feat(cli): supervise codex turns by default (TASK-708) - #3088
Codex turns now run under the seats supervisor by default (D211). A turn that makes no progress is named and killed instead of holding the seat. Claude and other adapters keep today's rule.
`superviseRunsByDefault(adapterName, env)` in `cli/src/commands/agent.js`: codex is on unless `COMMONLY_SUPERVISE_RUNS` is exactly `0`; other adapters need `1`.
An explicit `superviseRuns` option still wins over the default.
Tests: decision table (codex/claude/stub × unset/0/1/true), plus the real codex adapter run with the flag unset and with `=1`, asserting the turn line says `path: supervised`. The existing flag-off golden is unchanged.
Full cli suite: 2141 pass. 3 fail in `adapters.claude.environment` and `mention-event-types.contract`; the same 3 fail on the unmodified base.
Refs TASK-708.

…PERVISE_RUNS=0 (TASK-708)
A codex turn that stops making progress is now named and killed instead of
holding the seat (D211). The default comes from superviseRunsByDefault: codex
is on unless COMMONLY_SUPERVISE_RUNS is exactly '0'; every other adapter stays
on only with '1'. An explicit superviseRuns option still wins.
Tests: the decision table covers codex and claude with unset, '0' and '1';
the real codex adapter run passes with the flag unset and with '=1', and
the turn line reads path supervised.

Supervision now defaults on for Codex unless COMMONLY_SUPERVISE_RUNS=0. Other adapters require COMMONLY_SUPERVISE_RUNS=1. An explicit superviseRuns value remains authoritative. Codex spawn also checks for cancellation before starting a process.
Changes: Run supervision defaults

Summary
Resolve and verify supervision defaults cli/src/commands/agent.js, cli/src/lib/supervised-turn.js, cli/__tests__/supervise-runs-flag.test.mjs
superviseRunsByDefault applies the adapter and environment rules. performRun passes the resolved value to the turn runner when no explicit value is provided. Documentation and tests cover these rules and real-Codex runs.
Codex cancellation check

codex.spawn checks whether the signal is aborted before calling runCodex. The test verifies rejection with the abort reason and no child-process spawn call.

The public-state test at cli/__tests__/attach.test.mjs:1000 only calls bootstrapAgentRecordFromEnv and checks the sandbox record. It does not execute performAttach or performRun, and it does not assert the supervision behavior changed by this PR. Add a first-use public attach test that asserts the prior observable run behavior.

performAttach or performRun rather than only bootstrapAgentRecordFromEnv, and

outcomes. Keep the test focused on the unset-flag case and public attach flow.
The resolver table does not exercise performRun. Add a Codex run with COMMONLY_SUPERVISE_RUNS='0' and assert the plain path. A mutation that makes performRun ignore the Codex resolver would still pass the resolver test and the current supervised Codex cases.
This is a test-coverage improvement, not a production defect.

Prevent Codex from spawning after cancellation.
When a short configured stall window expires during Codex preparation, the supervisor can settle and retry before codex.spawn creates its child. The Codex adapter does not check signal before calling runCodex, and a child reported after settlement is outside the supervisor's killable set. It can remain running during the retry.
Add an abort check immediately before process creation and kill children reported after settlement. The default 15-minute stall window makes this less likely during normal preparation, but does not prevent the race when operators configure a shorter window.

…SK-708)
A supervised codex turn can stall during preparation, settle, and retry
while the first attempt is still awaiting setup. The attempt then reached
runCodex and created a child the supervisor had already given up on.
Check the signal synchronously right before runCodex, and cover the
already-aborted case with a test that asserts no spawn happens.

Fixed in 9d3c4f1: codex.spawn now checks the abort signal synchronously right before runCodex, so an attempt that stalls during preparation creates no child. A test covers the already-aborted case.
Self-review round (author review, COMMENT). Checked the diff against the TASK-708 done-when list:
superviseRunsByDefault: codex is on unless COMMONLY_SUPERVISE_RUNS is exactly '0'; claude, stub and other adapters need '1'. An explicit superviseRuns option still wins.
Decision table covers codex/claude/stub x unset/0/1/true/empty, and fails if the adapter check is dropped.
Real codex adapter run passes with the flag unset and with =1; the turn line reads path supervised.
Flag-off golden unchanged and passing.
Fixed since the last round: CodeRabbit's open thread on codex.spawn (discussion_r4433242141). The spawn checks the abort signal synchronously right before runCodex, so a stalled preparation creates no child. Test: an already-aborted signal spawns nothing (adapters.codex.test.mjs).
Ran: adapters.codex.test.mjs and supervise-runs-flag.test.mjs, 71/71 passing on this head.
Not run: the live stall walk on this head. The 3 pre-existing cli failures on main (adapters.claude.environment, mention-event-types.contract) are unchanged by this PR.
Cover cancellation during asynchronous preparation.
This test aborts the controller before codex.spawn() starts. It does not cover a signal that becomes aborted while the preparation awaits are pending.
Add a test that pauses one preparation await, aborts the controller, releases the await, and asserts the abort reason and zero _spawnImpl calls. This protects the race described by the change.
As per path instructions: a test for changed behavior must exercise the changed path and fail when the behavior is reverted.
