tmp is /private/tmp). #2986 resolved the cwd for the watchdog's transcript path, but claudeContextTokens still read under the unresolved cwd. A seat whose cwd is reached through a symlink therefore reported unknown context every turn and never recycled its session. Both call sites now pass physicalPath(ctx.cwd).
A stalled turn is bounded at window plus grace. At the grace deadline the adapter now also destroys its ends of the child's stdout and stderr. Before this, a pipe holder the descendant snapshot missed could keep close from firing indefinitely, either because ps failed at the stall or because the holder was reparented before it.
Tests:
adapter, symlinked cwd: the transcript is written under the resolved path and its tokens are read.
adapter, stall with an empty process table and a child whose close fires only once both pipes are destroyed: the turn rejects spawn_stalled.
The Claude adapter now uses physical workspace paths for transcript context lookups. It also destroys stdout and stderr after the stall-kill grace period. Tests cover symlinked working directories and stalled spawns whose pipes prevent closure.
Normal-turn and session-recovery transcript lookups use the physical workspace path. A test checks that a symlinked working directory returns 507 context tokens.

Summary
Destroy pipes after stall grace period cli/src/lib/adapters/claude.js, cli/__tests__/first-output-watchdog.test.mjs
After the stall-kill grace period, the adapter destroys stdout and stderr. A test verifies both streams are destroyed and the spawn rejects with spawn_stalled.
Team-Commonly/commonly#2576: Introduced Claude transcript context-token reads, which these changes update to use physical paths.
Team-Commonly/commonly#2986: Added the Claude first-output watchdog and survivor cleanup, which these changes extend with output-stream destruction and physical-path context lookups.
Team-Commonly/commonly#2784: Changed Claude transcript placement under Linux bwrap; these changes use the physical working-directory path for transcript lookup.
Merge Risk: Low · up to 68677
The implementation is mergeable, but adding the focused retry regression test would protect symlinked-workspace context accounting.

Assert that the grace period elapses first.
Moving both destroy calls into onStall would still pass this test. That would close the pipes before the child receives its grace period. Assert that neither pipe is destroyed before the deadline, then assert that both are destroyed at the deadline.
As per path instructions, “a test that would still pass with the fix reverted is not a test.”

Bound the failure path and clean up the child.
If either destroy call is removed, this promise never settles, so finally does not restore the environment. The adapter’s default 15-minute timer also remains active; an in-band test run can remain open until it expires. (raw.githubusercontent.com)
Use a bounded wait and emit the fake child’s close in cleanup so the adapter clears its timer even when the test fails.

Exercise the exited-child path.
This fake has no pid, so the adapter never calls _processTable for the initial snapshot. It also emits close without exit, unlike a real spawned child. (nodejs.org)
Give the fake a PID and emit exit on SIGTERM while keeping the pipes open. Assert that the empty process table was read and that pipe destruction then releases close.
As per path instructions, tests must exercise “the real input shape and the relevant branch.”

Add recovery-path coverage for the symlinked transcript lookup.
The symlink test exercises only the initial spawn. The separate recovery test does not use a symlinked cwd or assert contextTokens. Add a recovery test that reads tokens from the physical path and fails when the lookup at claude/src/lib/adapters/claude.js:1332 is reverted.

Extend the recovery-path test to use a symlinked cwd and assert the recovered
contextTokens value comes from the transcript at the physical path. Cover the
freshId spawn flow that passes physicalPath(ctx.cwd), so the test fails if the
lookup uses the symlink path instead.

Add a regression test for the resume-retry path with a symlinked cwd.
The retry branch now reads its context count from physicalPath(ctx.cwd), but no test reaches that branch. The new symlink test covers only the first spawn. Reverting the retry call alone would leave the suite passing, and the retry turn would return contextTokens: null for a symlinked cwd.
Add a test in cli/__tests__/adapters.claude.context-tokens.test.mjs that:
Creates a real directory and a symlink to it, then uses the symlink as cwd.
Passes a sessionId that the adapter resumes. The first spawn, which includes --resume, exits non-zero with stderr "already in use".
Has the retry spawn (--session-id) write its transcript under the project folder named after the resolved real path.
Asserts that newSessionId is the fresh UUID and that contextTokens is 507.

Grace-deadline pipe drop. It runs only inside the stall timer, after the tree SIGKILL and the survivor sweep, so healthy turns never lose output. A stalled turn has produced nothing to lose by definition. Node's close waits on stdout, stderr and exit, not stdin, so dropping our two read ends bounds close once the SIGKILLed root has exited. That is every stalled turn, at window plus grace. The new case binds it: an empty process table, a child that closes only after both destroy calls, spawn_stalled expected. Deleting either line leaves close unfired and the case times out.
physicalPath(ctx.cwd) at all four claudeContextTokens sites matches where claude names its project folder (process.cwd(), resolved). It falls back to the given path when realpath throws. The symlinked-cwd case expects 507 and fails without the change.
Not blocking, a note for the Linux arm: bwrap's --chdir takes opts.workspacePath. If that is ever the unresolved symlink while the jail binds the target elsewhere, the in-jail process.cwd() could differ from the host realpath. That's worth a Linux case when someone next touches the bwrap path; it's out of scope here. CI: 21 pass, 4 skipped.

Symlinked cwd. claudeContextTokens derives the project folder by replacing every non-alphanumeric character in the cwd. Both call sites now pass physicalPath(ctx.cwd). physicalPath is realpathSync, and it falls back to the input only when the path can't be resolved, in which case no transcript exists anyway. Claude Code names the folder after process.cwd(), which is the resolved path, so the two now agree. Mutant: restoring ctx.cwd at both sites fails "a symlinked cwd is read under its physical path, where claude writes it". Killed.
Grace-deadline pipe drop. The drop runs inside the grace timer, after killProcessTree(SIGKILL) and the sweep. It only runs on a turn already declared stalled. onStall fires once, and close then rejects with stalled whatever the output contains. So the drop cannot change a result that is still being produced: the stalled rejection already took precedence over every byte of stdout. Node's destroy() without an error argument emits close on the stream, not error on the child, so the child's error handler doesn't fire. The child's close fires once, and reject after settlement is a no-op, so the promise settles exactly once. Mutant: removing both destroy lines fails "at the grace deadline the pipes are dropped…" with a timeout. Killed.
Non-blocking:
The "bounded at window plus grace" claim depends on the root exiting after SIGKILL. A root in uninterruptible sleep (D state) still never emits exit, so close still never fires. That is unchanged by this PR and rare. Worth a word in the comment, not a blocker.
The grace test mocks destroy with a counter instead of a real stream, so it proves the calls happen and that close follows them, not that Node emits close on destroy. Acceptable for a unit test.
