fix(cli): name a hung claude turn stuck, not started on retries (TASK-710) - #3108
A claude turn that only retries is now named stuck and killed inside the first-output window, instead of holding its delivery until the 45-minute run timeout.
Why
The watchdog counted any stdout or stderr byte, or any transcript growth, as life. A provider that only retries writes system events (init, api_retry) plus spawn-time warnings on stderr, so a hung turn looked alive for the whole run.
Change
runClaude now uses stream-json (--output-format stream-json --verbose --include-partial-messages).
In stream mode (default for claude), only non-system stream-json lines count as life. Stderr and transcript growth do not.
A stall reads stuck: no model output after the first transcript row, or console locked when the console is locked.
The reply is the result event's text (claudeResultText); output with no result event is returned as it arrived.
Verification
Unit: first-output-watchdog.test.mjs and adapters.claude.test.mjs pass (46/46). New tests cover system-only streams, stderr at spawn, split chunks, locked console, missing transcript, and claudeResultText.
Live walk, claude 2.1.314, stub provider that only retries, window 8 s: 3 of 3 runs rejected spawn_stalled at about 8.4 s, and the process tree was gone.
Positive control, stub provider that streams a model event at once then holds 12 s, window 8 s: not killed, resolves the answer at 13.1 s.
Mutant with the stream-mode default flipped to false: no stall named within 25 s.
Full cli suite: 2,160 pass. Three failures (adapters.claude.environment bwrap case, mention-event-types.contract two cases) also fail on HEAD without this change.

Run claude with stream-json. In stream mode the watchdog counts only model
events (non-system stream-json lines) as life. Stderr and transcript growth
no longer count there. A spawn with no model event inside the window is
killed as "stuck: no model output after the first transcript row". The
reply is the result event's text (claudeResultText).
Live walk (claude 2.1.314, stub provider that only retries, window 8s):
3 of 3 named stuck and killed at ~8.4s. Positive control (12s turn after
its first model event, window 8s) is not killed and returns its text.
Mutant with the signal dropped: the stall is never named.

Claude invocations now request stream-json output. The adapter extracts response text from result events or assistant text and filters error output. The first-output watchdog counts parsed, non-system stdout events as progress in stream mode.

Claude stream invocation and result parsing cli/src/lib/adapters/claude.js, cli/__tests__/adapters.claude.test.mjs, cli/__tests__/first-output-watchdog.test.mjs
Initial and recovery invocations request stream-json output. The adapter passes stream mode to the watchdog, extracts result-event or assistant text, and filters failure output. Tests cover result errors on initial and recovery turns.
Claude turns that end in an error result are now treated as failures, and retry-only streams are stopped early. Two edge cases remain. An error result with no text may lose its diagnostic and get the wrong retry policy. A stream that ends before a complete message may lose partial text. Both are narrow and can be handled as follow-ups or fixed before merge.

… mode (TASK-710)
Review of 93f4e2b found four real defects in the stream-json change:
- A non-zero exit put multi-line NDJSON into the 2000-char failure detail,
so the head cut dropped the terminal error. claudeFailureTail keeps the
result text, or the lines that are not system or stream events.
- claudeResultText returned raw NDJSON for a turn with no result event. It
now returns the assistant text, and raw text only when there is no JSON.
- The watchdog did not recheck `started` after the lock probe, so output
arriving during the probe could still be killed as stuck.
- A missing transcript path still suppressed the stall in stream mode, though
the stream signal never reads the transcript. The guard now applies only to
plain-text spawns.
Live walk after the fix: 3 of 3 stuck at ~8.4s; positive control not killed.

Verdict: PASS for the TASK-710 done-when. Scope is cli/ only.
Evidence, at this head:
Live stall walk (claude 2.1.314, stub provider that only retries, window 8 s): 3 of 3 runs rejected spawn_stalled, named "stuck: no model output inside the window", killed at about 8.4 s, no child left.
Positive control (stub streams a model event at once, then holds 12 s, window 8 s): not killed, resolves "the answer" at 13.1 s.
Mutant with the stream-mode default flipped to false: no stall named within 25 s.
Unit: watchdog and claude adapter suites 50/50 with the new tests. Full cli suite: 2164 pass, and the 3 failures (adapters.claude.environment bwrap case, mention-event-types contract x2) also fail on HEAD without this change.
Review round (independent, sonnet) found 6 items. Fixed in 034df3d: failure detail on non-zero exit now keeps the terminal error (claudeFailureTail); no-result turns return assistant text, never raw NDJSON; started is rechecked after the lock probe; missing transcript no longer suppresses the stream-mode stall.
Residual, not fixed here:
If claude falls back to non-streaming, or a pre-model phase (hooks, MCP connect, a very large resumed context) runs past the window, no model line appears and the turn is killed. The 180 s default is the only margin.
No end-to-end runClaude test with stream-json stall; the live walk is the evidence for that path.
The existing assertion would fail if recovery returned the literal recovered\n, but it does not test result-event selection. An implementation that returns trimmed stdout instead of selecting a distinct result event would pass.
Emit assistant and result events with different text in the recovery stub, then assert that res.text equals the result event’s text.

…ly (TASK-710)
CodeRabbit on #3108: claudeResultText returned the text of a result event
even when is_error was set, so a failed turn that exits 0 posted its error
text as the seat's reply.
The result path now throws adapterFailure when is_error is true, outside the
parse guard. Both the first turn and the session-recovery retry pass their
spawn credentials, so the text is scrubbed like the exit-path failure tail.
Tests cover the first turn, the recovery turn, and the unit function.

Verdict: PASS. The change since 034df3d is the is_error fix only.
claudeResultText now raises adapterFailure when a result event has is_error: true, outside the parse guard. Both the first turn and the session-recovery retry pass their spawn credentials, so the text is scrubbed like the exit-path tail.
Tests: first turn (exit 0, is_error) rejects with one call; recovery turn (already-in-use, then is_error) rejects with two calls; unit test on claudeResultText. Watchdog and claude adapter suites: 53/53.
Not re-run: the live stall walk and positive control. The watchdog and stall path are unchanged since 034df3d, where both passed.

Keep stream text when a turn ends without a result event.

Keep stream text when a turn ends without a result event.
When Claude exits with code 0 and emits no result event, claudeResultText returns an empty string unless a complete assistant message exists. Its fallback reads only assistant blocks, so stream_eventtext_delta content is dropped. Partial-message streaming sends those deltas before the complete message, so a turn that ends after deltas but before the complete message loses all of its text.
Use the stream_event text deltas as a fallback only when the assistant text is empty. This avoids duplicating text from completed messages. Add a regression test for stream-json output that contains only stream_event text deltas, with no result or assistant event. Assert that the helper returns the concatenated delta text.

Retain text deltas in nonzero-exit failure details.
Claude stream-json output can contain text in stream_event.event.delta.text. When the process exits nonzero before a result event, claudeFailureTail filters out that event. If stderr is empty, runClaude loses the failure detail. The classifier then can treat a quota failure as RUNTIME instead of opening the quota circuit. This requires a failure-tail fix, not the successful-turn claudeResultText fallback.

Update claudeFailureTail to retain text deltas from stream_event events when no
result text is available. Extract text from content_block_delta events with
text_delta payloads and include it alongside the existing non-system, non-stream
output, preserving the current result-text preference.

Keep errors text when an error result has no result field.
523-524: Minor · Stability & Availability · Quick win
Keep errors text when an error result has no result field.
When an is_error result has no result text, reply is empty. adapterFailure then builds an error with the message claude. It has no status and no detail. classifySpawnFailure reads only that message, so the error falls through to the RUNTIME class. RUNTIME gets two quick retries and then opens the circuit. A rate-limit diagnostic in errors can therefore be lost. This depends on stream-json error results that carry errors and no result. The inspected sources do not establish that shape for stream-json. Use the errors text, then subtype, as the fallback message.

Cover credential redaction on the is_error result paths.
Neither new test passes runtimeToken, and neither error result echoes a credential. Removing the credential argument from the is_error path would leave both tests passing. Add a test for each turn type. Each test should echo the seat token in the error result and assert that the rejection excludes it.
The initial-turn test at lines 180-185 does not pass runtimeToken. The second is_error test, at line 194 (529 overloaded), has the same gap. Both cover the initial turn and the recovery turn, so the same assertion should apply to each.
